The appendix and ulcerative colitis - an unsolved connection.

The appendix is thought to have a role in the pathogenesis of ulcerative colitis, but the nature and basis of this association remains unclear. In this Perspective, we consider the biology of the appendix with respect to its immunological function and the microbiome, and how this relates to evidence that supports the involvement of the appendix in ulcerative colitis. In experimental models, removal of the inflamed appendix prevents colitis, and in human observational studies, appendectomy is associated with protection against ulcerative colitis. Further, among people who develop ulcerative col…

The spectrum of appendiceal disease in cystic fibrosis.

A retrospective review of 1,220 cystic fibrosis (CF) patients between 1965 and 1989 identified 60 patients who underwent appendectomy. Ten had appendectomy prior to referral and 16 had an incidental appendectomy (primarily meconium ileus). Among the remaining 34 patients, acute appendicitis was present in 19 (1.8% incidence). "Classic appendicitis"--acute abdominal pain shifting to the right lower quadrant (RLQ), focal RLQ tenderness, and elevated white blood cell (WBC) count--was present in 15. Four additional inflamed appendixes were removed in patients following incorrect preoperative diagn…

[Acute abdomen due to perforated appendicular mucocele].

Primary neoplasms of the appendix are present in approximately 0.2% to 0.3% of appendectomy specimens. Generally affect middle-age adults and elderly people. Approximately 30% to 50% of appendiceal neoplasms manifest with acute appendicitis by luminal obstruction. The definitive diagnosis is histological. We must avoid simple laparoscopic appendectomy and adopt an open procedure with a laparotomy appendectomy for a bening process. Right hemicolectomy is indicated when a malignant condition is suspected Follow-up is recommended in this pathology due to the risk of associated malignant neoplasms…

Atypical Duplex appendix arising from the ascending colon: a case report.

BACKGROUND: Duplex or vermiform appendix refers to the presence of an appendix beside the naturally occurring one. Although, duplex appendix emerges from the caecum most of the time, yet it is encountered in other parts of the colon. Inflammation of duplex appendix may represent not only a clinical, but also a surgical dilemma, and this would be confusing further among patients who already had prior appendectomy. CASE PRESENTATION: We present a case of 29-years old Egyptian male patient with history of appendectomy one and half year before presenting to the emergency department with recurrent…

Morbidity of laparoscopic surgery for complicated appendicitis: an international study.

BACKGROUND: Although laparoscopic appendectomy has some advantages over open appendectomy, some reports do show more postoperative intraabdominal abscesses. METHODS: A retrospective review of complicated appendicitis managed surgically by eight surgical groups from six countries was undertaken. Among 3,433 patients with appendicitis, 1,017 (29.5%) had complicated appendicitis, which included perforated or gangrenous appendicitis with or without localized or disseminated peritonitis. There were 74 preoperative abscesses (7.4%) and 5 small bowel obstructions. RESULTS: One patient died. There w…

A case of well-differentiated neuroendocrine tumor of the appendix in a 14-year-old male adolescent clinically presented with features of acute appendicitis: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Neuroendocrine neoplasms (NENs) of the appendix are rare in children even though they are the second commonest gastrointestinal tumor next to lymphoma. NENs are usually diagnosed incidentally in appendectomies done for appendicitis, occasionally resulting in a second operation to ensure adequacy of the management. This case report is aimed at emphasizing the significance of subjecting any appendectomy specimen to histopathologic examination, irrespective of the indication for appendectomy. PRESENTATION OF CASE: Here we presented a case of a 14-year-old male adoles…

Seeds or Parasites? Clinical and Histopathological Features of Seeds and Parasites in the Appendix.

OBJECTIVE: Parasites and plant seeds may both be found in appendectomy specimens. Each plant seed has a different appearance and can thus exhibit wide variations under the microscope. Fragmented seeds may histologically mimic parasites. The differential diagnosis between seeds and parasites can be challenging in such cases. This study aimed to determine the incidence of parasites, seeds, and foreign bodies in appendectomy materials and highlight the most characteristic histopathological features associated with these structures. MATERIAL AND METHOD: In this study, pathology slides of 9,480 pa…

Acute appendicitis in pregnancy: 50 case series, maternal and neonatal outcomes.

BACKGROUND: During pregnancy, the most common indication for non-obstetric surgery in acute abdomen is appendicitis. In pregnancy, appendicitis may be confused with pregnancy-related pathologies and may cause a delay in diagnosis or unnecessary surgery. The present study aims to evaluate the maternal and neonatal outcomes of patients undergoing appendectomy during pregnancy. METHODS: This study was designed retrospectively between 2011-2017. Appendicitis detection rates, admission and laboratory features, operation results and obstetric results were evaluated in pregnant women who underwent s…

Adenocarcinoma of Stump Appendicitis: An Extremely Rare Pathology - A Literature Review.

Emergency appendectomy for acute appendicitis is the most common emergency surgical procedure performed all over the world. While amputating the appendix from the cecum, a small (usually less than 5 mm) stump is left behind. Below this, the suture or stapler is applied to secure the base of the appendix, which is now known as a stump. Stump appendicitis, the inflammation of appendiceal remnant after an appendectomy, is a rare phenomenon. Additionally, the incidence of adenocarcinoma in the stump of the appendix is also extremely rare and can present with the symptoms of appendicitis. Adenocarc…

Granulomatous Appendicitis in the Pediatric Population.

BACKGROUND: The clinical significance of a diagnosis of granulomatous appendicitis (GA) is not well established in the pediatric population. METHODS: Retrospective review of the etiology and histopathology of pediatric GA at 2 large pediatric institutions. RESULTS: Forty-three (0.4%) patients had GA at a median age of 12.1 years. Twenty-two (51%) had perforated appendicitis that was medically managed prior to interval appendectomy. Sixteen of 21 noninterval appendectomies were performed for clinically suspected acute appendicitis. Among them, 1 had Crohn's disease identified during surgery. …

Calprotectin could be a potential biomarker for acute appendicitis.

BACKGROUND: Acute appendicitis is a common cause for a visit to the emergency department and appendectomy represents the most common emergency procedure in surgery. The rate of negative appendectomy however has remained high despite modern diagnostic apparatus. Therefore, there is need for a better preoperative screening of patients with suspected appendicitis. Calprotectin represents a predominant protein in the cytosol of neutrophil granulocytes and has been extensively investigated with regard to bowel pathologies. This study investigates the expression of calprotectin in the lumen of the v…

Ultrasound-based decision making in the treatment of acute appendicitis in children.

BACKGROUND/PURPOSE: Imaging techniques are used widely to diagnose appendicitis. However, the negative appendectomy rate remains at about 15%. The authors assessed ultrasound-based decision making in the treatment of acute appendicitis in children. METHODS: The authors prospectively studied 165 consecutive children (3 to 15 years old) evaluated for appendicitis. Diagnosis and treatment were based solely on ultrasound scan findings. Criterion for appendicitis was a diameter exceeding 6 mm. Severity was classified into 4 grades based on the appearance of intramural appendiceal structure. Patien…

Neuroimmunoendocrine peptides on inflammed and morphologically normal appendices removed due to clinical acute appendicitis.

INTRODUCTION: Despite clinical characteristics and complementary exams indicate acute appendicitis, 15% to 40% of all appendectomies result in removal of appendices with normal macro- and micromorphological aspects. Even so, manifestations of acute abdomen disappear immediately after the appendectomy, and never show back again. OBJECTIVE: To assess changes of neuroimmunoendocrine peptides on removed appendices due to clinical presentation of acute appendicitis. METHOD: This article presents an updated revision of acute appendicitis, based on references found on PUBMED, LILACS, MEDLINE, WHOLI…

Appendicitis is a Severe Disease in Elderly Patients: A Twenty-Year Audit.

BACKGROUND AND OBJECTIVES: Life expectancy has increased substantially. Elderly patients currently represent a large part of patients requiring emergency abdominal surgery. The aim of this study was to evaluate the postoperative outcomes of elderly patients who underwent appendectomy in a single French tertiary center. METHODS: We retrospectively reviewed the medical records of all patients who underwent appendectomy for acute appendicitis between January 1, 1994 and December 31, 2014. We used the French threshold of ≥ 75 y-old to define elderly patients. Hence, elderly patients who underwent…

Safety and feasibility of same-day discharge for uncomplicated appendicitis: A prospective cohort study.

BACKGROUND: Appendicitis is the most common gastrointestinal pediatric surgical emergency. With the introduction of laparoscopic techniques in the 1990s, recovery, pain, and hospital stay after laparoscopic procedures have been significantly reduced. While many laparoscopic procedures are performed as outpatient surgeries, pediatric appendectomy patients continue to be hospitalized for postoperative observation. Our goal was to evaluate the safety and feasibility of same day discharge after laparoscopic appendectomy for uncomplicated appendicitis. METHODS: After IRB approval, all pediatric pa…
