[Significance of activities of a medical sanitary detachment from complex expedition if the USSR Academy of Sciences (1925-1930) in the development of surgical treatment for osteoarticular tuberculosis in Iakutiia].

The paper shows the value of the activities of a medical sanitary detachment from the complex expedition of the USSR Academy of Sciences (1925-1930) in the development of surgical treatment for bone and joint tuberculosis and its sequelae in adults from Yakutia. A total of 981 operations were made from 1951 to 1994. Priorities were given to timely surgical interventions, including radical reparative spinal and joint operations, by using the Ilizarov apparatus.

Challenges and advancements in the diagnosis of central nervous system tuberculosis.

Central nervous system (CNS) tuberculosis is one of the most severe forms of extrapulmonary TB, often leading to substantial morbidity and mortality owing to its complex clinical presentation and frequent diagnostic delays. Despite global TB control efforts, CNS TB continues to present unique difficulties owing to its nonspecific clinical manifestations, the paucibacillary nature of cerebrospinal fluid (CSF), and the limitations of conventional diagnostic methods. The diagnostic dilemma is further compounded in resource-limited settings where advanced molecular tools and neuroimaging may not b

[A case of pulmonary tuberculosis complicated with multiple bone and joint tuberculosis].

A 29-year-old male complaining of fever and general fatigue was admitted to our hospital. On admission chest X-ray showed infiltrative shadows with cavities in the bilateral lung apical areas, and sputum examination for acid fast bacilli was smear positive, Gaffky 1. He was treated as pulmonary tuberculosis (TB), and chest X-ray findings and sputum examination improved after a few months of treatment with antituberculous chemotherapy (INH, RFP, EB, PZA). However, tuberculous cold abscess appeared in retropharyngeal area, subcutaneous tissue of thoracic cage and retroperitoneal space, and short

Paradoxical reaction of tuberculous vertebral osteomyelitis: a case series.

Paradoxical reactions of tuberculosis (TB) in vertebral osteomyelitis are very rarely reported. We experienced four cases of severe paradoxical reactions in tuberculous vertebral osteomyelitis. Four cases of tuberculous vertebral osteomyelitis were confirmed by an acid-fast bacilli smear or culture. The patients were human immunodeficiency virus negative, and were all initially treated with isoniazid, ethambutol, rifampicin and pyrazinamide. Their symptoms improved with anti-TB drugs. However, after 2-12 weeks, their symptoms had recurred, and spinal magnetic resonance imaging at the time of r

Is there a change in trend towards pyogenous spondylodiscitis compared to tubercular spondylodiscitis in India-A study of percutaneous biopsy evaluation in spondylodiscitis.

BACKGROUND: In Indian subcontinent where tuberculosis is endemic, the spinal infection was thought to be due to mycobacterium tuberculosis in most of the cases. Hence there is a practice of treating these patients with empirical antitubercular treatment. However, recent guidelines advice biopsy and tissue diagnosis before starting antibiotics. OBJECTIVE OF STUDY: Our retrospective study analyses the role of biopsy in establishing the microbiological diagnosis and thus identifying the incidence of pyogenic and tubercular spondylodiscitis presented to a tertiary care centre. MATERIALS AND METH

Pott's Spine: Retrospective Analysis of MRI Scans of 70 Cases.

INTRODUCTION: Tuberculosis is the commonest of the infections world wide and it can affect almost any part of the body, most commonly the thorax. The spine is affected in 50 % of the cases of skeletal tuberculosis. A tuberculous infection of the spine causes a bony destruction and collapse of the vertebrae, with a gibbus deformity, skip lesions, an intervertebral disc involvement, an epidural abscess, a paravertebral abscess and oedema in the soft tissue planes. Magnetic Resonance Imaging (MRI) is the most valuable investigation in the patients with spinal tuberculosis, as it can clearly demon

Deterioration of chest wall depression causing congestive hepatopathy after an open thoracostomy window in a patient with pectus excavatum and tuberculosis empyema.

A 27-year-old man with severe pectus excavatum, dextrocardia and spinal scoliosis underwent thoracoscopic pleural decortication due to failure of 1-month medical treatment for tuberculous empyema. One month after the pleural decortication, he again underwent open thoracostomy window for repetitive pleuro-cutaneous fistula with tuberculosis empyema. He was subsequently referred to our clinic for progressive dyspnea and bilateral leg edema 4 months after the open thoracostomy window. Evaluations revealed deterioration of the chest wall depression and further compression of the inferior vena cava

One-Stage Anterior Approach with Arch Plate to Treat Lumbosacral Tuberculosis.

This article describes the arch plate technique for treating lumbosacral tuberculosis. Lumbosacral tuberculosis often leads to the destruction of anterior vertebral columns and presacral or iliopsoas abscess, which requires an anterior approach to achieve thorough debridement. Due to the complexity of the anatomical structure of lumbosacral spine and the high requirement of fixation stability, a combined posterior approach to perform internal fixation is necessary, which is rather traumatic. On the other hand, most of the current anterior lumbosacral internal fixation systems cannot be applied

[Use of carbon-carbonic implants in surgery of tuberculous spondylitis].

For surgical treatment of spinal tuberculosis, carbon-carbonic implants were used for anterior spondylosis in 29 patients; in 14 of them bone autografts were additionally applied. Carbon-carbonic implants reliably fixed the operated part of the vertebral column, prevented an increase in kyphotic deformity, and, in combined plastic repair, created favorable conditions for consolidation of bone autografts. A bone-carbon block formed in the late postoperative period.

Tuberculosis of the central nervous system: overview of neuroradiological findings.

This article presents the range of manifestations of tuberculosis (TB) of the craniospinal axis. Central nervous system (CNS) infection with Mycobacterium tuberculosis occurs either in a diffuse form as basal exudative leptomeningitis or in a localized form as tuberculoma, abscess, or cerebritis. In addition to an extensive review of computed tomography and magnetic resonance features, the pathogenesis and the relevant clinical setting are discussed. Modern imaging is a cornerstone in the early diagnosis of CNS tuberculosis and may prevent unnecessary morbidity and mortality. Contrast-enhanced

Natural history of Pott's kyphosis.

The natural history of Pott's kyphosis is different from that of other spinal deformities. After healing of the spinal infection, the post-tubercular kyphosis in adults is static but in children variable progression of the kyphosis is seen. The changes occurring in the spine of children, after the healing of the tubercular lesion, are more significant than the changes that occur during the active stage of infection. During growth, there is a decrease in deformity in 44 % of the children, an increase in deformity in 39 % of the children and no change in deformity in 17 % of the children. The cr

Diagnosis and treatment of paediatric tuberculosis: An insight review.

Tuberculosis (TB) is a major public health problem, invading all age groups world-wide. It is an opportunistic infection affecting the individuals alone or with co-infections. Childhood TB is a neglected aspect and a significant health problem in epidemic areas. It constitutes more than 20% of TB incidence. Pediatric TB exists in the shadow of adult TB. The clinicians concentrate on pulmonary manifestation of TB, whereas it is a major problem in both pulmonary and extra-pulmonary infections. The rate of infection with this disease is mostly associated with poverty, social disruption and human
