Cricotracheal resection in children 2 years of age and younger.

OBJECTIVES: We examine the surgical outcomes of cricotracheal resection in children 2 years of age and younger. METHODS: We performed a retrospective case study involving a single tertiary care children's hospital. All patients who underwent cricotracheal resection from 1993 through January 2006 were included. Patients 2 years old and younger were compared to patients more than 2 years of age (range, 2 to 44 years). The primary outcomes measured were decannulation and complication rates. We used chi2 analyses for categorical variables to detect differences in proportions, Student's t-tests fo

Laser arytenoidectomy in children with bilateral vocal fold immobility.

Bilateral vocal fold immobility in children is a challenging problem because a balance between good airway and voice quality has to be achieved. Surgery to improve the airway is often postponed or avoided because of fear of losing the voice. In this study our results of laser arytenoidectomy in children are described. This was a retrospective case notes review at a tertiary level paediatric ENT department. The six patients in this case series ranged from nine to 16 years old at the time of laser arytenoidectomy. Post-operative airway and voice quality were assessed. All children in the series

[Diagnosis and therapy of diseases of the larynx in the history of medicine. Part III. After the invention of laryngoscopy].

BACKGROUND: Laryngology a discipline of its own: When in 1855 Garcia had invented the indirect laryngoscopy and Türck and Czermak had introduced the method in clinical medicine numerous laryngological centres were founded in Austria, Germany, England, and France. FIRST INTERVENTIONS AND SOLUTION OF TECHNICAL PROBLEMS: In 1859 Czermak had already examined 20 patients with laryngeal diseases and demonstrated that it was possible to apply local treatment such as cautery under laryngoscopic view. The surgeon von Bruns in Tübingen (Germany) reported in 1862 on the successful removal of a polyp in

A novel technique in the operative treatment of pyriform sinus carcinoma.

Pyriform sinus carcinoma is a highly malignant carcinoma of the head and neck region as a result of its location and its potential for metastatic spread. Decisions regarding the operative procedure remain very difficult. In this paper, we present a modified partial laryngo-pharyngectomy in which, following extended vertical laryngectomy. the cancerous mass is removed from the involved pyriform sinus and the remaining hypopharynx is reconstructed by using the preserved outer perichondrium of the resected thyroid cartilage. This technique has been applied with adequate success during the last fi

[Pharyngocele].

Pharyngocele is a lateral pharyngeal wall herniation located in the pyriform recess or at the vallecula. It is a rare condition and the patient complains of dysphagia, lateral cervical mass which increases in size with the Valsalva maneuver, and other symptoms of the upper digestive way. The elective diagnostic study is pharyngoesophageal swallow. We present the case report of a patient who had acute respiratory failure due to supraglottic mass occupation. The explorations are discussed and the case report is compared with those few reported in the literature.

We have noticed the association between cancer and laryngocele in only one case. Sleep parameters after surgery for obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a disorder that is frequently treated surgically. Few reports in the literature give objective reports of the effect of such treatment on polysomnographic parameters before and after treatment, although symptomatic improvement is common. This article reviews total sleep time (TST) spent in apnea, number of apneic episodes per hour or sleep apnea index (SAI), and oxygen desaturations in such patients treated with tracheostomy, uvulopalatopharyngoplasty (UPP), and combinations of these. Although improvement is noted in these parameters after UPP, this is not as i

Sulphur dioxide block of laryngeal receptors in rabbits.

In 6 rabbits moving average of activity of superior laryngeal nerve(SLN) increased when pressure in upper airways (Pua) was positive and decreased when it was negative. After SO2 exposure of upper airways SLN activity at Pua = 0 decreased to 40% and was no longer affected by changes in Pua. Activity of 67 fibers of SLN was recorded in 11 rabbits: 35 came from 'pressure' receptors, 27 from 'drive', and 5 from 'flow'. Thirty-three pressure receptors discharged at Pua = 0: 32 increased their firing rate with positive Pua and decreased it with negative Pua, one did the reverse. One pressure recept

Gelfoam paste injection for vocal cord paralysis: temporary rehabilitation of glottic incompetence.

Vocal cord paralysis frequently results in open glottic incompetence with aspiration, an ineffective cough, and poor voice production. Glottic competence can be restored temporarily be injecting the true vocal cord with Gelfoam paste. This clinical use of Gelfoam for temporary rehabilitation is indicated in : 1) situations in which paralysis may be temporary, 2) patients for whom an open operative procedure must be delayed, and 3) circumstances in which it is desirable to determine the effect of vocal cord injection prior to placement of nonabsorbable material. The injection of Gelfoam paste r

Two methods for producing peripheral anosmia in dogs.

Two methods for producing peripheral anosmia in the dog were described. One method involved infusion of 3.5% zinc sulfate into the nasal cavity. An anosmia lasting at least 6 weeks, and, in most cases, for several months, was produced. A second technique, producing short periods of reversible anosmia, was the insertion of a tracheostomy tube with inflatable cuff. Inflation of the cuff prevented the dogs from inhaling air over the olfactory mucosa. Deflation of the cuff restored functional olfaction. The advantages and disadvantages of each technique were discussed.
