Concomitant reconstruction of mandibular basal and alveolar bone with a free fibular flap.

Repair of long-span mandibular defects with a free fibular flap is now a routine procedure. However, the bone height of the neo-mandible after reconstruction with a fibular flap is about half that of the dentulous mandible. When a fibular graft is placed only at the inferior border of the mandible, the resulting vertical discrepancy between the graft segment and the occlusal plane can adversely affect implant mechanics or denture stability and retention. To overcome these problems, we developed a technique for two-strut type mandibular reconstruction. A vascularized fibular segment is used to

Zygomatic bone: an additional donor site for alveolar bone reconstruction. Technical note.

This article describes a procedure to harvest bone from the zygoma for alveolar bone reconstruction. A detailed description of the bone harvesting procedure and a preliminary report of 3 patients undergoing alveolar bone reconstruction and simultaneous dental implant placement in the maxillary anterior area is presented. The technique is indicated when a modest amount of bone is needed, for example, to cover exposed implant threads and expand a narrow alveolar ridge. It also could be used as an additional source of bone with other intraoral donor sites. Surgical access to the zygoma is simple

[Oromaxillofacial surgical treatment in aging patients].

The oral and maxillofacial surgical therapy of the elderly patient must include considerations of age-related physiological changes, both of inner organs and oral structures. Decreased hepatic and renal function may lead to delayed elimination of anaesthetic drugs and thereby make dose reductions necessary. Sclerosis or osteoporosis of jaw bones may render the surgical treatment of odontogenic diseases more difficult due to the increased fracture hazard. Painful ankylosis and rheumatic arthritis of the temporo-mandibular joint, tumors and fractures are common diseases of the elderly patient th

Evaluation of the effects of menopause on the quality and quantity of intraoral autogenous block graft donor sites using cone-beam computed tomography.

BACKGROUND: Menopause is characterized by the permanent cessation of menstruation and the loss of estrogen-related protective effects on systemic bone metabolism. One of the major consequences of this hormonal alteration is accelerated bone loss and deterioration of bone microarchitecture. Reduced estrogen levels lead to a decrease in bone mineral density and markedly increase the likelihood of osteoporosis development. OBJECTIVE: The present study aims to systematically assess postmenopausal changes affecting both the volume of graft material that can be harvested from intraoral autogenous b

An innovative technique to manage sinus membrane perforations: report of two cases.

The most frequent intraoperative complication of maxillary sinus elevation procedures is the perforation of the sinus membrane, for which various protocols and grafting materials have been proposed in the literature. This article describes a novel suturing technique to close large sinus mucosa perforations. The technique is demonstrated through two clinical cases in which the sinus perforations occurred (1) in the course of a maxillary sinus elevation procedure and (2) after the removal of a cystic lesion. Bone grafting material and dental implants were placed simultaneously with the sinus rep

Autologous platelet concentrates for bone graft enhancement in sinus lift procedure.

BACKGROUND: Autologous platelet (PLT)-rich plasma has been reported in some studies to promote osteogenesis. The goal of this study was to demonstrate that osteogenesis gained by mixing autologous PLT concentrates (APCs) with a small quantity of autologous bone graft could give a sufficient quality to lead to dental implant placement. The second goal was to compare this osteogenesis with that obtained by a traditional method (iliac bone graft), through clinical, radiologic, and histologic methods. STUDY DESIGN AND METHODS: Eighteen patients needing bilateral sinus floor augmentation were enro

Piezoelectric internal sinus elevation (PISE) technique: a new method for internal sinus elevation.

This article reports the surgical procedure of a transalveolar approach for sinus floor elevation using piezoelectric surgery without a mallet, called piezoelectric internal sinus elevation (PISE) technique. PISE is the modified technique of the osteotome-mediated sinus floor elevation. Osteotome-mediated sinus floor elevation uses a mallet to break the sinus floor and to push sinus membrane up. Using this technique might possibly bring positional vertigo to the patient. PISE technique does not use a mallet during the procedure to break the sinus floor and to elevate the sinus membrane; theref

Optimisation of the superplastic forming of a dental implant for bone augmentation using finite element simulations.

OBJECTIVES: The phenomenon of superplasticity has made it possible to form complex shapes that require extremely high degrees of ductility in titanium alloy with minimal internal stresses. Combined with the use of an investment casting material as the die material, which makes possible the forming of re-entrant angles, it is possible to produce membranes for ridge augmentation. The aim is to characterise the metal alloy sheet and simulate the superplastic forming process in three dimensions to produce process parameters, namely gas pressure as a function of time, to accurately adapt the titani

Evaluation of a prototype trilayer membrane (PTLM) for lateral ridge augmentation: an experimental study in the canine mandible.

The objective of this animal study was to evaluate a biodegradable/bioresorbable prototype trilayer membrane (PTLM) consisting of two collagen layers and an internal polylactide layer for lateral ridge augmentation in conjunction with two different bone grafting materials: particulate autograft or deproteinized bovine bone mineral (DBBM). In four mongrel dogs, two lateral bone defects per side were created in the mandible. The four defects per dog were randomly subjected to the following grafting treatments 3 months later: 1. PTLM+DBBM, 2. PTLM+particulate autograft, 3. ePTFE membrane+DBBM, 4.

Continuous intra-sinus bone regeneration after nongrafted sinus lift with a PLLA mesh plate device and dental implant placement in an atrophic posterior maxilla: a case report.

BACKGROUND: Sinus lift is a bone augmentation procedure that improves the alveolar crest height in an atrophic posterior maxilla. However, the regenerated bone volume can vary and generally has a tendency to decrease after sinus operation. This article describes nongrafted maxillary sinus lift using a bioresorbable unsintered hydroxyapatite combined with poly L-lactide (HA/PLLA) mesh plate device and dental implant placement in an atrophic posterior maxilla, after which continuous bone gain was observed around the implant apex during a postoperative follow-up period of 3 years. Synthetic Biomaterials for Alveolar Bone Regeneration: A Systematic Review of Clinical Evidence.

BACKGROUND: Alveolar bone resorption remains a major challenge in implant and prosthetic rehabilitation. While autologous bone grafts are still considered the gold standard, their biological and surgical limitations have promoted the use of synthetic biomaterials such as biphasic calcium phosphate (BCP), β-tricalcium phosphate (β-TCP), nanocrystalline hydroxyapatite, and bioactive glass. METHODS: This systematic review, conducted in accordance with PRISMA guidelines, was based on a comprehensive search performed in March 2025 across PubMed, MEDLINE, Embase, and Google Scholar. Histologic healing following tooth extraction with ridge preservation using mineralized versus combined mineralized-demineralized freeze-dried bone allograft: a randomized controlled clinical trial.

BACKGROUND: Mineralized and demineralized freeze-dried bone allografts (FDBAs) are used in alveolar ridge (AR) preservation; however, each material has advantages and disadvantages. Combinations of allografts aimed at capitalizing on the advantages each offers are available. To date, there is no evidence to indicate if a combination allograft is superior in this application. The primary objective of this study is to histologically evaluate and compare healing of non-molar extraction sites grafted with either mineralized FDBA or a 70:30 mineralized:demineralized FDBA combination allograft in AR

Xenograft versus extraction alone for ridge preservation after tooth removal: a clinical and histomorphometric study.

BACKGROUND: The preservation of bone volume immediately after tooth removal might be necessary to optimize the success of implant placement in terms of esthetics and function. The objectives of this randomized clinical trial were two-fold: 1) to compare the bone dimensional changes following tooth extraction with extraction plus ridge preservation using corticocancellous porcine bone and a collagen membrane; and 2) to analyze and compare histologic and histomorphometric aspects of the extraction-alone sites to the grafted sites. METHODS: Forty subjects who required tooth extraction and implan
