OBJECTIVES: To determine whether patients with obstructive sleep apnea who undergo uvulopalatopharyngoplasty (UPPP) have a significant incidence of postoperative complications that would justify overnight postoperative observation in the hospital.
STUDY DESIGN: Part 1: review of published medical literature to determine incidence of postoperative complications. Part 2: retrospective review of 117 patients undergoing UPPP with or without additional procedures.
METHODS: A literature search for existing studies describing the postoperative complications after UPPP for obstructive sleep apnea wa

Before the invention of CPAP therapy, the only effective surgical treatment for obstructive sleep apnea syndrome was tracheotomy. Now, surgical approaches mainly focus on two anatomical sites. Each procedure influences either the retropalatal or retrolingual portion of the pharynx. They might be applied individually, synchronously with other procedures, or sequentially with other therapeutic devices. Common methods of velar surgery include uvulopalatopharyngoplasty (UPPP), Laser-assisted uvuloplasty (LAUP), radiofrequency or uvulaflap. These techniques are indicated mainly in patients with mil…

Uvulopalatopharyngoplasty (UPPP) has been acknowledged as the surgical treatment of choice for snoring. More recently, laser-assisted uvulopalatoplasty (LAUP) has emerged as a reliable, safe, less invasive technique in the surgical treatment of snoring. Our experience includes 90 patients treated by means of Nd:YAG laser-assisted uvulopalatoplasty and the results are presented. The value of different factors (age, sex, snoring loudness, pharynx appearance, Müller manoeuver, presence of apnoeic episodes, overweight) in predicting success or failure of laser surgery has been evaluated. Good resu

A surgical cure for adult obstructive sleep apnoea syndrome (OSAS) is an attractive alternative to nasal continuous positive airway pressure, but current research suggests that uvulopalatopharyngoplasty is not effective in all patients. No subgroup of these patients, who might benefit from surgery to the oropharynx, has as yet been identified. In this study we examined the results of tonsillectomy either as an isolated procedure or as part of uvulopalatopharyngoplasty in seven patients, who had tonsillomegaly. In all seven there was a short-term improvement between the pre-operative and post-o

Outpatient uvulopalatoplasty is an inexpensive alternative to laser-assisted uvulopalatoplasty for relief of symptomatic snoring. This single-stage procedure successfully reduced snoring in properly selected patients. Patients reported improvement in most cases, with 55% indicating great improvement or complete resolution. Spouses reported 83% of the patients were significantly improved or resolved. To date we have had no complications, including bleeding, infection, or velopharyngeal insufficiency or stenosis.

Thirty-two patients undergoing uvulopalatopharyngoplasty (UPPP) for snoring have been studied prospectively using objective measurement of snoring levels. A significant reduction was found, especially in the supine posture. The quantitative reduction was small and correlations between subjective and objective changes in snoring volume were weak.

This study was designed to re-examine the same soft palates studied qualitatively in a previous investigation (Kuehn and Kahane, 1990), usin … stant anteriorly and increased in relative proportion to almost 30% of the total tissue area at the uvular base. The functional implications of these findings are discussed.

Even in the absence of sleep apnoea, heavy snoring may be a cause of excessive daytime sleepiness (EDS) and fatigue. The aim of this investigation was to study whether uvulopalatopharyngoplasty (UPPP) is effective in relieving snoring and excessive daytime sleepiness in nonapnoeic snoring patients. UPPP was assessed in 155 nonapnoeic, snoring patients (136 men and 19 women, mean age 45 yrs). Postoperative evaluation was made after 3 months in 105 patients, and after 12 months in 50 patients. Fifty four patients were evaluated after both 3 and 12 months. In 49 patients, a further evaluation was

One of the most important parts of the management of patients with the obstructive sleep apnoea syndrome is the assessment of the level of obstruction in order to allow the appropriate choice of treatment. We have recently developed the technique of sleep nasendoscopy which allows direct visualization of the site of obstruction in the sleeping patient. Having performed over a hundred of these investigations we are able to suggest a grading system for these patients dividing them into five grades. Grade 1 = simple palatal level snoring; grade 2 = single palatal level obstruction; grade 3 = pala

A 71-year-old man was noted to habitually snore loudly at night and have a predisposition to somnolence during the daytime. While dozing during the day, he developed cardiac arrest at the time when snoring stopped, and was resuscitated. By means of a respiration monitor, he was diagnosed as having sleep apnea syndrome (SAS) with a combination of obstructive, central, and mixed type. However, neither respiratory insufficiency nor cardiac insufficiency was observed, and there were no abnormal findings on laboratory tests and bronchoscopy. SAS complicated by cardiac arrest is usually seen in case

This study describes a simple method, based on a movable catheter technique, for use during routine polysomnography to identify the site of obstruction, and this has been applied to 51 patients with suspected sleep apnea. The obstruction was found to be retropalatal in 30, retrolingual in 7, and could not be determined in 14 patients (12 had no sleep apnea, 1 did not sleep, and 1 had central sleep apnea). Twelve of these patients had uvulopalatopharyngoplasty with preoperative and postoperative polysomnograms to determine the site of obstruction. The preoperative obstruction was retropalatal i

In selected patients with obstructive sleep apnea, the uvulopalatopharyngoplasty procedure may be performed to remove excessive tissue in the oropharyngeal airway. This may improve the symptoms of sleep apnea as well as snoring. Tracheostomy may be indicated in patients with obstructive sleep apnea with associated severe medical problems.
