Interrupted aortic arch--anatomical features of surgical significance.

Analysis of 26 autopsy specimens with interruption of the aortic arch has led to a clearer understanding of the vital importance of associated cardiovascular abnormalities. In all cases there was an associated patent ductus arteriosus "supporting" the distal systemic circulation. "Proximal" septal defects, 22 ventricular septal defects and 3 aorto-pulmonary windows, were found in all but one specimen. The infundibular component of the ventricular septum was displaced posteriorly and leftward in 9 hearts resulting in significant left ventricular outflow obstruction in 8. In one specimen the inf…

A Clinical Perspective of the Takotsubo Syndrome.

This article provides a contemporary review of the clinical features of the takotsubo syndrome. The author discusses hallmark elements that distinguish this novel acute cardiac condition from the more common acute coronary syndrome. This review includes relevant clinical detail surrounding findings on ECG, biochemical testing, and cardiac imaging and a discussion of complications, including acute decompensated heart failure, arrhythmias, ventricular thrombi, and left ventricular outflow obstruction. The review concludes with discussion of proper treatment, long-term survival, and recurrence.

Safety profile and utility of treadmill exercise in patients with high-gradient hypertrophic cardiomyopathy.

BACKGROUND: Exercise echocardiography in the evaluation of hypertrophic cardiomyopathy (HCM) provides valuable information for risk stratification, selection of optimal treatment, and prognostication. However, HCM patients with left ventricular outflow tract gradients ≥30mm Hg are often excluded from exercise testing because of safety considerations. We examined the safety and utility of exercise testing in patients with high-gradient HCM. METHODS: We evaluated clinical characteristics, hemodynamics, and imaging variables in 499 consecutive patients with HCM who performed 959 exercise tests.

A case report of complex congenital heart disease co-existing with hypertrophic cardiomyopathy.

BACKGROUND: Myocardial abnormalities are sometimes overlooked in congenital heart disease (CHD). The co-existence of hypertrophic cardiomyopathy is so uncommon that it is assumed to be a coincidence rather than an association. CASE SUMMARY: A 24-year-old gentleman, who was previously clinically well following a staged Fontan palliation for single-ventricle CHD, was transferred to our centre following an out-of-hospital cardiac arrest. He had return of spontaneous circulation after a period of cardiopulmonary resuscitation. Initial electrocardiogram showed sinus bradycardia. Computed tomograph

The Taussig-Bing anomaly: long-term results.

OBJECTIVES: The arterial switch operation (ASO) is the method of choice for the Taussig-Bing heart. The aim of the study was to analyse the long-term outcome of correction of the Taussig-Bing heart. METHODS: Between 1986 and 2011, 44 infants, including 18 newborns, underwent an ASO. The staged and the primary approach were used in 9 and 35 patients, respectively. Aortic arch (AA) obstruction (n = 26) and right ventricle outflow tract obstruction (n = 34) were common. The mean age at corrective surgery was 112.9 days; the mean weight was 4.17 kg. RESULTS: There were 1 early and 4 late deaths.

Do alternative approaches work in surgical septal myectomy?

Trans-aortic septal myectomy is the gold standard for septal reduction therapy. This technique has low peri-procedural mortality and excellent long-term survival. Moreover, it relieves the heart failure symptoms and improves the quality of life. Secondary chordal cutting along with septal myectomy has shown to improve the outcome but can potentially cause deterioration of left ventricular function. In patients with relatively thin inter-ventricular septum, abnormalities of mitral valve apparatus may be the main reason for systolic anterior motion and left ventricular outflow tract obstruction.

Accessory mitral valve tissue: anatomical and clinical perspectives.

Mitral valve is a complex cardiac structure composed of several components to work in synchrony to allow blood flow into left ventricle during diastole and not to allow blood flow into left atrium during systole. Accessory mitral valve tissue (AMVT) was defined as existence of any additional part and parcel of valvular structure which has an attachment to normal mitral valve apparatus in left-sided cardiac chambers. AMVT may present itself in different clinical circumstances ranging from a silent clinical course to thromboembolic events, heart failure, left ventricular outflow tract obstructio

Balloon expandable transcatheter heart valves for native mitral valve disease with severe mitral annular calcification.

Patients with mitral annular calcification (MAC) have high surgical risk for mitral valve replacement due to associated comorbidities and technical challenges related to calcium burden, precluding surgery in many patients. Transcatheter mitral valve replacement (TMVR) with the compassionate use of balloon expandable aortic transcatheter heart valves has been used in this clinical scenario. The purpose of this review was to summarize the early experience including successes and failures reported. TMVR might evolve into an acceptable alternative for selected patients with severe MAC who are not

Canadian Cardiovascular Society 2009 Consensus Conference on the management of adults with congenital heart disease: outflow tract obstruction, coarctation of the aorta, tetralogy of Fallot, Ebstein anomaly and Marfan's syndrome.

With advances in pediatric cardiology and cardiac surgery, the population of adults with congenital heart disease (CHD) has increased. In the current era, there are more adults with CHD than children. This population has many unique issues and needs. Since the 2001 Canadian Cardiovascular Society Consensus Conference report on the management of adults with CHD, there have been significant advances in the field of adult CHD. Therefore, new clinical guidelines have been written by Canadian adult CHD physicians in collaboration with an international panel of experts in the field. Part II of the g

Punch-out lesion following regression of a large left ventricular outflow rhabdomyoma.

Abstracts We report the case of a patient with rhabdomyoma of the left ventricular outflow tract, causing severe obstruction at birth. The tumour regressed completely by 6 years of age, leaving a punch-out lesion. The potential for spontaneous regression of these tumours and the formation of a myocardial lesion following rhabdomyoma regression are discussed.

Primary undifferentiated small round cell sarcoma of the right ventricle with rapid recurrence and aggressive clinical presentation.

A middle childhood boy presented with fatigue, pallor and vomiting that had been persistent for half a month and significantly worsened over the past day. On clinical examination, he exhibited tachycardia with a heart rate of 126 bpm. Transthoracic echocardiography, chest CT and cardiac magnetic resonance were performed. A round, dense mass measuring 5.60 cm×5.17 cm was identified in the right ventricle and right ventricular outflow tract, without involving the right atrium, causing obstruction of both the right ventricular inflow and outflow tracts. Subsequently, the patient underwent tricusp

Intermediate results after a modified Yasui procedure with the Lecompte maneuver.

Three patients with ventricular septal defect, obstructed aortic arch, and valvular aortic stenosis underwent a modification of the Yasui procedure (the Norwood type aortic reconstruction, intraventricular rerouting from the left ventricle to the dual arterial valves, and reconstruction of the right ventricular outflow tract by the Lecompte maneuver). With a follow-up of up to 60 months, the outcome is excellent without progressive lesions or reoperation.

Repaired tetralogy of Fallot across adulthood: from right ventricular remodelling to targeted intervention.

Adults with repaired tetralogy of Fallot (rTOF) constitute a rapidly growing adult congenital heart disease population with excellent early survival but substantial late morbidity. Chronic pulmonary regurgitation and/or residual right ventricular outflow tract obstruction drive progressive right ventricular dilatation, dysfunction, dyssynchrony, and myocardial fibrosis, predisposing to heart failure and ventricular arrhythmias. This review summarizes contemporary concepts in the surveillance and management of adults with rTOF, focusing on multimodality imaging, pulmonary valve replacement (PVR

Progression of outflow tract obstruction in the fetus.

Fetal ventricular outflow tract obstruction (OTO) is congenital heart disease with significant potential for progression before birth as a consequence of the unique nature of the fetal circulation. The pattern of evolution depends upon the timing of development, severity of obstruction and the influence of the OTO on the fetal atrioventricular valve and myocardial function. Critical aortic (AS) or pulmonary (PS) valve stenosis, the two most common forms of fetal OTO, may be associated with progressive ventricular and great artery hypoplasia if presenting early in gestation or with normal ventr

Apical transgastric echocardiography: new imaging projections.

New transgastric echocardiographic projections, obtained by monoplane transoesophageal echocardiography are presented. Starting from the transgastric short-axis view, the probe is first advanced 3-5 cm and slightly rotated clockwise. From this projection either a tricuspid valve long-axis or a subcostal-like 4-chamber view is obtained. Advancing the probe 48-55 cm from the incisor teeth and rotating it counter clockwise with maximal anterior flexion, an apical long-axis view is obtained. The left ventricle, including the apex, the left ventricular outflow tract, the aortic valve, the proximal

A rare combination of left ventricular noncompaction, patent ductus arteriosus, and persistent left superior vena cava demonstrated by multidetector computed tomography and echocardiography.

Noncompaction of the ventricular myocardium (NCM) is a disorder characterized by numerous prominent ventricular trabeculations and deep intertrabecular recesses. It may present in an isolated form or in association with other cyanotic heart diseases, obstructions of the ventricular outflow tract, and anomalies of coronary arteries. We report on a rare combination of NCM, patent ductus arteriosus, and persistent left superior vena cava in a 27-year-old man examined via echocardiography and multidetector computed tomography.

The Mini-Ross-Konno procedure.

Combining the Konno aortoventriculoplasty with the Ross procedure is being used increasingly for the management of infants and children with complex left-ventricular outflow tract obstruction (LVOTO) associated with aortic valve disease and/or aortic annular hypoplasia. We have successfully used a modification of the technique, effectively relieving the LVOTO without creating a ventricular septal defect (VSD). This modification we refer to as 'Mini-Ross-Konno' simplified the procedure and provided an optimal and long-lasting relief of the LVOTO.

Modified septal myectomy and repair of mitral valve apparatus for the treatment of hypertrophic cardiomyopathy.

A modified technique is described of septal myectomy using a resectoscope and mitral valve repair to relieve left ventricular outflow tract obstruction and correct mitral regurgitation in a young patient with hypertrophic cardiomyopathy. This modification offers better control over the extent of septal tissue excision and may reduce the incidence of iatrogenic ventricular septal defect. Using this procedure it was possible to correct systolic anterior motion of the anterior mitral leaflet while obviating the need to replace the mitral valve.
