Rupture of the gravid uterus remains one of the most disastrous complications of labour. Eighty cases of uterine rupture managed at Lagos University Teaching Hospital, Lagos over an eight year period were therefore reviewed to evaluate the characteristic of ruptured uterus in Lagos. The incidence of ruptured uterus was 5.01 per 1000 deliveries and unbooked patients accounted for 80%. Spontaneous rupture occurred in 73.8% while traumatic rupture was responsible for 26.3%. Non-native speakers of English are being seen for accent reduction in speech-language pathology clinics although there is little evidence that treatment is effective. Two types of treatment were evaluated. Traditional articulation-based treatment was successful for 2 native Korean speakers in improving English fricatives and affricate production. A second treatment using phonologically based contrasts was also successful for the same sounds with 2 other Korean speakers. Some issues in foreign accent reduction are discussed.

A study of the clinical profile of 59 patients who presented with hepatitis A virus infection showed that dark urine, fatigue, gastrointestinal complaints, and fever were the most common presenting symptoms. A retrospective survey done from 1987 till 1990 revealed that 23 patients bitten by pigs sought medical help at a teaching hospital in Uberlândia, in southeastern Brazil. Most cases (21) were from Uberlândia. The cases were evenly distributed by month and by year; most of them (14/16; 87.5%) occurred between 7.00 a.m. and 7.00 p.m. The male to female ratio was 6.7:1. To determine the clinical significance of the isolation of Nocardia sp. from blood cultures, clinical records of patients with bacteremia due to Nocardia sp. in our hospital from January 1986 to December 1990 were retrospectively reviewed. A prospective survey for congenital malformations at birth, at a teaching hospital, over a period of two and half years on 9405 consecutive single births has shown that prevalence of major congenital malformations in live births was 1.6 per cent and in still births 16.4 per cent. For several months in 1986, an outbreak of Streptococcus viridans pseudobacteremia occurred at a large teaching hospital. All sources of laboratory blood culture contamination were excluded. The Computer Vowel Trainer (CVT) uses linear prediction to estimate and display the shape of the speaker's vocal tract during an utterance. The assessment of the CVT was carried out over a period of three years with 14 profoundly deaf children divided into an experimental and a matched control group. Two main results were obtained: (a) children learning to articulate with the CVT achieve consistently higher articulation scores than those trained with conventional methods; (b) the younger the children, the greater their progress in vowel learning, retention and generalization.

Experience in urinary drug screening at St Vincent's Hospital, Melbourne, was evaluated over a period of 12 months after the introduction of a commercially available thin-layer chromatographic system which expanded the capabilities and improved the efficiency of the service. Specimens from 167 patients were screened when either drug overdose, or drug abuse, or poor compliance with prescribed medications was suspected. Records on 36,062 maternity cases admitted to 12 teaching hospitals throughout Indonesia between 1977 and 1980 were analyzed. A hospital maternal mortality rate of 37.4/10,000 cases (39.0/10,000 live births) was derived that was about ten times higher than rates reported from developed countries in the early seventies.
