[Traumatic and degenerative tendon lesions of the hand].

Tendon lesions are the second most common injury in the hand and therefore an important factor in orthopedic patients. Most injuries are open injuries to the flexor or extensor tendons; nevertheless, also less frequent injuries such as damage to the functional system of tendon sheath and pulley or dull avulsions need to be considered. Besides the clinical examination, ultrasound and MRI have proven to be important diagnostic tools. In the postoperative course of flexor tendon injuries, the principle of early passive movement is important to trigger "intrinsic" tendon healing to guarantee a goo…

Distal biceps tendon tears in women.

HYPOTHESIS: Is the presentation and outcome of surgical treatment of distal biceps tendon tears different in women than men? MATERIALS AND METHODS: From 1999 to 2008, 15 cases of distal biceps tendon tears in 13 female patients were treated surgically at a single institution. Mean age was 63 years (range, 48-79 years). A retrospective review evaluated patient presentation, diagnosis, and treatment. Postoperative outcomes were assessed by physical examination, a patient satisfaction survey, the American Shoulder and Elbow Surgeons (ASES) elbow assessment form, and the Disabilities of the Arm,

Disorder of the long head of the biceps tendon.

This paper focuses on biceps injuries in shoulder surgery. This topic is currently of great interest, and there has been much discussion about diagnosis, treatment and prevention. Biceps tendon pathology exists in many forms and is frequently associated with other shoulder disorders. These facts must be taken into consideration in the evaluation and treatment of patients with long head of the biceps pathology. There is much controversy about the long head of the biceps tendon (LHBT). The clinical significance to shoulder function, diagnosis and treatment are still a source of debate. The purpo

Arthroscopic visualization of subscapularis tendon lesions.

Certain lesions of the subscapularis tendon are difficult to identify during routine shoulder arthroscopy due to the fact that complete visualization of the tendon's insertion on the lesser tuberosity can be challenging. This article describes a maneuver used during shoulder arthroscopy that provides excellent visualization of the subscapularis tendon insertion on the proximal humerus. This technique is advantageous because (1) it allows for complete visualization of the intra-articular tendinous insertion of the subscapularis using a standard 30 degrees arthroscope inserted through the standa

The proximal biceps tendon: tricks and pearls.

The diagnosis and treatment of proximal biceps tendon injuries continue to be a challenge. The difficulty lies on determining if there is isolated biceps pathology versus concomitant rotator cuff tears or instability. Imaging modalities, such as magnetic resonance imaging, continue to provide us with the extra tool to help us confirm our suspicion of additional pathology. Symptomatic biceps tendon tears can undergo debridement, tenotomy, or tenodesis if nonoperative measures fail to provide relief. Reports from performing a biceps tenotomy often give similar functional outcomes compared with t

The results of arthroscopic subscapularis tendon repairs.

PURPOSE: The purpose of this study was to evaluate patients after an arthroscopic subscapularis tendon repair to determine their intermediate-term results. METHODS: The records of all patients who underwent an arthroscopic rotator cuff repair by the senior author (S.S.B.) between January 1, 2000, and December 31, 2003, were reviewed. Patients were excluded from the study if their rotator cuff was repaired but there was not a subscapularis tear. Forty patients were included in this study. The median follow-up from the date of surgery to the last clinical evaluation was 5.0 years (range, 3.2 to

Partial tear of the quadriceps tendon in a child.

We present a case of partial rupture of the quadriceps tendon in an 8-year-old girl. This is one of the youngest patients reported with a quadriceps tendon rupture, an entity seen predominantly in middle-aged people. The strength of the muscle tendon unit in a child makes tendon injuries extremely unusual as compared to apophyseal avulsions. The MR imaging findings of this unusual pediatric injury are illustrated.

Proximal biceps tendon: injuries and management.

The long head of the biceps tendon is a known pain generator of the shoulder. There are numerous pathologic entities that may affect this tendon, including tendonitis, partial tearing, and subluxation. These conditions are often associated with rotator cuff tears, especially those involving the subscapularis. Operative interventions include tenotomy and tenodesis. Tenodesis can be preformed in a proximal or distal location. Subpectoral tenodesis may have a lower recurrence rate than proximal-based techniques.

[Fluoroquinolone-induced Achilles tendon rupture].

HISTORY AND ADMISSION FINDINGS: A 72-year-old female dialysis patient with insulin-dependent diabetes mellitus who was under long-term medication with oral prednisolone due to chronic obstructive pulmonary disease was given levofloxacin for one week to treat an acute bronchitis (one 500 mg dose on the first day, 125 mg/day orally from second day onwards). One day after the end of levofloxacin treatment, the patient complained about a constant dragging pain above the right heel that receded under local application of diclofenac ointment and inactivity of the right foot. Twelve days after ending

Management of insertional tendinopathy of the Achilles tendon.

Posterior heel pain is common and disabling. Most cases respond to nonoperative treatment. The literature is confusing about the treatment rationale because many papers treat a variety of pathologies in the same way on an empirical basis. The authors critically review the literature with special reference to surgical treatment. The key to successful management of posterior heel pain is a proper understanding of the anatomy and pathological processes. Only then can appropriate treatment be recommended and proper advice about recovery times be offered to patients.

Acute and chronic peroneal tendon dislocations.

Peroneal tendon dislocation is an infrequent sports injury and can be difficult to diagnose. Posterior fibula tenderness and pain with eversion are useful signs to discriminate this from an ankle sprain. In the acute setting, a direct repair provides good results but may need to be augmented with additional soft tissue or bony restraints. For chronic injuries there are several methods of reconstruction, all with acceptable outcomes, although bone block procedures have the highest rate of secondary procedures.

[Management of isolated subscapularis tendon tears].

Isolated subscapularis tendon tears are rare and often of traumatic origin. Despite specific clinical tests and exact radiological tools (ultrasound, magnetic resonance imaging), these tendon ruptures are often overlooked. For restoration of normal biomechanical joint function, immediate operative reconstruction is recommended. Tears of the upper subscapularis and partial articular-side tendon can be safely managed arthroscopically. The biceps tendon is often involved, and adequate treatment with tenotomy or tenodesis must be given to prevent postoperative pain. Complete subscapularis tendon a

Gliding resistance of the posterior tibial tendon.

BACKGROUND: Abnormal gliding of the posterior tibial tendon may lead to mechanical trauma, degeneration, and eventually posterior tibial tendon dysfunction. Our study analyzed the gliding resistance of the posterior tibial tendon in intact feet and in feet with simulated flatfoot deformity. METHODS: An experimental system was developed that allowed direct measurement of gliding resistance at the tendon-sheath interface. Seven normal fresh-frozen cadaver foot specimens were studied, and gliding resistance between the posterior tibial tendon and sheath was measured. The effects of ankle and hin

Neglected tendon and nerve injuries of the hand.

We retrospectively reviewed 445 patients with neglected tendon and nerve injuries treated between 1983 and 2003. Of these patients, 355 patients (447 tendons) had flexor tendon injuries, 62 patients (84 tendons) had extensor tendon injuries, and 28 patients had isolated nerve injuries. (In addition to these, 48 patients had nerve injuries associated with flexor tendon injuries; making a total of 76 patients with nerve injuries.) Most of the patients presented for treatment from 2 months to 2 years after the injury. The majority (73%) of patients with flexor tendon injuries were treated by free…
