High-dose thiopental in the treatment of refractory status epilepticus in intensive care unit.

The authors studied prospectively the effects of thiopental anesthesia on seizure control, hemodynamics, and the course of intensive care in 10 patients with refractory status epilepticus. Clinical and electrophysiological seizures were terminated in every patient. Hemodynamically, thiopental was well tolerated, but slow recovery from anesthesia prolonged the need for intensive care.

Failure to recall (but not to remember): pure transient amnesia during nonconvulsive status epilepticus.

We report a patient with a generalized frontal-predominant nonconvulsive status epilepticus without clinically apparent altered consciousness. The patient was examined and EEG performed during and after the episode. Severe retrograde and anterograde amnesia during the seizure, contrasting with a preservation of ongoing memories formation that could be assessed only after its resolution, suggests a transient disconnection of access to stored representations. This unusual memory disorder is both clinically and electrographically dissimilar to other reported cases of transient epileptic amnesia. …

Some issues in non-convulsive status epilepticus in children and adolescents with learning difficulties.

Individuals with learning difficulties commonly suffer from concomitant conditions; epilepsies constitute up to 35%. The prevalence of epileptic syndromes in learning disabled people is less well documented. Non-convulsive status epilepticus (NCSE) occurs with many epileptic syndromes. It can present with insidious or paroxysmal change of neurovegetative, behavioural, cognitive or affective symptoms which can mistakenly be attributed to other causes. NCSE of generalized epilepsy syndromes can show localizing features and NCSE of localization-related epilepsy can show generalized epileptiform d…

Isoflurane in the management of status epilepticus after surgery for lesion around the motor area.

When conventional treatment for status epilepticus fails, general anaesthesia is recommended. We present our experience with isoflurane, an inhalational anaesthetic, in the management of four patients with status epilepticus which occurred soon after surgery for motor area lesion. The seizures were controlled with relatively small concentrations of isoflurane. Hypotension, the only adverse effect of isoflurane, was managed easily with the use of dopamine in physiological saline. Although status epilepticus occurring soon after surgery is transient, it carries a risk of persistent brain damage …

Generalized tonic-clonic status epilepticus in a child with cat-scratch disease and encephalopathy.

Cat-scratch disease occasionally can be associated with encephalopathy and present as status epilepticus. However, the majority of these patients recover without sequelae, as shown by our patient and previous reports. Cat-scratch disease with encephalopathy should be considered in the differential diagnosis of status epilepticus in children.

[Emergency management of convulsions and prevention of status epilepticus with rectal diazepam administered by parents].

The parents of 17 children with a history of convulsions, were shown how to administer liquid diazepam through the rectum using a 1 mL insulin syringe, in order to alleviate a convulsion at home or any where ever this may occur. Eight of the children had suffered from repetitive and/or prolonged non-temperature related convulsions, four had epileptic status and five had suffered from simple fever related convulsions, complicated in four and in a convulsive status in 1; the number of fever-related convulsions ranged between 1 and 6 per child and those non temperature related ranged from one dai

Seizure control via pH manipulation: a phase II double-blind randomised controlled trial of inhaled carbogen as adjunctive treatment of paediatric convulsive status epilepticus (Carbogen for Status Epilepticus in Children Trial (CRESCENT)).

BACKGROUND: Paediatric convulsive status epilepticus is the most common neurological emergency presenting to emergency departments. Risks of resultant neurological morbidity and mortality increase with seizure duration. If the seizure fails to stop within defined time-windows, standard care follows an algorithm of stepwise escalation to more intensive treatments, ultimately resorting to induction of general anaesthesia and ventilation. Additionally, ventilatory support may also be required to treat respiratory depression, a common unwanted effect of treatment. There is strong pre-clinical evid…

New-onset refractory status epilepticus with a hyperferritinemic hyperinflammatory phenotype in Kyasanur Forest Disease.

BACKGROUND: Kyasanur Forest Disease (KFD) is a tick-borne flaviviral infection endemic to India. While neurological manifestations are recognized, new-onset refractory status epilepticus (NORSE) has not been previously reported. We describe NORSE in the setting of severe KFD-associated hyperinflammation and multiorgan dysfunction. CASE PRESENTATION: A 53-year-old previously healthy woman with virologically confirmed KFD developed generalized tonic-clonic seizures that progressed to NORSE during a severe systemic illness. She had profound cytopenias, severe hepatitis, coagulopathy, myocarditis…

Initial experience of bortezomib in cryptogenic new-onset refractory status epilepticus: a case series.

BACKGROUND: Cryptogenic new-onset refractory status epilepticus (cNORSE) is a neurological emergency with limited evidence-based treatment options. Bortezomib, a proteasome inhibitor targeting plasma cells, has emerged as a potential immunomodulatory therapy in refractory autoimmune neurological disorders. It's additional inhibitory effects on interleukins make it a plausible therapeutic option in cNORSE. METHODS: We retrospectively evaluated patients with cNORSE treated with bortezomib at a tertiary epilepsy center between 2021 and 2026. RESULTS: Five patients with cNORSE were identified in…

Peri-Ictal MRI Abnormalities and Risk of Unprovoked Seizures After De Novo Status Epilepticus.

BACKGROUND AND OBJECTIVES: Status epilepticus (SE) is a neurologic emergency with possible long-term sequelae, including the development of epilepsy. We aimed to determine whether peri-ictal MRI abnormalities (PMA) are associated with unprovoked seizures after de novo SE. METHODS: Adults without epilepsy and a first nonhypoxic SE were included from a prospective database of the Christian Doppler University Hospital, Salzburg, Austria. MRI scans performed within 48 hours of SE diagnosis were evaluated for PMA on diffusion-weighted imaging (DWI), T2-weighted fluid-attenuated inversion recovery …

Management of pediatric status epilepticus after hospital arrival in Japan: A nationwide questionnaire survey.

BACKGROUND: Status epilepticus (SE) is a neurological emergency requiring rapid, stepwise treatment. In Japan, the Guidelines for the Treatment of Pediatric Status Epilepticus 2023 (GL2023) were published; however, real-world practice following their publication remains unclear. METHODS: We conducted a nationwide, cross-sectional web-based survey of pediatric neurologists between January and March 2025. One representative from each institution reported institutional practices for the in-hospital management of pediatric SE. Institutions were stratified by annual SE case volume (≥20 vs. ≤19 cas…

Lightweight deep learning model for nonconvulsive status epilepticus diagnosis using EEG time-frequency analysis.

Nonconvulsive Status Epilepticus (NCSE) is a persistent epileptic seizure state whose detection primarily relies on visual EEG inspection. Automated algorithmic detection of NCSE can effectively increase the detection rate, enabling early intervention and improving prognoses. Previous studies are limited by issues such as not being specifically designed for NCSE, complex feature extraction approaches, suboptimal accuracy, and difficulty in deployment on edge devices. This study proposes an algorithm for the automated detection of NCSE, integrating time-frequency analysis, deep learning, and Ex…

Repetitive Transcranial Magnetic Stimulation for Refractory and Super-refractory Status Epilepticus: A Systematic Review.

RATIONALE: Off-label treatments are often considered to treat refractory status epilepticus (RSE) and superrefractory status epilepticus (SRSE). To investigate the efficacy of repetitive transcranial magnetic stimulation (rTMS) as a treatment for (S)RSE, we performed a systematic review. MATERIALS AND METHODS: Cessation of (S)RSE after rTMS was extracted as the primary end point from manuscripts describing patients with (S)RSE treated with rTMS. Data relevant to epilepsy history, (S)RSE type and etiology, prior treatment for (S)RSE, prior duration of (S)RSE, rTMS parameters, number of treatme…

Association between electrographic seizures and better neurological functional outcomes in non-convulsive status epilepticus.

OBJECTIVE: In non-convulsive status epilepticus (NCSE), predicting patient outcomes is crucial in deciding treatment intensity. However, electroencephalograms (EEGs) of NCSE show various patterns, and their relationship with functional outcomes remains unclear. We aimed to examine the relationship between NCSE EEG patterns and functional outcomes. METHODS: We retrospectively collected data from 105 NCSE patients without cardiopulmonary arrest at two centers and compared functional outcomes in NCSE patients with electrographic seizures (ESz) and those with rhythmic/periodic patterns (RPPs) at …
