Radiomics and deep learning for immune checkpoint inhibitor outcomes: imaging-derived immune phenotyping and clinical translation across solid tumors.

Immune checkpoint inhibitors (ICIs) have transformed the treatment of multiple solid tumors, but durable benefit remains restricted to a subset of patients and conventional size-based imaging does not fully capture immune-related response patterns. Radiomics and deep learning offer a route to convert routine CT, MRI, and PET/CT into quantitative biomarkers of tumor phenotype, whole-body heterogeneity, and temporal treatment response. This narrative review critically synthesizes the evidence across solid tumors with an emphasis on four questions that are often separated in the literature: what

Taming the "death receptor": translating the first-in-class p75NTR modulator LM11A-31 from basic biology, across broad preclinical models, to clinical proof-of-concept.

BACKGROUND: The p75 neurotrophin receptor (p75NTR) is a critical regulator of diverse biological processes. Depending on the cellular context, p75NTR can promote trophic or degenerative signaling, which can influence a broad spectrum of pathological conditions, including neurodegenerative diseases, inflammatory/infectious conditions, and various central and peripheral nervous system injuries. These attributes of p75NTR and its widespread, frequently upregulated expression on affected cells and tissues, make it a compelling therapeutic target. Among various therapeutic targeting strategies, the

Translational Models for Glioblastoma: Revolutionizing Drug Development and Personalized Medicine through Clinical Insights.

Glioblastoma (GBM) remains one of the most aggressive and treatment resistant brain tumors and continues to present major challenges for effective therapeutic development. The failure of numerous late-stage clinical trials highlights the limited predictive value of conventional preclinical models. Although established cell lines, two-dimensional cultures, and animal models have been extensively employed, existing platforms fail to adequately recapitulate the complex tumor microenvironment, blood brain barrier function, and interpatient heterogeneity that drive therapeutic resistance in GBM. Modeling sporadic Alzheimer's disease using brain organoids: Emerging trends and translational opportunities.

Sporadic form of Alzheimer's disease (sAD), remains a complex neurodegenerative disorder with limited translational models. Brain organoids derived from human induced pluripotent stem cells (hiPSCs) have emerged as promising tools to recapitulate aspects of human brain development and pathology. Recent advances have introduced vascularized, immune-competent organoids capable of modeling hallmark features of sAD, including amyloid-β accumulation, tau pathology, and neuroinflammation. New strategies to enhance organoid maturation, cellular diversity, and aging phenotypes are pushing the boundari

Reperfusion Injury and Infarct Size: Why Translation has Been Difficult, and How We Move Forward.

PURPOSE OF REVIEW: To evaluate the reasons underlying the translational failure of cardioprotection in reperfused ST-elevation myocardial infarction (STEMI) and propose a framework for designing future clinical cardioprotection trials. RECENT FINDINGS: The 2024 JACC Scientific Statement on reperfusion injury reframed the field as a network of interrelated injury pathways but stopped short of providing a clinically actionable framework. Contemporary cardioprotection trials in enriched patient STEMI cohorts have produced largely neutral results: STEMI-DTU, PiCSO-AMI-I, EURO-ICE, and COOL AMI EU

New approach methodologies (NAMs) for preclinical and translational evaluation of mRNA-lipid nanoparticle (LNP) therapeutics.

Messenger RNA-lipid nanoparticle (mRNA-LNP) therapeutics have emerged as a versatile drug modality, enabling in vivo protein expression for vaccines, cancer immunotherapy, and the treatment of genetic and metabolic diseases. Although mRNA-LNP platforms achieved rapid clinical success during the COVID-19 pandemic, most candidates fail to progress beyond preclinical development, largely due to the limited capacity of conventional animal models to predict human-relevant efficacy, safety, inflammation, biodistribution, and population heterogeneity. New approach methodologies (NAMs), encompassing a

Mitochondrial Communication Networks in the Bone Microenvironment: From the Maintenance of Homeostasis to Translational Interventions for Bone Diseases.

Intercellular mitochondrial transfer has recently emerged as an important concept in bone biology, providing a new framework for understanding immune-metabolic cross-talk within the bone microenvironment. This microenvironment is a dynamic system that is both metabolically active and immunologically complex, and its homeostasis relies on finely tuned communication among multiple cellular populations. Increasing evidence suggests that mitochondrial transfer is a key mechanism integrating these diverse signaling networks. In this review, we systematically summarize recent advances in mitochondri

Single-cell technologies in deciphering drug resistance of multiple myeloma: mechanistic insights and clinical translation prospects.

The greatest hindrance to long-lasting remission in multiple myeloma (MM) is drug resistance and is modulated by the complex interaction of tumor-intrinsic plasticity and microenvironmental co-evolution. Multi-omics of single cells has transformed the exploration of this complexity, and has solved the dynamics of clonal architecture and ecosystem at an unprecedented resolution. The present review summarizes critical relapse-driver revealed by single cell technologies, including existing resistant subclones and non-genetic programs, as well as inflammatory stromal remodeling and immune exhausti

From Dish to Trial: Building Translational Models of ALS.

Amyotrophic lateral sclerosis (ALS) is the most common motor neuron disease, marked by progressive degeneration of upper and lower motor neurons. Clinically, genetically, and pathologically heterogeneous, ALS poses a major challenge for disease modeling and therapeutic translation. Over the past two decades, induced pluripotent stem cells (iPSCs) have reshaped our understanding of ALS pathogenesis and emerged as a promising translational platform for therapy development. ALS modeling has further expanded with the advent of three-dimensional systems, including ALS-on-chip platforms and organoid

Single-cell and single-nucleus transcriptomics of the dorsal root ganglion in neuropathic pain: cell-state remodeling and translational prospects.

Neuropathic pain remains a major unmet clinical challenge, as current therapies provide limited efficacy and poor tolerability. A major obstacle to developing effective analgesics is the pronounced cellular and molecular heterogeneity of the dorsal root ganglion (DRG), which integrates neuronal, glial, immune, and stromal responses to injury. Recent advances in single-cell RNA sequencing (scRNA-seq) and single-nucleus RNA sequencing (snRNA-seq) have enabled cell-type-resolved analyses of these responses and revealed disease-associated cellular states that are obscured by bulk tissue profiling,

Targeting E3 ubiquitin ligases: Mechanistic breakthroughs and novel clinical translation pathways for tumor radioimmunotherapy.

The combination of radiation therapy and immunotherapy has become a cornerstone of modern clinical cancer treatment. However, the inherent radiation resistance of tumors and complex immune evasion mechanisms remain major bottlenecks limiting their long-term effects and sustained efficacy. E3 ubiquitin ligases critically influence tumor sensitivity to radioimmunotherapy by controlling protein stability across DNA repair, immune signaling, and stress-response pathways. This review systematically dissects the multidimensional molecular network through which E3 ubiquitin ligases regulate radiosens

Translational study of targeting Dynamin-1 and podocyte actin cytoskeleton to reduce proteinuria in chronic kidney disease.

Essential for podocyte function in the kidneys, Dynamin-1 (Dyn-1) is a protein composed of four domains. GTP binding and hydrolysis, necessary for membrane fission during clathrin-mediated endocytosis, are handled by its GTP hydrolase domain. Dyn-1 polymerizes into helical structures encircling vesicle necks during GTP hydrolysis, thereby facilitating membrane fission and cargo release, crucial for cell signaling and nutrient absorption. In podocytes, Dyn-1 regulates actin dynamics through interactions with actin-binding proteins, maintaining maintenance of actin cytoskeleton and cellular arch

Implementing Evidence to Inform Recovery After Cardiac Surgery via Median Sternotomy Is Feasible and Effective Using a Research Translation Framework.

BACKGROUND: After cardiac surgery including median sternotomy, patients are often advised to restrict upper limb activity, lifting, and weight bearing to reduce the risk of wound dehiscence and sternal instability. "Keep Your Move in the Tube" (KMIT) is an evidence-based strategy developed for patients after a sternotomy for cardiac surgery that promotes early resumption of functional activities using biomechanical principles. To date, there is inconsistency of KMIT implementation to improve patient recovery and outcomes. The application of a knowledge translation approach may facilitate the t
