Can a valid diagnosis of disruptive behavior disorder be made in preschool children?

OBJECTIVE: Disruptive behavior problems are the most common reason preschool children come to mental health clinics. Yet consensus on the conceptualization and measurement of such problems in young children is lacking. DSM-IV is the most widely used nosologic system for children, but the majority of the validation sample consisted of school-age children and adolescents. It is debatable whether behavioral problems in young children should be considered within a diagnostic framework at all, since normative behavioral disruption occurs during the preschool period. Developing valid methods for ass

[Long-term stability of inpatient cognitive-behavioral therapy of adolescents with anxious-depressed school absenteeism].

90 adolescents (aged 12-18 years) with chronic anxious-depressive school absenteeism with or without comorbid disruptive symptoms underwent an open inpatient treatment. The treatment was manual guided with a cognitive-behavioral focus. In case of long lasting school absence youth were integrated on a special school initially. The present analyses cover 2- and 9-months follow-ups. At discharge continuous school attendance was achieved by 83 (92.2%) adolescents, by 75 (83.3%) at 2-month and by 63 (70.0%) at 9-month follow-up. For youth with school absenteeism within the follow-up interval absenc

Reappraisal is an effective emotion regulation strategy in children with Tourette syndrome and ADHD.

BACKGROUND AND OBJECTIVES: Difficulties in emotion regulation (ER) have been associated with several psychiatric disorders, emphasizing a need for a greater understanding of the concept and its associations with disruptive behavior. We aimed to study the ER strategy of cognitive reappraisal with an experimental test to increase our knowledge of emotional processes in child psychopathology. METHODS: In the present study, we examined emotional reactivity and cognitive reappraisal with a computer task in 160 medication-naïve children aged 8-12 comprising four groups: Fifty-eight children with To

Pharmacological treatment of disruptive behavior in Smith-Magenis syndrome.

Smith-Magenis syndrome (SMS) is a complex genetic syndrome caused by an interstitial deletion of chromosome 17p11.2. Children and adults with SMS appear to have unique neurobehavioral problems that include: sleep disturbance, self-injurious and maladaptive behaviors, stereotypies, and sensory integration disorders. We gathered retrospective psychotropic use information from parents or other caregivers of 62 individuals with SMS who were asked about use of psychotropic medication from a list of commonly used psychiatric medications. For those drugs identified, respondents were asked to rate the

The final hours: a linguistic analysis of the final words of a suicide.

A journal, two suicide notes, and two tape recordings from a young man who took his own life were analyzed by applying a computer program to analyze texts. The analysis indicates a calming trend from the journal entries to the suicide notes to the tape recordings, with an additional increase in positive emotions expressed from the first recorded message to the second recording made just an hour or two before his death. The usefulness of this approach for understanding the psychodynamics of suicidal behavior is discussed.

Child-directed interaction: prediction of change in impaired mother-child functioning.

The first phase of parent-child interaction therapy (PCIT), called child-directed interaction, teaches parents to use positive and differential social attention to improve the parent-child relationship. This study examined predictors of change in mother and child functioning during the child-directed interaction for 100 mother-child dyads. The children were 3-6-years-old and diagnosed with oppositional defiant disorder. After establishing that significant improvements occurred in mother report of child disruptive behavior, parenting stress, and parenting practices, these three variables were c

Bipolar Disorder Hospitalizations and Substance Use Disorders: A Nationwide Retrospective Study From 2008 To 2015.

OBJECTIVE: Bipolar Disorder (BD) is often complicated by co-occurring substance use disorders (SUD). We assessed the prevalence of SUD among BD hospitalization episodes and analyzed its association with hospitalization outcomes. METHODS: We performed a retrospective observational study using a database containing all hospitalizations registered in Portuguese public hospitals from 2008 to 2015. Hospitalizations with a primary or secondary diagnosis of BD were selected. To compare episodes with and without a diagnosis of SUD, an independent sample t-test was used for age, whereas the non-parame

Income gradients within child and adolescent antisocial behaviours.

BACKGROUND: Low income is a widely studied risk factor for child and adolescent behavioural difficulties. Previous research on this relationship has produced mixed findings. AIMS: To investigate the level, shape and homogeneity of income gradients in different types of antisocial behaviour. METHOD: A representative sample of 7977 British children and adolescents, aged 5-16 years, was analysed. Hypotheses concerning the shapes and homogeneity of the relationships between family socioeconomic status and multiple antisocial behaviour outcomes, including clinical diagnoses of oppositional-defian

Sleep in children with disruptive behavioral disorders.

This study compared sleep in patients with Conduct Disorder/Oppositional Defiant Disorder (CD/ODD) and normative children and evaluated the associations between sleep and behavioral symptoms in patients. Participants were 30 patients, aged 7 to 12 years, with diagnoses of CD/ODD and their age and gender matched controls. Patients with CD/ODD and their parents reported significantly more sleep problems than did the control children and their parents (p values < 0.01). By actigraphy, CD/ODD children with comorbid ADHD slept significantly less than did the patients with CD/ODD alone and the contr

Risperidone reduces aggression in boys with a disruptive behaviour disorder and below average intelligence quotient: analysis of two placebo-controlled randomized trials.

The present study aimed to analyse the effect of risperidone on a priori defined core aggression items. Data were pooled from 163 boys (aged 5-12 years, with or without comorbid attention-deficit/hyperactivity disorder) with a DSM-IV diagnosis of either conduct disorder or oppositional defiant disorder who had participated in either of two identical, 6-week, randomized, double-blind, placebo-controlled trials. All received treatment with either placebo or oral risperidone solution (0.01-0.06 mg/kg/day). Subjects had below average intelligence [intelligence quotient (IQ) 36-84] and a score of >

Psychopathy and comorbidity in a young offender sample: taking a closer look at psychopathy's potential importance over disruptive behavior disorders.

The authors examined the construct of psychopathy as applied to 130 adolescent offenders using 3 psychopathy measures and a broad range of DSM-TV Axis I diagnoses and psychosocial problems. Measures used in the study included the following: (a) Psychopathy Checklist-Youth Version, (b) Antisocial Process Screening Device, (c) a modified version of the Self-Report Psychopathy-II scale, and (d) the Adolescent Psychopathology Scale. Results from this study offer incremental support for the construct validity of psychopathy in youth. Psychopathy evidenced better convergent and discriminant validity

A meta-analytic examination of comorbid hyperactive-impulsive-attention problems and conduct problems.

The author quantitatively reviewed prevalence rates, defining features, associated features, developmental trajectory, and etiology to examine 3 taxonomic questions about comorbid hyperactive-impulsive-attention problems (HIA) and conduct problems (CP): Do HIA and CP co-occur randomly? Does comorbid HIA-CP differ from HIA-only and CP-only? Do HIA and CP combine synergistically? Results showed that HIA and CP co-occur at a greater than random rate, that comorbid HIA-CP differs from HIA-only and CP-only in multiple ways, and that there is little evidence that HIA and CP combine synergistically.

CBCL pediatric bipolar disorder profile and ADHD: comorbidity and quantitative trait loci analysis.

OBJECTIVE: The pediatric bipolar disorder profile of the Child Behavior Checklist (CBCL-PBD), a parent-completed measure that avoids clinician ideological bias, has proven useful in differentiating patients with attention-deficit/hyperactivity disorder (ADHD). We used CBCL-PBD profiles to distinguish patterns of comorbidity and to search for quantitative trait loci in a genomewide scan in a sample of multiple affected ADHD sibling pairs. METHOD: A total of 540 ADHD subjects ages 5 to 18 years were assessed with the Schedule for Affective Disorders and Schizophrenia for School-Age Children-Pre

Eating disorder or oesophageal achalasia during adolescence: diagnostic difficulties.

Marine was a fourteen and a half-year-old adolescent female hospitalized for an eating disorder (ED) of the anorexic type with purging behaviors. She has had a complicated life course, made up of disruptions and discontinuities, both family and school. Since the age of five, Marine had been intermittently treated in psychiatry for a diagnosis of oppositional defiant disorder. The current illness started with spontaneous and induced vomiting associated with major weight loss (body mass index, 15.27 kg m-2). The diagnosis of anorexia nervosa was established after several opinions from profession

Prevalence of chronic health conditions in children with intellectual disability: a systematic literature review.

A systematic review of the prevalence rates of chronic health conditions in populations of children with intellectual disability was provided. We identified 2,994 relevant studies by searching Medline, Cinahl, and PsycINFO databases from 1996 to 2008. We included the 31 studies that had sufficient methodological quality. The 6 most prevalent chronic health conditions in children with intellectual disability were epilepsy (22.0/100), cerebral palsy (19.8/100), any anxiety disorder (17.1/100), oppositional defiant disorder (12.4/100), Down syndrome (11.0/100), and autistic disorder (10.1/100). Hypothalamic pituitary adrenal activity and autonomic nervous system arousal predict developmental trajectories of children's comorbid behavior problems.

The combined effects of hypothalamic-pituitary-adrenal (HPA) axis activity and autonomic nervous system (ANS) arousal were examined on developmental trajectories of children's comorbid internalizing and externalizing problems. Participants were 394 urban dwelling, primarily African American, youth (50% male, age 11-12 years). Parent-reported child behavior problems were obtained initially, 3, 6, and 12 months later. Saliva samples (collected at the initial assessment) were assayed for cortisol (HPA) and alpha-amylase (ANS). Cross-domain latent class growth analysis identified a stable comorbid

The relationship between three types of aggression and peer relations in elementary school children.

Previous studies have repeatedly found that aggression causes various internalizing and externalizing problems. Despite the robust relationship, exactly how aggression causes these problems remains unclear, although it is plausible to postulate that this occurs both directly and indirectly (via other behavioural factors). One possible indirect factor might be the aggravation of peer relations. The poor peer relations of aggressive children could make them isolated psychologically or physically from peers, which in turn might result in depressive or disruptive problems. This study examined the
