Idiopathic chronic calcific periarthritis in a child.

Calcific periarthritis is a calcium deposition disease of the periarticular tissues. Deposits of calcium from calcific periarthritis can be found in the periarticular tissues of the shoulders, hips, elbows, wrists, and knees. This disease is often the manifestation of another primary process, such as end-stage renal disease, collagen vascular disease, and systemic diseases (eg, diabetes mellitus, rheumatoid arthritis), among others. Furthermore, calcific periarthritis has been linked to certain areas of the body because of pathologic stress related to repetitive motions, microtrauma, and local

Predictive value of coronary artery calcification for osteoporosis in COPD patients: a retrospective AI-based study.

BACKGROUND: Patients with chronic obstructive pulmonary disease (COPD) are at high risk for osteoporosis (OP), yet OP screening remains suboptimal in practice. The "bone-vascular axis" hypothesis suggests shared mechanisms between vascular calcification and bone loss. Coronary artery calcification (CAC), a reliable imaging marker of atherosclerosis, may serve as a surrogate indicator of OP risk in this population. Advances in artificial intelligence (AI) now enable automated CAC quantification from routine chest CT scans, offering a potentially efficient, low-cost strategy for opportunistic OP…

Reoperations after failed aortic valve neocuspidization: Histopathological analysis of explanted leaflet material.

OBJECTIVES: Aortic valve neocuspidization (AVNeo) provides favorable mid-term outcomes, yet the histological mechanisms of valve failure requiring reoperation remain incompletely characterized. We performed histopathological and immunohistochemical analysis of explanted leaflet material from failed AVNeo valves to analyze their modes of failure at the histological level. METHODS: Leaflet specimens explanted during reoperation for failed AVNeo between January 2021 and December 2025 underwent routine processing with hematoxylin and eosin staining. Additional staining for connective-tissue remod

Atraumatic Breast Fat Necrosis Mimicking Malignancy in End-Stage Renal Disease: A Case Report.

BACKGROUND Breast fat necrosis is a benign process in which fat undergoes aseptic saponification by blood and tissue lipase. It commonly occ … ing surgical excision. Histopathological examination revealed extensive fat necrosis and widespread vascular calcification affecting variably sized vessels in a Mönckeberg sclerosis-like pattern. The calcification was severe enough to cause focal complete vascular occlusion with resultant infarction of breast fat. CONCLUSIONS This case highlights a rare and unusual pres …

The relation between cardiovascular calcifications and diffuse idiopathic skeletal hyperostosis.

INTRODUCTION: Diffuse idiopathic skeletal hyperostosis (DISH) has been associated with vascular calcifications, but data are inconclusive. We aimed to assess the association between DISH and calcifications of the coronaries, thoracic aorta, aortic valve, and mitral annulus/valve. METHODS: A total of 3514 male individuals with current or past smoking behavior who underwent chest computed tomography (CT) from the NELSON lung cancer screening trial were included. Calcification volumes were quantified on chest CT using in house developed automatic validated software. DISH presence was assessed vi…

Role of the calcium-sensing receptor in regulating vascular function.

Functional expression of the calcium-sensing receptor (CaSR) in calcitropic tissues, for example, parathyroid glands and kidneys, is important for maintaining Ca2+ homeostasis. It is also established that the CaSR is present in tissues previously thought to be noncalcitropic and this review discusses the role of the CaSR in vascular function, focusing mainly on contractility but also outlining its role in cell proliferation and calcification. Stimulation of the CaSR by extracellular Ca2+ concentration ([Ca2+]o) on perivascular sensory nerves and vascular endothelial cells is associated with va

Gut microbiota-derived TMAO drives the kidney-bone-vascular axis in chronic kidney disease complications.

Chronic kidney disease (CKD) is often accompanied by comorbidities such as skeletal disorders and vascular calcification, which significantly elevate the mortality risk among affected patients. In recent years, dysbiosis of the gut microbiota has emerged as a key pathological factor in the progression of CKD, particularly through the accumulation of its metabolic product, trimethylamine-N-oxide (TMAO). TMAO is produced in the liver via the gut microbiota's metabolism of dietary precursors such as choline, betaine, and L-carnitine. It is primarily excreted via the kidneys, and its levels are ma…

Current Therapy in CKD Patients Can Affect Vitamin K Status.

Chronic kidney disease (CKD) patients have a higher risk of cardiovascular (CVD) morbidity and mortality compared to the general population. The links between CKD and CVD are not fully elucidated but encompass both traditional and uremic-related risk factors. The term CKD-mineral and bone disorder (CKD-MBD) indicates a systemic disorder characterized by abnormal levels of calcium, phosphate, PTH and FGF-23, along with vitamin D deficiency, decreased bone mineral density or altered bone turnover and vascular calcification. A growing body of evidence shows that CKD patients can be affected by su…

Cutaneous calciphylaxis. An underrecognized clinicopathologic entity.

Calciphylaxis (CPX), an uncommon syndrome characterized, in part, by progressive cutaneous vascular calcification, is seen principally in the setting of renal failure-associated hyperparathyroidism and is difficult to distinguish histologically from other microvasculopathies. We assessed histologic specimens from 13 cases of clinicopathologically classic CPX of the skin and reviewed documented histologic findings in the literature. Our series included 7 "early" and 6 "late" lesions (absence or presence of tissue necrosis, respectively). Histologically, early lesions were subtle and almost inap…

Ferroptosis of smooth muscle cells in vascular diseases: from basic principles to clinical translation.

Vascular smooth muscle cells (VSMCs), which form the media layer of blood vessels, play a vital role in vascular homeostasis and remodeling. Dysfunction of VSMCs represents a key pathological basis and an important contributor to vascular diseases. Ferroptosis, an iron-dependent accumulation of lipid hydroperoxides, is a novel form of regulated cell death. VSMC ferroptosis is involved in a range of vascular diseases, such as atherosclerosis, vascular calcification, hypertension, aortic aneurysm, aortic dissection, neointimal hyperplasia, intracranial aneurysm, and pulmonary arterial hypertensi…

Digital gangrene and pneumatosis intestinalis associated with calciphylaxis.

Reports of calciphylaxis or calcific uremic arteriolopathy associated with acral and gastrointestinal involvement are rare. We describe a 36-year-old white woman with end-stage renal disease on hemodialysis after failed kidney and pancreas transplantation who developed dry gangrene of bilateral digits, osteomyelitis, and small bowel ischemia within several months of each presentation. She had multiple débridements of a septic right ankle. Computed tomography angiography showed severe vascular calcification and pneumatosis intestinalis. She underwent intestinal resection for gangrenous small bo…

Castleman's disease of the mediastinum: MR imaging features.

We present the MR imaging features in two cases of mediastinal Castleman's disease of the hyaline-vascular type. T1-weighted MR images showed rounded hilar masses of intermediate or high signal intensity with areas of low signal due to vessels and calcification. On T2-weighted images, the masses were of high signal intensity. Mediastinal masses with the above signal characteristics and vascular signal voids may suggest a diagnosis of Castleman's disease.

The structural consequences of calcium crystal deposition.

Calcium pyrophosphate dihydrate and basic calcium phosphate (BCP) crystals are the most common calcium-containing crystals associated with rheumatic disease. Clinical manifestations of calcium crystal deposition include acute or chronic inflammatory and degenerative arthritides and certain forms of periarthritis. The intra-articular presence of BCP crystals correlates with the degree of radiographic degeneration. Calcium crystal deposition contributes directly to joint degeneration. Vascular calcification is caused by the deposition of calcium hydroxyapatite crystals in the arterial intima. Th…

Cardiac valve calcification: an immutable pathologic finding in chronic kidney disease?

Although less frequent than vascular calcification, cardiac valve calcification (CVC) is a relevant clinical problem affecting about 2%-10% of adults from the general population aged 75 years and older, and is 5- to 10-fold more prevalent in individuals with impaired kidney function. An expanding body of evidence suggests that mineral metabolism abnormalities aside from traditional cardiovascular risk factors are involved in CVC pathogenesis. Nonetheless, very few studies have investigated whether mineral metabolism manipulation impacts CVC. In this issue of the Journal of Nephrology, it is re…

Emerging role for the vitamin D receptor activator (VDRA), paricalcitol, in the treatment of secondary hyperparathyroidism.

BACKGROUND: Secondary hyperparathyroidism (SHPT) is common in chronic kidney disease (CKD) patients. Classically, SHPT is induced by hypocalcemia, hyperphosphatemia, and calcitriol deficiency, that cause not only renal osteodystrophy but also systemic toxicity, particularly cardiovascular disease. OBJECTIVE: Treatment with calcitriol, the active form of vitamin D, reduces serum parathyroid hormone (PTH) levels but may result in both hypercalcemia and hyperphosphatemia, increasing the risk of vascular calcification in CKD. Are the new vitamin D receptor activators (VDRAs) more useful in the tr…

Kidney-bone, bone-kidney, and cell-cell communications in renal osteodystrophy.

The relationship between bone and the kidney in renal osteodystrophy is a complex interplay of kidney to bone connections, bone to kidney co … ment with BMP-7 resulted in normalization of both high and low turnover states. Finally, we discuss vascular calcification as it relates to bone metabolism. We discuss the roles of BMP-7 and 2 other bone regulatory proteins, osteoprotegerin (OPG) and alpha2-HS glycoprotein (AHSG, human fetuin), in the human vasculature and their implications for vascular calcification.

MicroRNA Profiles in Calcified and Healthy Aorta Differ: Therapeutic Impact of miR-145 and miR-378.

Our goal was to elucidate microRNAs (miRNAs) that may repress the excess bone morphogenetic protein (BMP) signaling observed during pathological calcification in the Klotho mouse model of kidney disease. We hypothesized that restoring healthy levels of miRNAs that post-transcriptionally repress osteogenic calcific factors may decrease aortic calcification. Our relative abundance profiles of miRNAs in healthy aorta differ greatly from those in calcified mouse aorta. Many of these miRNAs are predicted to regulate proteins involved in BMP signaling and may control osteogenesis. Two differentially
