Endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy is an established treatment for choledocholithiasis but may rarely lead to pyogenic liver abscess (PLA). We report the case of a 72-year-old woman with a history of biliary stenting who presented with jaundice and cholestasis. ERCP with lithotripsy and sphincterotomy successfully cleared multiple stones, but she developed high fever and leukocytosis within 48 h. Imaging revealed a 6-cm abscess in segment 7 of the liver. Ultrasound-guided percutaneous catheter drainage was performed, yielding purulent material that cultur…

The clinical presentations of 20 patients with four or more choledochal stones were compared with those of 68 patients who had one to three choledochal stones, investigated during the same time period. All patients underwent endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy. Patients with multiple choledochal stones usually presented with insidious onset of painless jaundice, simulating malignant bile duct obstruction, in contrast to the abrupt onset of cholangitis or pain experienced by patients with one to three stones. The latter patients had an increased n…

Sphincteroplasty is an inferior partial sphincterotomy followed by suture approximation of both the duodenal and common bile duct walls. We have described our operative technic employed in 770 sphincteroplasties performed in the past 14 years.

Non-operative methods to treat obstructive jaundice and cholangitis caused by cholelithiasis are endoscopic and percutaneous biliary drainage. Usually, endoscopic papillotomy and stone removal by basket catheter is available for treating choledocholithiasis. It is necessary to place a drainage catheter into the bile duct in patients who have severe cholangitis or remaining stones. On the other hand, percutaneous biliary drainage is useful in case of emergency and in patients reconstructed by the Billroth II method. Biliary drainage is necessary to treat severe cholangitis as soon as possible,

BACKGROUND: The most common adverse event of endoscopic retrograde cholangiopancreatography is pancreatitis. Precut sphincterotomy has been regarded as a risk factor. Some authors have stated that early precut may actually reduce post-ERCP pancreatitis risk. However, early precut as a preventive measure has not been compared to other preventive measures, such as pancreatic duct stent placement.
AIM: To compare the efficacy of early precut sphincterotomy versus pancreatic duct stent placement in high-risk subjects undergoing endoscopic retrograde cholangiopancreatography for the prevention of …

In the modern era of therapeutic endoscopy, pain persisting despite endoscopic sphincterotomy and anatomy that makes the ampulla endoscopically inaccessible prompt referral for surgical transduodenal sphincteroplasty (TS). A retrospective review of sphincter of Oddi dysfunction and pancreas divisum patients who underwent TS for refractory pain in a recent time period was undertaken.

The treatment of biliary lithiasis has changed during the past 20 years. Cholecystectomy remains the gold standard for cholelithiasis, but many options are available for calculi of the common bile duct. Among them are surgical open or laparoscopic choledochotomy, biliary-enteric anastomosis, transduodenal sphincterotomy (TDS), endoscopic sphincterotomy. With the aim to describe the current place of TDS, we reviewed the patients operated on in our department between 1976 and 1992. We found 78 patients with a mean age of 58 years (26-89 years). 34 (43%) of them had acute cholecystitis, with 26 b…

Chronic anal fissure is a painful disorder caused by linear ulcers in the distal anal mucosa. Even though it counts as one of the most common benign anorectal disorders, its precise etiology and pathophysiology remains unclear. Current thinking is that anal fissures are caused by anal trauma and pain, which leads to internal anal sphincter hypertonia. Increased anal basal pressure leads to diminished anodermal blood flow and local ischemia, which delays healing and leads to chronic anal fissure. The current treatment of choice for chronic anal fissure is either lateral internal sphincterotomy …

Pyogenic liver abscess is a potentially life-threatening condition primarily caused by bacterial spread from the gastrointestinal or biliary tract to the liver. As endoscopic retrograde cholangiopancreatography (ERCP) with biliary sphincterotomy becomes more common in the treatment of choledocholithiasis, the risk of developing pyogenic liver abscess increases, with Klebsiella pneumoniae continuing to emerge as a more commonly identified pathogen in North America. Our case describes this unique presentation of a 54-year-old female diagnosed with Klebsiella liver abscess (KLA) following multipl…

Several treatments for anal fissure offer different responses and risks for complications. Reversible chemical sphincterotomy with topical nitroglycerin is an alternative to surgical sphincterotomy. The aim of this observational prospective study was to compare the effectiveness of topical nitroglycerin with the conventional medical treatment. Forty-three patients with anal fissure were treated: 22 (16 chronic and 6 acute) received 0.25% nitroglycerin topically, and 21 (16 chronic and 5 acute) received conventional treatment. Nitroglycerin or placebo was administered in two daily applications …

PURPOSE OF REVIEW: Endoscopic retrograde cholangiopancreatography is an advanced endoscopic procedure used mainly for the purpose of biliary intervention. During an ERCP a duodenoscope is used to assess the ampulla of Vater to inject contrast medium into the common bile duct through biliary cannulation. Biliary cannulation can be challenging, with studies showing unsuccessful biliary cannulation with standard technique greater than 10%. Needle-knife fistulotomy is a unique precut sphincterotomy technique that can assist with difficult biliary cannulation.
