Combined Minimally Invasive Treatment of Delayed Aortoesophageal Fistula Caused by Fishbone.

Aortoesophageal fistula caused by esophageal foreign body is a rare, catastrophic condition. We report a case of delayed aortoesophageal fistula caused by fishbone and associated with severe hematemesis, mediastinal abscess, and esophageal tear. We performed thoracic endovascular aortic repair to control the bleeding and video-assisted thoracoscopic surgery to drain the mediastinal abscess and to repair the esophageal tear. The patient recovered with limited physical and physiologic impairment after systematic treatment. This case highlights the feasibility of combined thoracic endovascular ao

Emphasizing mesenteric blood vessels in laparoscopic colon cancer surgery video images.

This study aims to reduce surgeon burden by emphasising mesenteric blood vessels buried in the mesentery using image processing techniques. We have developed an image processing system that can be connected to a laparoscope. The first step of the method is superficial vessel deletion using an opening process. Next, the mesenteric blood vessels are extracted by multiple regression analysis using hue, saturation, and value color space information in the image. Finally, the enhanced regions are determined by areas obtained by a labeling process. Although examples with both true and false positives

Microinvasive video-assisted thoracoscopic sympathicotomy for primary palmar hyperhidrosis.

BACKGROUND: Although video-assisted thoracoscopic surgery for palmar hyperhidrosis is now widely accepted as the approach of choice, the optimal technique has remained a subject of controversy. We have used 2-mm dual port video-assisted thoracoscopic sympathicotomy for primary palmar hyperhidrosis. This study evaluates the short-term results of the technique. METHODS: A retrospective review was carried out of 45 patients, 20 men and 25 women, with a mean age of 24.2 years. In the period from April 1998 to August 1999, 90 consecutive video-assisted sympathicotomy for primary palmar hyperhidros

Uniportal bilateral video-assisted sequential thoracoscopic extended thymectomy.

Standard video-assisted thoracoscopic surgery has been reported as a minimally invasive approach alternative to sternotomy for management of myasthenia gravis (MG) associated with thymoma or thymic hyperplasia. Uniportal video-thoracoscopy is an evolution of standard multi-portal video-thoracoscopy for management of several thoracic diseases but its role for resecting mediastinal tumor remains under-evaluated. Herein, we describe our experience with bilateral uniportal thoracoscopic sequential extended thymectomy with case and video illustrations.

Video-assisted thoracoscopic thymectomy (VAT-T) with lateral thoracotomy for stage II and III thymoma.

Thymoma has malignant potential and is the most common anterior mediastinal tumor. Video-assisted thoracic surgery (VATS), which is less invasive surgical procedure, is a good option for resecting Masaoka stage I tumors. Whether VATS is appropriate, depends on the surgeon's judgment and accurate imaging diagnosis. We introduce a technique involving a combination of video-assisted thoracoscopic thymectomy (VAT-T) and lateral thoracotomy for stage II and some stage III thymomas that have locally invaded the lung and/or pericardium.

Video-assisted thoracoscopic wedge resection for pulmonary sequestration.

A 60-year-old woman underwent a video-assisted thoracoscopic wedge resection of intralobar pulmonary sequestration instead of a lobotomy because the lesion was localized in the right basal segment. Preoperative 3-dimensional computed tomography was useful for identifying an aberrant artery arising from the thoracic aorta and distributing to the lesion. A successful outcome more than 4 years after the surgery indicates that a wedge resection under video-assisted thoracoscopy may prove to be a therapeutic option for localized pulmonary sequestration.

Surgical resection of extramedullary haematopoiesis in the posterior mediastinum.

Extramedullary haematopoiesis is a rare disease that is usually associated with haematologic disorders such as thalassemia, myelodysplastic syndrome, and hereditary spherocytosis. It frequently occurs in the liver, spleen, and lymph nodes. Rarely, it occurs in the posterior mediastinum. We report the case of a 59-year-old man with lateral posterior mediastinal masses that were incidentally detected during treatment for hereditary spherocytosis. We performed video-assisted thoracic surgery to confirm the diagnosis and differentiate the masses from neurogenic tumours and other posterior mediasti

Video-assisted thoracic resection for intralobar pulmonary sequestration.

We present a case in which video-assisted thoracic resection for intralobar pulmonary sequestration (ILPS) was successfully performed. A 36-year-old woman had repeated pneumonia. Chest computed tomography (CT) showed a round mass in the right lower lobe of the lung. Subsequent three-dimensional CT revealed that a large anomalous artery arising from the descending thoracic aorta was distributing to the posterior basal segment containing the lesion and was draining into the inferior pulmonary vein. The patient was diagnosed with ILPS and underwent surgery. The anomalous artery was divided, and t

Feasibility of video-assisted bilateral neck exploration for patients with primary hyperparathyroidism and failed or discordant localization studies.

PURPOSE: Minimally invasive video-assisted parathyroidectomy (MIVAP) is generally adopted for patients affected by primary hyperparathyroidism (pHPT) with clear preoperative localization. Standard bilateral neck exploration (BNE) is considered the obligate surgery for patients with unlocalized glands. We reviewed our experience of minimally invasive video-assisted BNE in patients with pHPT and negative or discordant localization studies. METHODS: From a prospective series of 576 MIVAP for pHPT, 107 patients (19 males, 88 females; mean age 58 years) with failed localization studies underwent B

Perioperative outcomes of robot-assisted, laparoscopic or thoracoscopic, and open surgery across 13 cancer operations: a single-institution cohort of 36,208 cases.

To compare perioperative outcomes of robot-assisted surgery, video-assisted (laparoscopic or thoracoscopic) surgery and open surgery across 13 oncologic procedure groups, because aggregate technology-level comparison is confounded by procedure mix and by the calendar time at which each specialty adopted robotics. In this single-institution retrospective cohort study, 36,208 index oncologic procedures in 34,785 patients (January 2016 to December 2025) were identified from the institutional clinical data warehouse. The whole cohort and 22 procedure-level pairwise comparisons were adjusted by sta

Robotic Sympathectomy for Hyperhidrosis.

In hyperhidrosis, the body's sweat glands overact. Excessive sweating results from this overactivity, and for many hyperhidrosis patients, managing symptoms can be difficult in day-to-day life. Both surgical and non-surgical types of treatment are available for hyperhidrosis. Surgical treatments include microwave sympathectomy (video-assisted thoracic surgery and robotic). Da Vinci Si and Xi robotic systems are used. This review summarizes the outcomes, complications, advantages, and disadvantages of robotic sympathectomy. We conducted a literature search using PubMed, Cochrane, and Scopus. Af

Thoracoscopic mesh implantation as a definitive treatment approach for peritoneal dialysis-associated hydrothorax.

Pleuroperitoneal leakage with the formation of hydrothorax is a rare complication of peritoneal dialysis, usually necessitating termination of peritoneal dialysis. We hypothesized that implantation of a polypropylene mesh on the diaphragm using video-assisted thoracoscopic surgery might induce permanent closure of pleuroperitoneal leakage. We report a case series of n = 12 peritoneal dialysis patients with pleuroperitoneal leakage and right-sided hydrothorax who underwent video-assisted thoracoscopy with mesh implantation from 2011 to 2020. Pleuroperitoneal leakage had been confirmed before su

Meta-analysis of subxiphoid approach versus lateral approach for thoracoscopic Thymectomy.

BACKGROUND: Compared with traditional open surgery for thymectomy, video-assisted thoracoscopic surgery (VATS) reduces hospital stay, decreases postoperative pain, and recovers faster. VATS has become increasingly popular in the past decade. VATS techniques to perform a thymectomy include subxiphoid video-assisted thoracoscopic surgery (SVATS) or lateral video-assisted thoracoscopic surgery (LVATS). In this study, our objective was to systematically review on VATS thymectomy and draw a meta-analysis on the outcomes between the two approaches. METHODS: We searched online databases and identifi

Right Thoracoscopic Aberrant Right Subclavian Artery Division and Subclavian-Carotid Transposition.

Video-assisted thoracoscopic surgical division of obstructive congenital vascular rings has been demonstrated to be a safe technique in the setting of right-sided or double aortic arch, requiring a left-sided thoracic approach. This report describes 2 cases of aberrant right subclavian artery division using right video-assisted thoracoscopic surgery and subclavian-carotid transposition. This approach is safe and feasible in the management of symptomatic aberrant right subclavian artery without the morbidity of a thoracotomy incision, although patients require ongoing surveillance.
