Acute effects of nicotine withdrawal syndrome in pilots during flight.

BACKGROUND: Pilots who smoke are occasionally obliged to abstain from nicotine intake during flight and may during this period exhibit certain symptoms leading to performance decrement. METHODS: We studied 20 healthy male aviators, who were regular smokers, (mean age 33.7 +/- 1.4 yr) operating military fixed- and rotary-wing aircraft (C-47 Dakota, F-16, A. Bell 205). All pilots were subjected to a 12-h abstinence from cigarette smoking, during which time they performed flight duties. After landing, we studied the intensity of the nicotine withdrawal syndrome, as well as its effect on physiolo

The aeromedical risk associated with asymptomatic cholelithiasis in USAF pilots and navigators.

BACKGROUND: The U.S. Air Force (USAF) aeromedical policy regarding incidentally discovered, asymptomatic cholelithiasis required the aircrew to undergo cholecystectomy prior to being considered for return to flying duties. HYPOTHESIS: The merit of continuing this USAF policy was evaluated at the request of the U.S. Air Force Surgeon General. METHODS: A medical literature review of the natural history of cholelithiasis in the general population was completed. The USAF aircrew waiver file (1972-92) and the Ellingson Aerospace Medicine Consultation Service (ACS) data file (1955-92) were reviewe

Endovascular treatment of bilateral carotid artery pseudoaneurysms after blunt carotid injury.

The detection of blunt carotid artery injures has improved because of more aggressive screening protocols. Initial treatment depends on multiple factors; however, controversy exists with regard to the treatment of pseudoaneurysmal degeneration, especially in this age of endovascular treatment options. Current options include anticoagulation, open surgical repair, and endovascular repair. We report a rare case of bilateral carotid artery pseudoaneurysm degeneration after bilateral carotid artery dissection caused by blunt trauma.

Acute posttraumatic stress symptoms and depression after exposure to the 2005 Saskatchewan Centennial Air Show disaster: prevalence and predictors.

OBJECTIVES: The purpose of this study was to determine the prevalence of acute distress-that is, clinically significant posttraumatic stress symptoms (PTSS) and depression-and to identify predictors of each in a sample of people who witnessed a fatal aircraft collision at the 2005 Saskatchewan Centennial Air Show. DESIGN: Air Show attendees (N = 157) were recruited by advertisements in the local media and completed an Internet-administered battery of questionnaires. RESULTS: Based on previously established cut-offs, 22 percent respondents had clinically significant PTSS and 24 percent had cl

The effectiveness of the parachutist ankle brace in reducing ankle injuries in an airborne ranger battalion.

The purpose of this study was to determine if the parachutist ankle brace (PAB) decreases the number and severity of ankle injuries in an airborne Ranger battalion. A retrospective study was performed covering a 38-month period. A computer database was used to track all jump injuries with a diagnosis of ankle pain, sprain, or fracture. The frequency was calculated for ankle injuries per 1,000 jumps and the average length of medically restricted duty per ankle injury. A total of 13,782 static line parachute jumps were conducted during the study period. Without the PAB, 35 ankle injuries were se

Male reproductive effects of solvent and fuel exposure during aircraft maintenance.

Few studies have addressed the effects of mixed, low-level exposures to complex mixtures on a man's reproductive potential. In this prospective study, each subject was evaluated before first exposure and at 15 and 30 weeks after exposures had begun. A total of 50 men working on aircraft maintenance at an Air Force installation were included in the study. In addition, eight unexposed men were concurrently sampled. Industrial hygiene (IH) sampling and expired breath samples were collected for jet fuel as measured by total napthas, benzene--a component of jet fuel, 1,1,1-trichloroethane, methyl e

Evaluation of trauma care in a developing country highlighted by a major aircraft accident.

The aim of this study was to evaluate the day-to-day trauma care in a developing country highlighted by a major accident. In this accident, early management was not carried out according to triage principles. Scene mortality and in-hospital mortality were 72% (n = 55) and 14% (n = 3), respectively. Overall mortality rate was 76%. Five survivors were minor wounded. Three laparotomies, one thoracotomy and three tube thoracostomies were performed in the acute phase. Skeletal injuries, mainly rib fractures (43.3%) and haemothorax (10.8%), were the most frequent pathologies seen. One liver lacerati

Capacity to Compensate for Central Hypovolemia and Effects of Menstrual Cycle Phases.

BACKGROUND: Tolerance to central hypovolemia is dictated by exhaustion of the physiological capacity to compensate called the compensatory reserve. Such physiological compromise can have detrimental impact on performance in aerospace environments as well as survival from hemorrhage on the battlefield. We induced central hypovolemia using progressively stepwise lower body negative pressure (LBNP) in women during various phases of the menstrual cycle to test the hypothesis that similar tolerance across all menstrual cycle phases would be reflected by similar changes in compensatory reserve.METHO

Skydiving: The audiological perspective.

Skydiving is a popular recreational sport for the young and old. There is minimal research pertaining to skydiving and its relation to the audiological system. The risks of skydiving in relation to the auditory system should be explored further. Aims: The main aim of this study was to explore the relationship between skydiving and audiology in South Africa. The sub-aims of the study focused on determining if skydivers were provided with safety precautions before they commenced with the dive, determining the middle ear pressure before and after the skydive and identifying the audiological sympt

The enhanced recovery after surgery (ERAS) Greenie Board: a Navy-inspired quality improvement tool.

The United States Navy uses a visual feedback system for pilots, named 'the Greenie Board', to improve flight manoeuvres on aircraft carriers. Given that increased compliance with enhanced recovery after surgery protocols reduces postoperative complications, we decided to apply a similar feedback system to our institutional enhanced recovery programme. We undertook a prospective 12-month audit of 194 patients assigned to our enhanced recovery programme and evaluated adherence to the anaesthesia-related components of our protocol, before and after implementing a Greenie Board. A compliance scor

Parachute without a ripcord: the skydive of communication interaction.

Identifying and rating the outcomes of an intervention is not a new concept, but has gained impetus and currency with the emergence of evidence-based practice to support clinical decision making. In this paper, we present a metaphor as a unifying framework for the many different goals and outcomes that may come into focus across extended interventions with individuals who use aided communication. The metaphor is that of skydiving. We explore the value of this metaphor in understanding outcome measures for interventions, using analysis of interview data collected with adults who have used high-

Combining electrodermal responses and cardiovascular measures for probing adaptive automation during simulated flight.

Adaptive automation increases the operator's workload in case of hypovigilance and takes over more responsibility if workload becomes too high. Two consecutive studies were conducted to construct a biocybernetic adaptive system for a professional flight simulator, based on autonomic measures. Workload was varied through different stages of turbulences. In a first study with 18 participants, electrodermal responses of experimental subjects oscillated very close to the individual set point, demonstrating that workload level was adjusted as a result of adaptive control, which was not the case in

Analysis of construction injury burden by type of work.

BACKGROUND: To lay groundwork for identifying patterns of injury etiology, we sought to describe injury experience associated with types of work performed at construction sites by examining workers' compensation (WC) claims for the 32,081 construction workers who built Denver International Airport (DIA). METHODS: Injury rates and WC payment rates were calculated for 25 types of work based on claims and payroll data reported to DIA's owner-controlled insurance program according to National Council on Compensation Insurance job classifications. By linking DIA claims with corresponding lost-work

Comparison of internal dose measures of solvents in breath, blood and urine and genotoxic changes in aircraft maintenance personnel.

Solvents and fuels are in widespread use both in civilian and military populations. 1,1,1-trichloroethane (TCA), xylene, toluene, methyl ethyl ketone (MEK) and methylene chloride are found in a variety of compounds including degreasing agents, paints, coatings, pesticides and paint strippers. Toluene and xylene are also found in fuels, which are complex mixtures of hundreds of agents. The purpose of this investigation was twofold. The first was to determine the optimum medium to measure internal dose of solvents comparing blood, urine and breath. The second was to determine if low level exposu

[Increased association of cardiac and thoracic vascular lesions after closed trauma of the thorax].

The association of a cardiac and a thoracic vascular lesion following blunt trauma seems real based on our experience and other published reports. Over the last 4 years, we have operated upon 3 patients with this association. The cardiac lesions included severe myocardial contusion (documented by electrocardiographic and enzymatic changes), a tear of the aortic valve, and occlusion of a coronary artery. The vascular lesions consisted in rupture of the aortic isthmus in 2 patients and traumatic pseudoaneurysm of the innominate artery. This association justifies a detailed cardiovascular evaluat

Fatigue risk management for cabin crew: the importance of company support and sufficient rest for work-life balance-a qualitative study.

Knowledge about cabin crew fatigue associated with ultra-long range (ULR) flights is still limited. Current ULR scheduling for cabin crew is therefore predominantly based on flight crew data. Cabin crews' views on fatigue, and their strategies for mitigating it, have seldom been sought. To better understand the causes and consequences of cabin crew fatigue, semi-structured focus group discussions were held. Thematic analysis was undertaken with data from 25 cabin crew. Participants indicated that the consequences of fatigue are twofold, affecting 1) cabin crew health and wellbeing and 2) safet

A strategy for in-flight measurements of physiology of pilots of high-performance fighter aircraft.

Some pilots flying modern high-performance fighter aircraft develop "hypoxia-like" incidents characterized by short periods of confusion and cognitive impairment. The problem is serious and recently led to the grounding of a fleet of aircraft. Extensive discussions of the incidents have taken place but some people believe that there is inadequate data to determine the cause. There is a tremendous disconnect between what is known about the function of the aircraft and the function of the pilot. This paper describes a plan for measuring the inspired and expired Po2 and Pco2 in the pilot's mask,
