SLE and acute Addisonian crisis due to bilateral adrenal hemorrhage: association with the antiphospholipid syndrome.

This report describes a 68-year-old woman with SLE and positive antiphospholipid antibodies who developed an acute Addisonian crisis due to bilateral adrenal hemorrhage. We suggest that adrenal bleeding can be part of the thrombo-embolic complications seen in the secondary antiphospholipid syndrome.

Hypoadrenalism in patients with pulmonary tuberculosis in Tanzania: an undiagnosed complication?

Addison's disease is rarely diagnosed in most African countries although tuberculosis, one of its major causes, is a widespread problem. In this study adrenal function was assessed using the Synacthen test in 50 patients with chronic pulmonary tuberculosis admitted to hospital in Dar es Salaam. Sixteen patients (32%) had an impaired response. Two had subnormal basal cortisol levels, one of whom had a normal response to Synacthen. There was no significant difference between the patients with an impaired cortisol response and those with a normal response with respect to frequency of non-specific

Autoimmune polyglandular syndrome and GAD65 related-temporal lobe epilepsy.

INTRODUCTION: Autoimmune polyglandular syndromes (APS) encompass a broad spectrum of disorders. APS III is characterized by autoimmune thyroiditis (AT) plus any autoimmune disorder, excluding Addison's disease. METHODS AND RESULTS: This study explores the link between GAD65 related-temporal lobe epilepsy (GAD-TLE) and APS III. We retrospectively analyzed 29 GAD-TLE patients. Eighteen patients (62%) experienced at least one autoimmune disease, and ten patients (56%) fulfilled diagnostic criteria for APS III. In addition to AT and GAD-TLE, five patients had type 1 diabetes mellitus, one patient

A Rare Case of Autoimmune Polyendocrinopathy-candidiasis-ectodermal Dystrophy Syndrome: Dental Perspective on Diagnosis and Management.

AIM: To report a unique case of Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) in a young boy and discuss the oral health impact and management of the disease. BACKGROUND: Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) is a rare autoimmune disorder with various clinical manifestations. Biallelic mutations in the autoimmune regulator (AIRE) gene lead to impairment of central immune tolerance and a targeted attack on various endocrine and non-endocrine organs. Patients classically suffer from a triad of disorders, including chronic mucocutaneous

Role of chemokines in endocrine autoimmune diseases.

Chemokines are a group of peptides of low molecular weight that induce the chemotaxis of different leukocyte subtypes. The major function of chemokines is the recruitment of leukocytes to inflammation sites, but they also play a role in tumoral growth, angiogenesis, and organ sclerosis. In the last few years, experimental evidence accumulated supporting the concept that interferon-gamma (IFN-gamma) inducible chemokines (CXCL9, CXCL10, and CXCL11) and their receptor, CXCR3, play an important role in the initial stage of autoimmune disorders involving endocrine glands. The fact that, after IFN-g

Prevalence and clinical associations of 10 defined autoantibodies in autoimmune polyendocrine syndrome type I.

The prevalence of autoantibodies against nine intracellular enzyme autoantigens, namely 21-hydroxylase, side-chain cleavage enzyme (SCC), 17 alpha-hydroxylase, glutamic acid decarboxylase 65, aromatic L-amino acid decarboxylase, tyrosine phosphatase-like protein IA-2, tryptophan hydroxylase (TPH), tyrosine hydroxylase, cytochrome P450 1A2, and against the extracellular calcium-sensing receptor, was assessed in 90 patients with autoimmune polyendocrine syndrome type I. A multivariate logistic regression analysis was performed for the presence of autoantibodies as independent predictors for diff

Risk of cancer of unknown primary after hospitalization for autoimmune diseases.

Cancer of unknown primary (CUP) is a heterogeneous syndrome diagnosed at metastatic sites. The etiology is unknown but immune dysfunction may be a contributing factor. Patients with autoimmune diseases were identified from the Swedish Hospital Discharge Register and linked to the Swedish Cancer Registry. Standardized incidence ratios (SIRs) were calculated for subsequent CUP and compared with subjects without autoimmune diseases. A total of 789,681 patients were hospitalized for any of 32 autoimmune diseases during years 1964-2012; 2,658 developed subsequent CUP, giving an overall SIR of 1.27.

Vitamin B12 deficiency as a cause of severe neurological symptoms in breast fed infant - a case report.

BACKGROUND: Vitamin B12 (cobalamin, cbl) deficiency in children is rare and may occurs in exclusively breast fed infants of mothers on vegetarian or vegan diet with lack of appropriate supplementation. The clinical manifestation of vitamin B12 deficiency include neurological disorders, megaloblastic anemia and failure to thrive. Routine and commonly used laboratory tests such as cell blood count (CBC) or serum vitamin B12 level are sufficient for appropriate diagnosis. Typical therapy is based on intramuscular cobalamin injections. Early diagnosis and early onset of treatment are crucial facto

Receptors, antibodies, and disease.

Abnormal antibody production is now recognized as the basis of specific endocrine and neurological diseases and their complications. Among the autoimmune diseases, the best understood from a mechanistic point of view are myasthenia gravis, Graves' disease, several variants of insulin resistance, and a variant of bronchial asthma. In each of these human disorders, the clinical symptoms can be traced to the actions of antireceptor antibodies produced by a deranged immune system. The autoantibodies produced in these diseases are functionally heterogeneous. They may produce the clinical symptoms o

[Xp21 contiguous gene deletion syndrome].

Contiguous gene syndromes (CGS) arise from microdeletions and other aberrations of a chromosome region containing several gene loci. One of the CGSs is Xp21 contiguous gene deletion syndrome (Xp21.3-p21.2) (Xp21 deletion syndrome). Slightly more than 100 male patients suffering from Xp21 contiguous gene deletion have been described in the world literature. This article describes a clinical case of a boy with Xp21 deletion syndrome. Chromosomal microarray analysis revealed a microdeletion of the X chromosome region from position 28085320 to position 33391678 with a size of 5306358 bp, including

Clinical manifestations and associated factors in acquired hypoaldosteronism in endocrinological practice.

INTRODUCTION: Hypoaldosteronism can be congenital or acquired, isolated or part of primary adrenal insufficiency, and caused by an aldosterone deficit, resistance, or a combination of both. Reduced mineralocorticoid action can induce a decrease in urine K+ and H+ excretion and an increase in urine Na+ excretion, leading to hyperkalemia, and/or hyponatremia, often combined with metabolic acidosis. We aimed to characterize the clinical manifestations of hypoaldosteronism, and their associated factors. METHODS: Retrospective analysis of 112 episodes of hypoaldosteronism diagnosed in 86 adult pat

An unusual presentation of primary adrenal insufficiency with new onset type 1 diabetes: case report and review of the literature.

OBJECTIVES: To describe an atypical presentation of primary adrenal insufficiency in conjunction with new onset type 1 diabetes. CASE PRESENTATION: Here, we describe a case of new-onset type 1 diabetes (T1D) presenting simultaneously with an unusual presentation of primary adrenal insufficiency in a previously healthy 16-year-old. He was admitted for a typical presentation of diabetic ketoacidosis, but with extreme hyponatremia. An extensive workup revealed a low aldosterone level, appropriate cortisol level, and positive 21-hydroxylase antibodies. While the phenomenon of multiple autoimmune

Chronic primary adrenal insufficiency after unilateral adrenonephrectomy: A case report.

RATIONALE: Unilateral adrenalectomy as part of surgical resection of renal cell carcinoma (RCC) is not thought to increase the risk of chronic adrenal insufficiency, as the contralateral adrenal gland is assumed to be capable of compensating for the lost function of the resected gland. However, recent studies have indicated that adrenalectomy might cause irreversible impairment of the adrenocortical reserve. We describe a case of chronic primary adrenal insufficiency in a 68-year-old man who previously underwent unilateral adrenonephrectomy, which was complicated by severe postoperative adrena
