Effect of early and late compliance on the effectiveness of acamprosate in the treatment of alcohol dependence.

BACKGROUND: The aim of this study is to assess the influence of early and late compliance of acamprosate on attendance and abstinence duration in the treatment of alcohol dependence. METHODS: Individual patient data of 2,305 patients from 11 randomized controlled trials comparing acamprosate (n = 1,128) with placebo (n = 1,177) were used to predict early and late compliance and to study the effect of early and late compliance on attendance and abstinence duration using regression analysis and structural equation modeling. RESULTS: Early compliance was predicted by baseline motivation to beco

Adrenocortical and pituitary glucocorticoid feedback in abstinent alcohol-dependent women.

BACKGROUND: The long-term ingestion of alcohol diminishes hypothalamic-pituitary-adrenal (HPA) axis reactivity in alcohol-dependent men, potentially altering future relapse risk. Although sex differences in HPA axis functioning are apparent in healthy controls, disruptions in this system have received little attention in alcohol-dependent women. In this study, we assessed the basal secretory profile of adrenocorticotropic hormone (ACTH) and cortisol, adrenocortical sensitivity in both the presence and absence of endogenous corticotropic pituitary activation, and feedback pituitary glucocortico

Review article: the diagnosis and management of alcoholic hepatitis.

BACKGROUND: Alcoholic hepatitis is a severe, cholestatic liver disease occurring in patients with alcohol abuse. Mortality is substantial; however, therapies may improve clinical outcomes. AIM: To provide an updated review of the epidemiology, diagnosis, staging and treatment of alcoholic hepatitis. METHODS: A MEDLINE literature search was performed to identify pertinent articles. Relevant clinical abstracts were also reviewed. RESULTS: Severe alcoholic hepatitis occurs in a small fraction of patients who abuse alcohol. The 28-day mortality ranges from 30% to 50% in most series. Diagnosis i

An ongoing process: a qualitative study of how the alcohol-dependent free themselves of addiction through progressive abstinence.

BACKGROUND: Most people being treated for alcoholism are unable to successfully quit drinking within their treatment programs. In few cases do we know the full picture of how abstinence is achieved in Taiwan. We tracked processes of abstinence in alcohol-dependency disorders, based on study evidence and results. This research explores the process of recovery from the viewpoint of the alcohol-dependent. METHODS: Semi-structured interviews were conducted in two different settings, using purpose sampling, during 2003-2004. The data were analyzed using content analysis. Participants were 32 adult

Five-year outcomes of alcohol-dependent persons treated with motivational enhancement.

OBJECTIVE: The purpose of this study was to determine whether the superior treatment effect of motivational enhancement therapy (MET) previously demonstrated at 6 months was sustained at a 5-year posttreatment follow-up. METHOD: Patients with mild to moderate alcohol dependence had completed a trial in which all patients were assessed, attended a brief feedback session, and were randomized to four sessions of MET, nondirective reflective listening, or no further counseling. The primary drinking outcome was unequivocal heavy drinking (UHD), defined as drinking 10 or more standard drinks on six

The need for substance abuse after-care: longitudinal analysis of Oxford House.

AIMS: There is a need to explore the processes of social support and self-efficacy change over time among individuals in recovery homes, and to assess the extent to which residents remain abstinent, obtain and maintain employment, refrain from criminal activity, and utilize health care systems both while within the and after leaving such settings. DESIGN: Residents were recruited and interviewed at an initial baseline phase and then re-interviewed at three subsequent 4-month intervals. SETTING: Oxford Houses are recovery home residences for individuals with substance abuse and dependence pro

Recovery of hippocampus-related functions in chronic alcoholics during monitored long-term abstinence.

AIMS: The hippocampus (HC) is characterized by high vulnerability to noxious influence, but also by a considerable regenerative potential. Although deficits in HC-related functions are among the most commonly reported cognitive sequelae in alcoholism, little and conflicting information is available concerning regeneration upon abstinence. The present study has been designed to evaluate (i) the frequency of measurable dysfunction in so called HC tests and (ii) its predictive value for risk to relapse in a cohort of 50 severely affected chronic alcoholic patients and (iii) to monitor recovery of

Low serum amylase levels in drinking alcoholics.

BACKGROUND: We have reported that the serum level of amylase, different from other pancreatic enzymes, increases temporarily after abstinence in alcoholics. To elucidate the mechanism of this phenomenon, pancreatic isoamylase, salivary isoamylase, and amylase in urine were measured together with total serum amylase. METHODS: Total serum amylase, pancreatic isoamylase, and salivary isoamylase values were measured in 38 male patients admitted to the National Alcoholism Center, Kurihama Hospital, for alcoholism after abstinence. In an investigation of amylase secretion, amylase in urine was meas

Prospective study of alcohol consumption patterns in relation to symptomatic gallstone disease in men.

Although the association between alcohol intake and gallstone disease has been characterized previously, the relation between alcohol consumption patterns, specific types of alcoholic beverages, and risk for cholelithiasis has not been addressed systematically. These issues were examined prospectively in a cohort of men who were free from symptomatic gallstone disease in 1986 and were followed to 1996. During follow-up, 2.4% of the men reported newly symptomatic gallstones that were diagnosed by ultrasonography or x-ray, or a cholecystectomy. After adjusting for other known or suspected risk f

Haematological abnormalities in early abstinent alcoholics are closely associated with alterations in thrombopoietin and erythropoietin serum profiles.

Numerous reports exist on haematological pathology in alcoholism. However, no data are available regarding a potential involvement of haematopoietic growth factors in the recovery from alcohol-induced haematological abnormalities upon abstinence. Therefore, thrombopoietin (TPO) and erythropoietin (EPO) serum levels along with haematological and other routine laboratory parameters were closely followed in 14 thoroughly characterized male alcoholic patients over one to five months of controlled abstention from alcohol. Haematological changes in these early abstinent alcoholics consisted predomin

Neuropsychological function and psychosocial status of alcohol rehabilitation program residents.

This investigation examined the relationship between changes in psychosocial status and changes in neuropsychological function of clients undergoing treatment for substance use. A sample of 74 adults enrolled at a rehabilitation program for substance use were randomly assigned to two groups (one serving as a comparison group for practice effects). Participants were tested three times over 60 days on self-esteem, motivation, depression, and neuropsychological function. Consistent with other studies, participants demonstrated impaired baseline neuropsychological function that improved with alcoh

Thyroid and adrenal dysfunction in abstinent alcoholic men: locus of disturbance.

Certain neuroendocrine abnormalities (e.g., blunted plasma adrenocorticotropic hormone [ACTH] response to corticotropin-releasing hormone [CRH] administration and blunted serum TSH response to thyrotropin-releasing hormone [TRH] administration) are common in alcoholic patients. It was the objective of this study to evaluate whether they are centrally mediated: that is, whether they are secondary to increased activity of CRH and/or TRH neurons. We evaluated the nocturnal secretion (2200 hours to 1000 hours, q 15 min) of plasma ACTH, serum cortisol, and serum TSH, and their responses to the comb

The principle of stewardship truthful communication for the restoration of trust in public health officials.

In the wake of widespread public skepticism toward health authorities, this paper introduces a stewardship model of communication to guide public health officials in restoring trust. In critically examining the broader ethical concept of stewardship as developed by the World Health Organization and the Nuffield Council on Bioethics, the paper argues that public health officials-as stewards of the population's health and well-being-have an ethical duty to convey scientific knowledge truthfully. Rather than promoting excessive transparency or manipulating public perception through noble lies, th

Human ophthalmomyiasis caused by Oestrus ovis in Bulgaria: Case Report.

Ophthalmomyiasis is an uncommon parasitic infestation of the human eye caused by dipteran larvae, most frequently Oestrus ovis. It is rarely reported in temperate countries such as Bulgaria. We present a case of external ophthalmomyiasis in a 35-year-old urban resident, with no livestock exposure, caused by O. ovis. Larvae were morphologically identified after extraction. Approximately 30 motile larvae were mechanically removed over several days. Microscopy confirmed first-instar O. ovis larvae. Treatment included systemic and topical antibiotics, anti-inflammatory agents, and eye irrigation,

Network analyses of Oppositional Defiant Disorder (ODD) symptoms in children.

Based on parent and teacher ratings of their children, this study used regularized partial correlation network analysis (EBIC glasso) to examine the structure of DSM-5 Oppositional Defiant Disorder (ODD) symptoms. Parent and teachers (N = 934) from the general community in Malaysia completed questionnaires covering DSM-5 ODD symptoms. The most central ODD symptom for parent ratings was anger, followed by argue. For teacher ratings, it was anger, followed by defy. For both parent and teacher ratings, the networks revealed at least medium effect size connections for temper and argue, defy, and a

Primary cutaneous histoplasmosis difficult to treat in immunocompetent patient: case report and literature review.

Histoplasmosis is an infection caused by the dimorphic fungus Histoplasma capsulatum. The disease is endemic in several regions of tropical and temperate climate. The fungus presents opportunistic behavior, causing widespread infection in immunocompromised patients, resulting from complication of primary pulmonary infection, due to exogenous reinfection or reactivation of a quiescent source. In immunocompetent individuals, approximately 95% of pulmonary infections are asymptomatic. However, prolonged exposure to high amount spores may lead to acute or chronic lung infection. Due to the low amo

First record of Haemaphysalis concinna (Acari: Ixodidae) in Lithuania.

The relict tick Haemaphysalis concinna Koch, 1844 (Acari: Ixodidae) has high medical and veterinary importance as it is known as a vector or a carrier of various pathogens causing human and animal diseases. Haemaphysalis concinna is a widely distributed species in forests of temperate Eurasia. In Europe, it is found mainly in deciduous or mixed forests near shores of lakes or rivers. However, there is still a lack of knowledge about current distribution, relative density, and other ecological parameters of H. concinna. On June 30th, 2019, one specimen of H. concinna was found on the fur of a h

Description of the rare localization of Dirofilaria repens in human in the right inguinal lymph node.

D. repens is the causative agent of subcutaneous dirofilariasis in canine animals. In recent years, human invasion by this vector-borne helminthiasis has been recorded in many countries with temperate climate, but the localization of this helminth in the cavities of the human body is extremely rare. Cases of atypical localization of D. repens are described in the organs of the chest, cervical lymph node, in the spermatic cord and epididymis. It is usually requires differential diagnosis with neoplastic or metastatic processes. We report a case of the detection of Dirofilaria repens in the righ
