A variety of systemic diseases may manifest with laryngeal symptoms. Cricoarytenoid arthritis with or without limitation of vocal fold motion is an example. It has been described in up to 25% of rheumatoid patients. Ankylosing spondylitis is classified among the seronegative spondylarthritides. Besides its systemic features, rare reports of cricoarytenoid involvement have been published. Six previous cases reported have been characterized by a long history of ankylosing spondylitis before the cricoarytenoid joint involvement occurred. We describe only the seventh case of cricoarytenoid arthrit…

Chronic leg ulcers occur in 1% of the adult population with considerable associated morbidity, and community costs estimated at up to 600 million pounds p.a. in the UK. Leg ulcers appear to be relatively common in patients with RA and are widely believed to be resistant to treatment. Such ulcers are often attributed to vasculitis, but little is known of the occurrence of other potential aetiological factors. This article reviews the epidemiology, natural history and aetiology of chronic leg ulcers in RA as well as the therapeutic options available.

Discoveries of the biological properties of bacterial superantigens have given rise to much speculation about their possible role in the pathogenesis of autoimmune diseases. There is an increasing body of data to support these speculations. This field of research has the potential to provide a long-elusive understanding of the mechanisms which underlie microbial involvement in the pathogenesis of autoimmune polyarthritides.

Rheumatoid arthritis (RA), previously considered to be a benign controllable disease with a good prognosis in the majority of patients, is now known to be a severe, progressive disease in terms of radiographic damage, severe functional deterioration, progressive work disability and premature mortality. The traditional approach to RA therapy (from less toxic to more toxic drugs) is inadequate and the risk of drug toxicity is generally overestimated while that of severe disease is underestimated. Consequently, aggressive treatment could be considered in an attempt to reverse the inflammation pri…

Lymphedema is a rare complication of rheumatoid arthritis (RA). Diagnosis is clinical: long-standing, painful swelling of a whole limb in association with RA. Cases described in the literature are predominantly of the upper limbs, sometimes bilateral. Diagnosis can be confirmed by biopsy of lymph nodes, lymphography, or preferably lymphoscintigraphy. The etiology of the edema is unknown. Mechanical obstruction and lymphangitis have been suggested. Pharmacological and surgical treatment of the edema have been disappointing, and treatment of the underlying RA does not improve the lymphedema. Phy…

Fifty-five patients with rheumatoid arthritis were treated with 65 triple arthrodeses of the hindfoot from March 1975 through July 1985. Twelve patients (12 procedures) have died, and follow-up evaluation could not be completed on three patients (four procedures), leaving 40 patients (49 procedures) available for clinical and roentgenographic evaluation. There were 32 women and eight men, with an average age at the time of surgery of 50 years. The follow-up period averaged five years. Standard operative technique involved medial and lateral incisions with staple fixation and local bone graftin…

Injections of local anaesthetics with or without steroids are widespread in the treatment of muscular pain. Occasionally, however, they are followed by serious bacterial infections. Three cases of septic arthritis, one localised to the hip, one to the knee and one to the sacroiliac joint are presented. One should offer great consideration to the possible complications following injections into or near joints.

Six patients with pre-existing rheumatic joint disease presented with overwhelming septicaemia but without overt signs of joint inflammation. Joint aspirates demonstrated multifocal staphylococcal infective arthritis. Despite intensive care all six died from the infection or its immediate sequelae. The contrast between this clinical entity and classical infective arthritis, presenting with one or more swollen, tender joints, is discussed.

Three patients with longstanding seropositive rheumatoid arthritis had fever and marked synovial fluid leukocytosis in multiple joints. No infectious agents were found in synovial fluid or blood, and the patients recovered uneventfully without prolonged antibiotic therapy. Acute, severe exacerbations of synovitis in patients with rheumatoid arthritis may simulate a septic process and involve multiple joints.

Our aim was to examine the efficacy of chloroquine in psoriatic arthritis (PsA) and to assess whether chloroquine therapy exacerbated psoriasis. Thirty-two patients had been given chloroquine therapy while attending the University of Toronto Psoriatic Arthritis Clinic. Twenty-four patients continued therapy for at least 6 months, and 18, or 75%, demonstrated > 30% reduction in the actively inflamed joint count. Moreover, there was a significant decrease in the number of actively inflamed joints. A control group, consisting of 24 patients taking no remittive agents, seen during the same period …

A case of articular sporotrichosis occurring in the wrist and ankle of a 39-year-old man is reported. Review of the literature on this disorder confirms the diagnostic difficulties presented by this usually chronic, progressive, fungal arthritis. The diagnosis of this entity should be considered in the evaluation of any patient presenting with a progressive destructive arthritic process. The pathological and radiological features of a typical case are discussed and the mechanisms of infection and type of individual affected by this disease are described.

Four patients with osteoarthritis of the hip, who developed septic arthritis of the affected joint, are reported. The septic arthritis developed insidiously and was diagnosed with difficulty. One patient died, two required subsequent joint excision, and one arthrodesis despite antibiotics. The presence of a recognised predisposing factor to septic arthritis, such as rheumatoid arthritis or a surgically replaced joint, can provide a diagnostic pointer. These cases suggest that osteoarthritis, similarly, is a predisposing factor. It is concluded that joint sepsis should be considered if a patien…

A survey was conducted to determine the frequency and semiological characteristics of spondylarthropathies encountered in hospital consultat … opean patients. The symptoms of HIV-positive patients were reminiscent of those describing reactive arthritis in such patients. The results of this study contradict the reputed scarcity of ankylosing spondylitis and other spondylarthropathies in black Africa. HIV infection may increase the incidence of reactive arthritis and, as a consequence, that of spond …

An historical review of radionuclide applications to the assessment of synovitis is presented. Contemporary radiopharmaceuticals are grouped into vascular, bone, and inflammatory markers, and their clinical utility and efficacy in disclosing peripheral synovitis and osteoarthritis are surveyed. There is focus on the debate over the usefulness of quantitative sacroiliac joint measurements for screening sacroiliitis, the significance of the negative peripheral joint scan in patients presenting with polyarthralgia, and the suitability of the joint scan to monitor the response to medical treatment…

Although arthralgia is a common complaint in Mediterranean spotted fever, frank arthritis is very unusual. We report two cases of this condition during the course of Mediterranean spotted fever. Synovial fluid was inflammatory in both patients and the disease resolved after doxycycline therapy. Immunofluorescence test for Rickettsia conorii in synovial fluid was negative, suggesting a reactive arthritis.
