Regional Consortia of Cancer Center Shared Resources: Models for Collaborative Infrastructure and the Emergence of a National Alliance.

Shared resource facilities are foundational to the scientific productivity of National Cancer Institute (NCI)-designated cancer centers, yet institutional silos frequently constrain member access to specialized instrumentation and expertise. Interinstitutional consortia of shared resources offer a pragmatic solution, enabling reciprocal service access at internal rates while preserving institutional autonomy. Here we describe two established regional consortia, the Mid-Atlantic Cancer Center Shared Resource Consortium (MACCSRC) and the Fred Hutchinson/University of Washington/Seattle Children'…

Intracranial hemorrhage in critically ill patients hospitalized for COVID-19.

In this study, we report three cases of spontaneous intracranial hemorrhage in patients who were initially hospitalized at our tertiary care center in Washington, DC with symptoms of COVID-19. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was diagnosed in all three patients, who were critically ill, requiring intubation and ventilatory support. During their protracted hospitalizations, subsequent imaging disclosed intracranial hemorrhages, including intracerebral and subarachnoid hemorrhages, in the context of anticoagulation and coagulopathy. We believe this is relate…

Oral hygiene instruction and health risk assessment in dental practice.

We studied oral hygiene instruction given to 109 patients in 19 Washington State dental practices to investigate the extent to which therapists targeted their efforts toward patients with high disease risk. Patients were examined prior to instruction and prophylaxes. Therapists' instructions were tape-recorded and their content analyzed: therapists' expectations were scored. There were no statistically significant associations between patients' initial plaque levels and the process/content of the oral hygiene instructions delivered. On average, therapists spent 9.4 minutes of each prophylaxis …

Correlates of placental abruption.

All cases (884) of placental abruption reported on Washington State birth or fetal death certificates in 1980 and 1981 were compared with 789 randomly selected births from the same time period. The incidence of reported cases of placental abruption was 6.5 per 1000 total births. The combined stillbirth and first month death rate of all abruption cases was 21%. Increased risks of placental abruption were associated with pre-eclampsia, diabetes, and unmarried status. Neither parity nor maternal age was associated with an increased risk. Infants born after abruption were significantly smaller-for…

Unfamiliar aspects of familial polyposis coli.

Patients with familial polyposis coli or Gardner's syndrome are at risk for a variety of extracolonic manifestations. In a series of patients followed at the University of Washington, we have found several recurring and unusual manifestations, including upper gastrointestinal polyposis, small bowel obstruction secondary to desmoid tumors or adhesions, recurring pancreatitis, and adenoma of the papilla of Vater. In one family with familial polyposis only, a set of twins had different manifestations; one twin had familial polyposis only, whereas the second had classic extracolonic manifestations…

Bacteriological survey of sixty health foods.

A bacteriological survey was performed on 1,960 food samples encompassing 60 types of health foods available in the Baltimore-Washington, D.C., metropolitan area. No consistent bacteriological distinction (aerobic plate counts, total coliform and fecal coliform most probable numbers) was observed between foods labeled as organic (raised on soil with compost or nonchemical fertilizer and without application of pesticides, fungicides, and herbicides) and their counterpart food types bearing no such label. Types and numbers of samples containing Salmonella were: sunflower seeds, 4; soy flour, 3; …

A Pilot TB Screening Model in a U.S. Prison Population Using Tuberculin Skin Test and Interferon Gamma Release Assay Based on Country of Origin.

The objective of this study was to compare tuberculosis (TB) screening results before and after implementation of a stratified testing strategy screening pilot study, incorporating interferon gamma release assay (IGRA) and tuberculin skin test (TST), based on country of origin. In 2015, the Washington State Department of Corrections began screening people born outside of the United States for TB with IGRA, while U.S.-born people continued screening by TST. Of 405 (75%) foreign-born men screened with IGRA, 403 had valid test results and IGRA screening positivity was 10.4% (N = 42). In contrast,…

The emergence of SARS-CoV-2 in Europe and North America.

Accurate understanding of the global spread of emerging viruses is critical for public health responses and for anticipating and preventing future outbreaks. Here we elucidate when, where, and how the earliest sustained severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission networks became established in Europe and North America. Our results suggest that rapid early interventions successfully prevented early introductions of the virus from taking hold in Germany and the United States. Other, later introductions of the virus from China to both Italy and Washington state, Unit…

Paragonimus kellicotti flukes in Missouri, USA.

Paragonimiasis is an infection caused by lung flukes of the genus Paragonimus. In Asia, P. westermani infections are relatively common because of dietary practices. However, in North America, cases of paragonimiasis, which are caused by P. kellicotti flukes, are rare. Only 7 autochthonous cases of paragonimiasis were reported during 1968-2008. In 2009, we reported 3 new case-patients with paragonimiasis who had been seen at our medical center over an 18-month period. Six additional case-patients were identified in St. Louis, Missouri, USA, and treated at Washington University-affiliated health…

Vaginal colonization by Escherichia coli as a risk factor for very low birth weight delivery and other perinatal complications.

This study evaluated the relationship of vaginal Escherichia coli colonization to birth weight <1500 g and other perinatal complications in a cross-sectional study of 2646 women at the University of Washington Medical Center, Seattle, between October 1992 and January 1995. Vaginal E. coli colonization was more strongly associated with delivery at <34 weeks (relative risk [RR], 1.7; 95% confidence interval [CI], 1.3-2.3) and very low birth weight (RR, 1.9; 95% CI, 1.3-2.7) than with prematurity between 34 and 36 weeks or low birth weight. Heavy growth of E. coli had a higher risk of very low bi…

Multiple sclerosis in black children.

Six cases of childhood multiple sclerosis (MS) are presented. All patients (three boys and three girls) were black and came from the Washington, DC, metropolitan area. Age at onset of the disease ranged from 8 to 17 years. Although motor deficits, optic neuritis, and ataxia were similar to those previously reported in pediatric patients, mental symptoms were more prominent and the course more intractable. The pattern of disease was characterized by a high frequency of relapses, with short remission and rapid progression. Within 1 to 7 years, one patient had died, one became severely handicappe…

Reproductive history and postmenopausal risk of hip and forearm fracture.

This case-control study was designed to investigate the relation between reproductive history and occurrence of hip and forearm fractures in postmenopausal women. Three hundred and fifty-five King County, Washington women who sustained a fracture between 1976 and 1980 and 562 control women were interviewed regarding their reproductive history and other factors. After controlling for the confounding effects of age, obesity, and use of noncontraceptive estrogens, the authors found that women who gave birth four or more times had a risk of fracture similar to women who had not given birth (odds r…

Extracorporeal Membrane Oxygenation for Severe Burn Injury: A Multidisciplinary Roundtable.

Extracorporeal membrane oxygenation (ECMO) has been used to successfully minimize, replace, or avoid the use of mechanical ventilation in burned and nonburned populations. Further research on ECMO in the burned population is warranted, particularly given the insufficient patient numbers reported in the literature. A panel composed of national leaders in ECMO and/or burn injury discussed the use of ECMO in patients with burn injuries as part of the MedStar Washington Hospital Center Burn Center Multidisciplinary Lecture Series. Panelists examined current methods, institutional practices, and cl…

NEW PARISH RECORDS FOR AEDES JAPONICUS IN LOUISIANA, USA.

Aedes japonicus is an invasive container-breeding mosquito species of considerable medical and veterinary importance. This species is native to eastern Asia but spread to North America in the late 1990s and to central Europe later. In the United States, Ae. japonicus was first detected in the Northeast in 1998. It has since spread to all states east of the Mississippi river including the five bordering states along the river's western bank (Minnesota, Iowa, Missouri, Arkansas, Louisiana) as well as to Nebraska, Oklahoma, Texas, Washington, Oregon, and Hawaii. In Louisiana, Ae. japonicus was fi…
