A pernicious leucoencephalopathy.

Pernicious anaemia may manifest various neurological symptoms and signs ranging from the subtle to the dramatic. We describe a young man with cobalamin deficiency presenting with sensorimotor deficits, ataxia, dysarthria, mild cognitive deterioration and altered mood of insidious onset. The MRI brain findings were in keeping with a leucoencephalopathy without evidence of MRI changes in the spinal cord. This constellation of features has been reported rarely. His response to treatment as well as the marked improvement of the leucoencephalopathy on imaging suggests at least partial reversibility

To Determine Vitamin B12 Deficiency in Type 2 Diabetes Mellitus Patients on Metformin Therapy.

INTRODUCTION: India harbors the second-largest population with diabetes, with over 100 million, and type 2 diabetes mellitus (T2DM) constitutes the major share. Metformin remains the first-line pharmacotherapy for T2DM due to its safety profile, cost-effectiveness, and beneficial metabolic effects. MATERIALS AND METHODS: The aim of the study was to assess the frequency of vitamin B12 deficiency in patients with T2DM on metformin therapy and compare it with their cohabiting family members who are not on metformin but share similar dietary habits. RESULTS: This study included 180 participants

Vitamin B12 Deficiency in the Diagnostic Work-Up of Global Developmental Delay: A Treatable and Time-Sensitive Condition.

Background: Vitamin B12 deficiency is a recognized but frequently under-integrated cause of global developmental delay (GDD) in infancy and early childhood. Early diagnosis is critical because neurological impairment may be partially or completely reversible with timely treatment. Objective: This narrative review aims to synthesize current evidence on the role of vitamin B12 deficiency in the diagnostic evaluation of GDD, with a focus on clinical phenotype, risk factors, biomarkers, treatment outcomes, and practical integration into contemporary diagnostic algorithms. Methods: A structured, no

Delusional parasitosis with shared psychotic features secondary to vitamin B12 deficiency and mild cognitive impairment: a case report.

BACKGROUND: Visual illusions are misperceptions of real external stimuli and, when persistent, may indicate underlying neurological or metabolic disorders. Delusional parasitosis is characterized by a fixed belief of infestation without medical evidence and may occur secondary to medical conditions. However, the relationship between visual illusion-related perceptual abnormalities and delusion formation, particularly in reversible metabolic states such as vitamin B12 deficiency, remains unclear. CASE PRESENTATION: An 80-year-old man presented with a persistent belief that mites infested his b

Vitamin B12 deficiency, restless legs syndrome and poor sleep quality in adolescents: A cross-sectional study.

Although the role of dopaminergic dysfunction and iron metabolism in the pathophysiology of restless legs syndrome (RLS) has been widely investigated, the potential association between vitamin B12 status and RLS remains insufficiently explored. In particular, data in pediatric and adolescent populations are limited and inconsistent. Moreover, while sleep disturbance is a well-recognized feature of RLS, the combined relationship between vitamin B12 levels, RLS, and sleep quality has not been adequately evaluated in adolescents. Therefore, the present study aimed to investigate the association b

Beyond Folate: The Emerging Role of Maternal Vitamin B12 in Neural Tube Development.

Background/Objectives: Folic acid (FA) supplementation can effectively reduce the occurrence of neural tube defects (NTDs). Vitamin B12 is involved in folate metabolism; however, studies have not reached a definitive conclusion on the association between vitamin B12 and NTDs independent of folate levels. A systematic review and meta-analysis were performed to summarize existing research and investigate the effect of vitamin B12 on NTDs. Methods: Studies were systematically searched in PubMed, Web of Science, Embase, and Cochrane, published before 1 March 2024. Standardized mean difference (SMD

Novel genetic variants associated with stable isotope-based vitamin B12 bioavailability measurements in healthy Indian adults.

BACKGROUND: Vitamin B12 deficiency is a prevalent health concern that can lead to hematological and neurological disorders. Despite adequate intake, vitamin B12 deficiency/insufficiency can arise due to interindividual differences in absorption and metabolism influenced by genetic factors, gastrointestinal health, and medications. Although previous studies have explored the genetic basis of circulating vitamin B12 concentration, they have not been validated using precise, stable, isotope-based measurements of vitamin B12 bioavailability. OBJECTIVES: To measure the plasma bioavailability of an

Integrative proteomic and metabolomic analysis reveals the mechanisms underlying ischemic stroke induced by vitamin B12 deficiency.

BACKGROUND AND OBJECTIVE: Vitamin B12 deficiency is an emerging public health concern increasingly linked to neurological disorders, including ischemic stroke, yet its underlying molecular mechanisms remain poorly understood. This study aimed to elucidate the molecular basis of vitamin B12 deficiency-associated ischemic stroke by integrating serum-based metabolomic and proteomic analyses. METHODS: This study enrolled patients with first-episode acute ischemic stroke, stratified into vitamin B12-deficient (≤ 250 pg/mL) and non-deficient groups, along with age- and sex-matched healthy controls.

Neurodevelopmental and neuropsychiatric disorders in cobalamin C disease: a case report and review of the literature.

Cobalamin C disease is the most common complementation class of cobalamin disorders. Here, we present a case of a 14-yr-old male with early-onset cblC disease and autism spectrum disorder (ASD) admitted to our inpatient medical service for behavioral decompensation. We use this case to highlight key aspects of the neurodevelopmental and neuropsychiatric disorders associated with cblC disease. By incorporating a comprehensive review of existing literature, we highlight salient domains of psychological impairment in cblC disease, discuss the full range of neuropsychiatric presentations, and revi

Associations of serum folate and vitamin C levels with metabolic dysfunction-associated fatty liver disease in US adults: A nationwide cross-sectional study.

BACKGROUND: Clinical research results on the relationship between folate and non-alcoholic fatty liver disease are contradictory. Metabolic dysfunction-associated fatty liver disease (MAFLD) is a recently proposed concept. Evidence about the relationship between serum folate and MAFLD, especially considering the status of serum vitamin C, is scarce. This study was aimed to investigate the association of serum folate levels with the prevalence of MAFLD, and further to analyze the potential impact of serum vitamin C status on their association. METHODS: Totally 2,797 participants from National

Impact of baseline vitamin B12 status on the effect of vitamin B12 supplementation on neurologic function in older people: secondary analysis of data from the OPEN randomised controlled trial.

BACKGROUND/OBJECTIVES: The available evidence from randomised controlled trials suggests that vitamin B12 supplementation does not improve neurologic function in older people with marginal but not deficient Vitamin B12 status. This secondary analysis used data from the Older People and Enhanced Neurological function (OPEN) randomised controlled trial to assess whether baseline vitamin B12 status or change in vitamin B12 status over 12 months altered the effectiveness of dietary vitamin B12 supplementation on neurologic function in asymptomatic older people with depleted vitamin B12 status at s

Associations between medication use and homocysteine levels in an older population, and potential mediation by vitamin B12 and folate: data from the B-PROOF Study.

BACKGROUND: Elevated homocysteine levels are a risk indicator for cardiovascular disease, fractures and cognitive decline. Previous studies indicated associations between homocysteine levels and medication use, including antihypertensive, lipid-lowering and antidiabetic medication. However, results were often contradictory and inconclusive. Our objective was to study the associations established previously in more detail by sub-classifying medication groups, and investigate the potential mediating role of vitamin B12 and folate status. MATERIALS AND METHODS: Baseline data from the B-PROOF (B-
