A 40-year-old woman with a history of injury to the left foot was initially treated for an ankle sprain. When the initial roentgenograms were reassessed, os peroneum fracture with diastasis was diagnosed. This may be the second case of fracture of the uncommon anomaly to be reported in the literature. A fracture of the os peroneum may be confused with ankle sprain. Only careful examination of serial roentgenograms may avoid an incorrect diagnosis and further complications.

Twenty-nine wrists of 29 patients were treated with three procedures: distal ulnar resection (Darrach's procedure), distal ulnar recession, or hemiresection-interposition arthroplasty. The indications were pain and limitation of motion associated with primary osteoarthritis of the distal radioulnar joint, derangement after distal forearm bone fracture, Madelung's deformity, and distal radioulnar sprain. The age of the patients averaged 48.3 years. Follow-up averaged 1 year and 9 months. Radiographic evaluations were done preoperatively and postoperatively. Relief of pain was good in the wrists

The authors give an account of their observations of a female patient who was treated for a sprained ankle by an injection of a crystalline suspension of triamcinolone. This therapeutic procedure caused cutaneous and subcutaneous atrophy of the external malleolar region and a strip of atrophy right along the lymph course of the leg. This observation led to bibliographical research and a review of the histological and clinical phenomena characterizing this type of accident. They note that the medico-legal position of the practitioner may be in question.

To study the incidence of fibulocollateral ligament ankle sprains in the young male athlete, a survey of 84 varsity basketball players was done. Seventy percent of the players had a history of an ankle sprain. Eighty percent of those with a positive history had multiple sprains. Most of the injuries were mild, but in 32% of the injuries, the athlete missed more than 2 weeks of play. No medical attention was sought in 55% of the cases. The clinical features of 311 patients with apparently sprained ankles were analysed and compared with radiographs. Measurable swelling was found to be a constant feature of ankle fractures and bruising was very common. The incidence of fractures increased with age. No other physical sign nor the history was useful in predicting fracture in this context. It is concluded that advanced age, bruising and particularly swelling are strong indications for ankle radiography and the absence of swelling is a strong contraindication.

Lateral instability of the ankle in a young athlete is often very disabling. When conservative management of this instability fails, a surgical procedure described by Chrisman and Snook reconstructs both the anterior fibulotalar and fibulocalcaneal ligaments. In eleven patients who were followed for more than two years, this procedure gave predictably good results. Return to full sports activities can be expected for the young athlete.

Strapping with adhesive tape plays a significant part in the prevention of joint injuries, particularly at the ankle and hand, and is important during early management of injury and rehabilitation. Recurrence of injury when the athlete recommences training is less likely when supportive strapping is applied.

Arthrography of the ankle joint is a reasonably accurate and helpful method of delineating the extent of injuries to the lateral ligament. Forty-six arthrograms were obtained following acute injuries, with 26 patients having operative evaluation. Eight false negative results were encountered, all in combination with fairly massive capsular tears. Although ankle arthrography in acute injuries is useful, it is not infallible. An explanation for the large number of false negative results is offered and additional suggestions are made about the technique of examination.
