A detailed review of the TJUH experience and the published literature on gunshot and stab wounds to the spine has been presented. The following statements are supported. (1) Military (high-velocity) gunshot wounds are distinct entities, and the management of these injuries cannot be carried over to civilian (low-velocity) handgun wounds. (2) Gunshot wounds with a resultant neurologic deficit are much more common than stab wounds and carry a worse prognosis. (3) Spinal infections are rare following a penetrating wound of the spine and a high index of suspicion is needed to detect them. (4) Extr…

OBJECTIVES: Cervical spine disorders are common in the older population. The paravertebral muscles are essential to the support and stabilisation of the cervical spine but have been little studied. The aim was to determine whether pathological changes develop in these muscles in patients with severe cervical spine disease, which, if present, might contribute to the pathogenesis and symptomatology of their disorder.
METHODS: Open biopsies of superficial and deep paravertebral muscles were obtained during the course of surgical procedures to alleviate cervical myelopathy. Most of these patients…

STUDY DESIGN: This case report illustrates a patient presenting with sciatica and diagnosed with epithelioid sarcoma involving the spine.
OBJECTIVES: The treatment of this patient involved multiple mass resections and decompressions of the spinal canal. Radio- and chemotherapy were applied once a clear diagnosis was obtained.
SUMMARY OF BACKGROUND DATA: Epithelioid sarcoma is a rare tumor mainly arising in the extremities. Confusion with a benign inflammatory process are possible. Treatment after histologic diagnosis involves wide resection. To our knowledge, this report represents the first…

Fractures, tumors, and other causes of instability at the cervicothoracic junction pose diagnostic and treatment challenges. The authors report on 23 patients with instability of the cervicothoracic region, which was treated with posterior plate fixation and fusion between the lower cervical and upper thoracic spine. During operation AO reconstruction plates with 8- or 12-mm hole spacing were affixed to the spine using screws in the cervical lateral masses and the thoracic pedicles. Postoperative immobilization consisted of the patient's wearing a simple external brace for 2 months. The follow…

The author draws attention to errors which are still encountered in the treatment of deformities of the spine, and to recent findings. In conservative as well as surgical treatment of scolioses little attention is paid to curvatures in the sagittal plane. Thoracic hypokyphosis is relatively frequent, it has a significant impact on pulmonary functions, it reduces the effectiveness of orthosis and plays also a significant role in the indication for surgery. Conservative treatment involves the wearing of an orthosis, physical exercise and other rehabilitation procedures do not affect the curvatur…

The value of radiography, ultrasound and magnetic resonance imaging (MRI) in detecting skeletal and intraspinal pathology was assessed in infants with anorectal malformations, and the need for spinal MRI examination in this group of patients was evaluated. Twenty-one infants were examined with radiography, ultrasound and MRI of the lower spine. The detection of skeletal and intraspinal abnormalities was compared for the three imaging modalities. Fifteen patients were normal in all three examinations. Radiography showed bony skeletal abnormalities in six children, although sometimes very subtle…

Retropleural thoracotomy was used in 15 patients for the management of ventral thoracic and thoracolumbar spine abnormalities. This approach provides the shortest direct route to the anterior thoracic spine and avoids pleural cavity entry. A detailed description of retropleural thoracotomy and the clinical experiences of 15 patients treated with it are presented.

This article reviews the indications for, and the technique of, posterior operations on the cervical spine. Cervical hemilaminectomy and laminectomy are discussed, including diagnosis, nonoperative treatment, and results and complications of surgery. Laminectomy for other conditions, cervical trauma, and cervical fusion are reviewed also.

Unstable degenerative spondylolisthesis of the cervical spine is very rare. Slip usually occurs at the C3 on C4 or C4 on C5 levels, immediately above a stiff lower cervical spine. There are two clinical patterns: that with neurological involvement causing cervicobrachial pain or myelopathy and that with neck pain alone. The diagnosis can be made by flexion/extension radiography. All of our eight patients had localised fusion, three anterior and five posterior, and all had satisfactory results one to seven years after operation.

Intraspinal synovial cysts are an uncommon but well recognised cause of backache and sciatica, and should be considered in patients, in particular with degenerative spondylolisthesis, who are symptomatic. MRI is the initial investigation of choice. If there is any doubt as to the diagnosis, CT with or without facet joint arthrography is helpful.

This article discusses infectious and inflammatory processes of the cervical spine. Major emphasis is placed on infectious discitis/osteomyelitis and epidural abscess, particularly the epidemiologic, bacteriologic, pathophysiologic, and clinical aspects, as well as the major role played by magnetic resonance imaging and other imaging modalities used in the detection and diagnosis of these processes.

Low back pain (LBP) is one of the most common reasons that patients seek medical attention. Although acute LBP is generally a self-limiting condition, the estimated cost for this health care problem exceeds $8 billion annually. MR accurately depicts both the morphologic as well as biochemical sequelae of disc degeneration. Additionally, MR is superior in its ability to depict disease processes that can present in an indistinguishable fashion. Although multiple mechanisms have been proposed for the possible etiology of disc degeneration, it remains incompletely understood at this time. In addit

Median postoperative survival time averaged 11.5 months. In palliative surgery of the spine, posterior decompression and fixation using CDI permit most patients to retain ambulation without requiring external orthotics. The rate of postoperative complications is reduced by the introduction of CDI.

Rheumatoid arthritis affects the cervical spine in up to 88% of patients with seropositive disease. Neurologic sequelae result either from direct compression by a rheumatoid pannus or from spinal subluxations. Indications for and techniques of neurosurgical intervention are discussed.

In an effort to define better diagnostic and treatment criteria, we reviewed the cases of 50 patients with injuries of the upper dorsal spine from T2-T9, seen between January 1983 and July 1988. The majority of injuries were caused by road traffic accidents, followed by falls from a height, and train accidents. More than 50 per cent had associated injuries, of which four were missed; 50 per cent had neurological deficits, and the majority of these cases had burst fractures and fracture-dislocations. Treatment was non-operative in the majority of cases, with only four cases undergoing anterior …

Between 1965 and 1991, 132 patients with vertebral dislocation following hyperextension injury of the cervical spine were treated at the Spinal Injury Department of the Metropolitan Rehabilitation Centre in Konstancin, Poland. The series accounted for over 25 per cent of patients with hyperextension injuries and for about 7 per cent of all cervical-spine-injured patients treated during these years. These changes are most commonly found in two situations: (1) Injuries of the upper cervical spine. (2) Injuries sustained by individuals with advanced pathological changes in the spine. Such conditi…
