[Reconstruction of the I carpo-metacarpal joint by transplantation of the II metatarsophalangeal joint on the neuro-vascular pedicle].

In the article are presented the results of treatment of a patient with the consequences of severe damage of hand. As the basic method of treatment there was used free microneurovascular autotransplantation of tissue complexes. The main attention was paid to the problem of reconstruction of the I carpometacarpal joint by transplantation of the II metatarsophalangeal articulation on the neurovascular pedicle. There was achieved a favourable anatomic and functional result. There are indicated the advantages of the given method in case of irreversible destruction of the I carpometacarpal joint.

Rupture of the tibialis posterior tendon.

The clinician should be aware of the possibility of a rupture of the tibialis posterior tendon in adults who have developed a unilateral flatfoot deformity. By careful physical examination, the diagnosis can readily be made. In more advanced cases roentgenographic changes of medial subluxation of the talar head and evidence of sagging of the talonavicular joint are noted on the lateral view. If conservative treatment fails to result in a satisfactory clinical response, I believe the treatment of choice is a flexor digitorum longus transfer through the navicular. If this procedure fails, bony s

Restoration of strong grasp and lateral pinch in tetraplegia due to cervical spinal cord injury.

Patients with tetraplegia who have "strong" sixth cervical neurologic (C-6) function often can be given active grasp and strong lateral pinch by tendon transfers and tenodeses. Wrist control can be retained by the extensor carpi radialis brevis and flexor carpi radialis and can permit transfer of the extensor carpi radialis longus to provide finger flexion. Either the brachioradialis or pronator teres then is available for transfer to restore adduction-opposition of the thumb with an in situ tendon graft of a paralyzed flexor superficialis rerouted to the thumb through a palmar fascial pulley.…

Robotic-Assisted Latissimus Dorsi Transfers Around the Shoulder.

Robotic-assisted surgery has gained prominence in orthopedic practice due to its precision, enhanced visualization, and minimally invasive nature. While robotic applications for orthopedic surgery are well established in arthroplasties, their use in soft-tissue surgery remains limited. This study presents the indications, technique, and clinical outcomes of robotic-assisted latissimus dorsi tendon transfers for shoulder reconstruction. The robotic approach allows accurate identification of the axillary and radial nerves, minimal soft-tissue trauma, reduced bleeding, and excellent cosmetic outc…

Acute fracture of an extensively ossified segment of the Achilles tendon.

Extensive tendon ossification is thought to be rare and is hypothesised to occur due to cell-mediated tissue remodelling. Literature outlini … , however, was unsuccessful. Surgical excision of the calcified fragment and flexor hallucis longus tendon transfer were suggested as a potential solution. This case is compared with and builds on current literature surrounding pathophysiology and optimal treatment of extensively ossified segments of the Achilles tendon.

Median to Radial Nerve Transfer: An 8-Year Experience From a Lower-Middle Income Country.

INTRODUCTION: With an incidence of 2-16%, radial nerve palsy is one of the common forms of nerve injuries globally. Radial nerve palsy causes debilitating effects including loss of elbow extension, wrist drop and loss of finger extension. Reparative surgical pathways range from primary repair and neurolysis, to nerve grafting, nerve transfers, and tendon transfers. Due to ease of performance and acceptability and reproducibility of outcomes, tendon transfers are considered the gold standard of radial nerve palsy repair. However, independent finger function cannot be achieved and as such may no…

Outcome of lower trapezius transfer to reconstruct massive irreparable posterior-superior rotator cuff tear.

BACKGROUND: Management of massive irreparable posterior-superior rotator cuff tear can be very challenging. This study reports the outcome of the lower trapezius transfer to reconstruct massive irreparable posterior-superior rotator cuff tear. METHODS: Included were 33 patients with an average age of 53 years (range, 31-66 years). All patients had symptomatic massive irreparable rotator cuff tear that failed conservative or prior surgical treatment and underwent reconstruction with lower trapezius transfer prolonged by Achilles tendon allograft. The tear was considered irreparable based on th

Arthroscopic-Assisted Latissimus Dorsi Transfer for the Management of Irreparable Rotator Cuff Tears: Short-Term Results.

BACKGROUND: Irreparable rotator cuff tears associated with shoulder functional impairment represent a challenge, especially in young and active patients. Latissimus dorsi muscle-tendon transfer is performed to replace the irreversibly lost contractile elements in patients with irreparable tears of the posterosuperior aspect of the rotator cuff. METHODS: From 2008 to 2010, we enrolled twenty-seven patients (mean age, sixty years; range, forty-six to sixty-seven years) with irreparable, full-thickness rotator cuff tears involving at least two tendons who underwent arthroscopic-assisted latissim…

[Static anterior glenohumeral subluxation following coracoid bone block in combination with pectoralis major transfer: a case report and biomechanical considerations].

We report the case of a patient who experienced post-traumatic static anterosuperior subluxation of the shoulder. The displacement worsened after surgical treatment associating a preglenoid bone block and pectoralis major transfer. We studied the lines of action of the conjoint tendon on an anatomic model. The biomechanical study demonstrated that alone, the coracoid bone block did not provoke the anterior glenohumeral subluxation. On the contrary, the compression components of the humeral head into the glenoid cavity increased and could not explain the worsening dislocation. Inversely, the te

Free composite groin flap and vascularized external oblique aponeurosis for traumatic avulsion injuries of the foot.

Avulsion trauma to the dorsum of the foot is usually combined with skin and tendon loss. Five patients with such composite tissue loss were treated with composite free groin flaps including the external oblique aponeurosis based on the superficial circumflex iliac vessels. The groin flaps resurfaced the skin defect and the vascularized fascia was used to replace the absent extensor tendons in a single-stage reconstruction. All the composite free tissue transfers were successful. When compared with conventional treatments, the advantages of this technique are one-stage reconstruction, faster wo

Lengthening of the lateral column and reconstruction of the medial soft tissue for treatment of acquired flatfoot deformity associated with insufficiency of the posterior tibial tendon.

We analyzed our results of surgery for acquired flatfoot deformity after dysfunction of the posterior tibial tendon. This included lengthening the proximal lateral column by calcaneal osteotomy and reconstructing the medial soft tissue. Nineteen patients (9 women and 10 men; average age, 52.9 years [range, 24-72 years]) were treated for stage II and stage II-III insufficiency of the posterior tibial tendon. The medial soft tissue surgery included 18 reconstructions of the tendon, 11 transfers of the flexor digitorum longus tendon, 13 repairs of the deltoid ligament, and 3 repairs of the spring

Quadriceps tendon reconstruction using a fascia lata included in a reverse-flow anterolateral thigh flap.

Quadriceps tendon re-rupture after surgical repair is an overall estimated 2% complication. We report a case of reconstruction in a large tendon and soft tissue defect using a reverse-flow anterolateral thigh (ALT) perforator flap including fascia lata in a 75-year-old man presented with septic necrosis of a reconstructed quadriceps tendon. A reverse-flow ALT flap was transferred to the knee defect; the fascia lata was sutured to the residual tendon. Post-operative flap congestion and infection were successfully treated with debridement and conservative treatment. One year after surgery, the p

[Progress in soft tissue reconstruction of adult-acquired flatfoot deformity].

OBJECTIVE: To review the progress in clinical and biomechanical study on soft tissue reconstruction of adult-acquired flatfoot deformity (AAFD). METHODS: The recent original articles of soft tissue repair and tendon transfer for AAFD were extensively reviewed. RESULTS: The soft tissue procedures for AAFD can be divided into two components: static restoration of medial column stability and dynamic reconstruction of the posterior tibial tendon. The most important static structure to be repaired for AAFD is the spring ligament. On the other hand, various methods can be used for dynamic reconstr…

Latissimus dorsi transfer for treatment of irreparable rotator cuff tears.

Massive rotator cuff tendon ruptures are not uncommon in older patients. We propose the transfer of the latissimus dorsi muscle for treatment of irreparable ruptures associated with functional impairment and chronic pain. Five women and 11 men were so treated and reviewed with an average follow-up of 26 months. The mean age was 60 years. Four patients also had subscapularis deficiency. Results were assessed with the Constant score and the Oxford shoulder score. Humeral head position was analysed. Statistical analysis was performed by the Wilcoxon non-parametrical test. The Constant score incre
