Orthostatic Hypertension: Clinical Spectrum and Gaps in Knowledge.

PURPOSE OF REVIEW: Orthostatic hypertension (oHT) is increasingly recognized as a clinically relevant but poorly understood disorder of autonomic blood pressure control. This review aims to summarize the current understanding of the clinical spectrum and pathophysiology of oHT, evaluate its prognostic significance, and identify key gaps in knowledge to guide future mechanistic and clinical studies. RECENT FINDINGS: Recent evidence has shifted the perception of oHT from a physiological variant to a condition associated with adverse outcomes. In younger adults, oHT has been linked to incident h

A brainstem maestro conducting the somatic and autonomic motor symphony.

Somatic and sympathetic tones fluctuate together seamlessly across daily behaviors. In this issue of Cell, Zhang et al. describe populations of spinal projecting neurons in the rostral ventromedial medulla (rVMM) that harmonize somatic motor function and sympathetic activation. The coordinated regulation plays a vital role in supporting behaviors associated with various arousal states.

Pediatric cervical sympathetic chain schwannoma with Horner syndrome: a rare case presentation.

Schwannomas are rare, benign, slowly growing neurogenic tumors that originate in peripheral, spinal, or cranial nerves other than the optic and olfactory nerves. In the head and neck, these tumors usually arise from the vagus nerve and the cervical sympathetic chain in the parapharyngeal space. Cervical sympathetic chain schwannomas represent a rare subgroup of schwannomas; fewer than 60 cases have been reported in the literature. These tumors are rarely seen in children and adolescents. Because patients typically present with an asymptomatic neck mass and vague complaints, clinical suspicion

[Sympathetic skin response and multiple sclerosis: descriptive and prospective study, and association with neuroperineal disorders].

OBJECTIVE: Describe the sympathetic skin responses (SSRs), reflecting the sweat activity mediated by the sympathetic nervous system, in patients with multiple sclerosis (MS) followed for urinary disorders and associate SSRs abnormalities with neuroperineal disorders. METHODS: Prospective monocentric study of a population suffering from MS, who received symptom scores (Expanded Disability Status Scale [EDSS], Urinary Symptoms Profile [USP], Neurogenic Bowel Dysfunction [NBD], International Index of Erectile Function [IIEF] and Female Sexual Function Index [FSFI]) and whose SSRs were collected …

Autonomic mechanisms associated with heart rate and vasoconstrictor reserves.

INTRODUCTION: Hemorrhage is accompanied by baroreflex-mediated tachycardia and vasoconstriction. The difference between baseline and maximum responses is defined as the heart rate (HR) and vasoconstrictor 'reserve'. OBJECTIVE: To test the hypothesis that higher HR and vasoconstrictor reserves in subjects with high tolerance (HT) to central hypovolemia is associated with greater reserve for sympathoexcitation and vagal withdrawal compared with low tolerant (LT) subjects. METHODS: R-R intervals (RRI), systolic arterial pressure (SAP), estimated stroke volume, and muscle sympathetic nerve activ

Influences of secretory activities in rat submandibular glands on tissue kallikrein circulating in the blood.

Changes in serum levels of rat tissue kallikrein (rK1) in venous blood were measured, using a newly developed radioimmunoassay, before and after autonomic nerve stimulations of submandibular salivary secretion. rK1 secreted into saliva under these conditions was measured by radioimmunoassay and by enzymic activity assay, using the fluorogenic peptide substrate D-Val-Leu-Arg-7-amino-4-trifluoromethylcoumarin (AFC). Following an overnight fast, serum rK1 concentration was 30-40 ng ml-1. Unilateral electrical stimulation of the submandibular sympathetic nerve supply (at 50 Hz in bursts of 1 s eve

Sympathetic and parasympathetic cardiac control in athletes and nonathletes at rest.

A simple model to characterize sympathetic and parasympathetic effects on heart rate (R) was tested during rest in 10 nonathletes and 8 world-class oarsmen. The model states that R = mnR0, where R0 is the intrinsic cardiac rate, and m and n depend only on sympathetic and parasympathetic activity, respectively. The multipliers, m and n, were determined by dual pharmacological blockade in two sessions under similar conditions, but in one session propranolol and in the other atropine was given first. In agreement with the model, when corrections were made for atropine-induced blood pressure chang

Parasympathetic control of blood circulation and secretion in the nasal mucosa.

The effects of selective parasympathetic nerve activation on the secretory response and the vascular exchange and capacitance sections in the cat nasal mucosa were studied. The vascular events were investigated by measuring the local disappearnce of 125I- and changes in gross pulse rate from 131I- labelled serum albumin as monitored over the nose. A frequency-dependent increase in nasal secretion and local blood content occurred in the range 0.5-12 imp/sec: an increase in disappearance rate was observed at the same time. This indicates that the vascular and secretory responses are activated si

Splenic contraction associated with sudden, transient hypotension and sympathoexcitation in humans.

The spleen is traditionally recognised for its roles in immunity and blood filtration, and it is known to contract and release stored erythrocytes in hypoxic conditions. Recent findings suggest that the spleen may also contribute to cardiovascular regulation. This study investigated whether the spleen contracts during conditions that activate the sympathetic nervous system, including transient hypotension and psychophysiological stress responses. Eighteen healthy volunteers underwent four interventions known to be associated with either unloading of the arterial baroreceptors or sympathoexcita…

Electrocardiographic abnormalities secondary to acute central nervous system injury.

Most patients with acute central nervous system (CNS) lesions, including acute ischemic stroke, intracranial bleeding, traumatic brain or spinal cord injury, exhibit electrocardiographic (ECG) abnormalities. These are generally transient and considered secondary to the neurological condition. The most common neurogenic ECG alterations include ST-segment elevation or depression, T wave inversion, prominent U wave, prolonged QTc interval, sinus bradycardia and atrioventricular block. The primary mechanism involves autonomic nervous system alterations due to damage to the central autonomic networ

Chronic Expression of Interleukin-10 Transgene Modulates Cardiac Sympathetic Ganglion Resulting in Reduced Ventricular Arrhythmia.

The cardiac autonomic nervous system (CANS) is intimately connected to the regulation of electrophysiology and arrhythmogenesis in cardiac s … ambulatory electrocardiography showed that IL-10 overexpression significantly inhibited the cardiac sympathetic nervous system activity and improved heart rate variability. Meanwhile, we observed decreased LSG function as well as prolonged ventricular effective refractory period and suppressed VA after myocardial infarction (MI) in the IL-10 group. In addition, IL-10 overex …

Evaluation of Sympathetic Vasomotor Activity of the Brachial Arteries Using Doppler Ultrasound.

BACKGROUND The sympathetic nervous system is principally accountable for peripheral artery regulation, and its effect is that vasospasm occurs in the medium and large arteries of the extremities, resulting in decreased flow of blood. Because of this information of how the body works, our goal is to create a noninvasive and repeatable self-test model that uses Doppler ultrasound examination. MATERIAL AND METHODS The study was conducted on 31 healthy and active participants who volunteered for the study. Written informed consent was obtained from all participants. The baseline diameter and flow …

Heart rate variability and vagal tone in schizophrenia: A review.

Recent heart rate variability (HRV) research has identified diminished levels of parasympathetic activity among schizophrenia patients. Over … stress response as a result of deficits in parasympathetic activity. Moreover, this diminished parasympathetic nervous system (PNS) response, also known as decreased vagal tone, has been found to relate to increased symptom severity. Although these findings may cause speculation that the observed vagal tone disruption merely results from anxiety produced by the presence of …

Neuron-Glia Crosstalk in the Autonomic Nervous System and Its Possible Role in the Progression of Metabolic Syndrome: A New Hypothesis.

Metabolic syndrome (MS) is characterized by the following physiological alterations: increase in abdominal fat, insulin resistance, high concentration of triglycerides, low levels of HDL, high blood pressure, and a generalized inflammatory state. One of the pathophysiological hallmarks of this syndrome is the presence of neurohumoral activation, which involve autonomic imbalance associated to hyperactivation of the sympathetic nervous system. Indeed, enhanced sympathetic drive has been linked to the development of endothelial dysfunction, hypertension, stroke, myocardial infarct, and obstructi…
