Radiographic screening at four months of infants at risk for congenital hip dislocation.

We report on a radiographic screening programme at four months of age for infants who were clinically normal at neonatal examination but were considered to be 'at risk' for congenital dislocation of the hip because of their family history, breech presentation, or a persistent click. From a total population of 13,662 live births over a two-year period, 357 (2.6%) infants at risk were identified. Of these 46 had abnormal radiographs (six subluxations, 40 acetabular dysplasia). In 12 infants treatment resulted in a normal hip; 34 required no treatment but were followed up until their radiographs …

Delegation of obstetric care in Indonesia.

An intervention and a control area were selected. Results of the intervention, which consisted of training TBAs, midwives, and health center doctors, were positive inasmuch as there was an increase in referrals of breech presentation, prolonged labor and intrapartum fever in the intervention area. Pregnant women in general were also more often referred in the intervention area as compared to the control area. The same outcome was seen in referrals of neonates weighing under 2000 or more than 4000 g. At the district hospital level, a great number of procedures were left to general practitioners…

Relative Risk and Incidence for Developmental Dysplasia of the Hip.

OBJECTIVE: To determine the incidence and associated risk factors of developmental dysplasia of the hip (DDH) in a modern population without universal screening. STUDY DESIGN: Children with DDH were identified from the Manitoba Centre for Health Policy's Data Repository by the use of International Classification of Diseases diagnosis codes as well as physician billing tariffs for surgical procedures for DDH for all children born between 1995 and 2012. To identify the outpatient-treated patients, ultrasound scans and radiographic imaging for DDH were reviewed for 2004-2012. Overall incidence w

Epidemiology, presentation and management of congenital muscular torticollis.

AIM OF STUDY: Congenital muscular torticollis is a condition of debatable aetiology and management. Untreated, cervical function and facial cosmesis may be severely compromised. The aim of this study was to establish the epidemiology, presentation and management of congenital muscular torticollis in Singapore. PATIENTS: Ninety-one patients with torticollis were seen at the National University Hospital (NUH) from January 1994 to December 1997. Torticollis was first noted at a median age of 2 months with the median age of presentation being 6 months. At presentation, a sternomastoid tumour was

Changing rates of the modes of delivery over the decades (1976, 1986, 1996, 2006, and 2016) based on the Robson-10 group classification system in a single tertiary health care center.

AIM: To identify the underlying factors that may affect the changing rates of modes of delivery over the decades based on Robson-10 group cl … and 2016, respectively. CS has become the preferred method of delivery for multiple pregnancies and breech presentations over the years. TGCS Group 5 was the greatest contributor to increased CS ratios. Relative contributions of TGCS Groups 1 and 3 were decreased and relative contributions of Groups 2 and 4 were increased. Operative vaginal deliveries decreased over …

The use of ultrasound for the diagnosis of spinal hemorrhage in a newborn.

We present a case of an unusually large spinal epidural hematoma in an infant after a total breech extraction, which was diagnosed by high-resolution ultrasonography. The lesion appeared unusually large, homogeneous, and isoechoic to the spinal cord. This case demonstrates the ability of ultrasound to diagnose the nature of the lesion, its exact location and full extent. Magnetic resonance imaging was used in this case only as a secondary modality to demonstrate the hemorrhagic nature of the lesion.

Uterus didelphys without vaginal septum identified intraoperatively at cesarean delivery: a case report.

INTRODUCTION AND IMPORTANCE: Uterus didelphys is a rare Müllerian duct anomaly (MDA), accounting for approximately 10% of congenital uterine malformations. Although often asymptomatic, it may be associated with adverse reproductive outcomes including miscarriage, preterm birth, and malpresentation. CASE PRESENTATION: We describe a 20-year-old primigravida who presented at 39 weeks of gestation for elective cesarean section due to breech presentation. She had no history of infertility, miscarriage, or gynecologic complaints. Pregnancy progressed uneventfully, and a healthy male infant was deli…

Intracranial Tuberculoma in a Pregnant Lady: A Hitherto Unknown Case and a Successful Outcome.

Central nervous system tuberculosis (CNS-TB) is rarely suspected in pregnancy because its clinical presentation may masquerade other common conditions in pregnancy, such as eclampsia. In high tuberculosis endemic areas, CNS-TB should be suspected with a high degree of suspicion among unimmunized and immunocompromised individuals. We hereby report a case of a 32-year-old pregnant woman conceived by in vitro fertilization due to tubal blockage causing infertility, probably due to chronic infection, who presented with a history of multiple seizure episodes without a history of similar complaints

Factors Affecting Elective and Non-elective Cesarean Sections: A Multinomial Regression Analysis Using Robson's Classification in a Southeast European Country.

BACKGROUND: Albania, a middle-income Southeast European country, is experiencing an increase in cesarean section rates. This study aims to analyze cesarean section practices in Albania using the Robson classification to identify patterns and provide insights into elective and non-elective cesarean trends. METHODS: This retrospective cohort study was conducted at the University Hospital of Obstetrics and Gynecology "Koco Gliozheni" in Albania, a leading tertiary hospital, from January to May 2023, involving 5,315 consecutive women who delivered during this period, including both live births an

What Risk Factors and Characteristics Are Associated With Late-presenting Dislocations of the Hip in Infants?

BACKGROUND: Most infants with developmental dysplasia of the hip (DDH) are diagnosed within the first 3 months of life. However, late-presenting DDH (defined as a diagnosis after 3 months of age) does occur and often results in more complex treatment and increased long-term complications. Specific risk factors involved in late-presenting DDH are poorly understood, and clearly defining an associated set of factors will aid in screening, detection, and prevention of this condition. QUESTIONS/PURPOSES: Using a multicenter database of patients with DDH, we sought to determine whether there were d

Bilateral upper-limb palsy of the newborn infant: Taxonomy, differential diagnosis, and clinical pathways.

Bilateral reduction or absence of upper-limb movement in a newborn infant is a rare but critical presentation that cannot be anchored on the unilateral paradigm of brachial plexus birth injury (BPBI). Bilaterality substantially widens the differential to include cervical spinal cord injury, neuromuscular disorders, and non-neurological movement limitation; thus, default attribution to bilateral BPBI risks missing a serious central pathology. No standardized terminology or investigation framework currently addresses this phenotype. This review introduces bilateral upper-limb palsy of the newbor

Targeted US Screening in High-Risk Newborns and Infants: Indications, Techniques, and Disease Findings.

Newborns and infants with certain perinatal exposures, congenital conditions, and genetic syndromes are at increased risk for clinically significant diseases and may benefit from early detection. In these children, imaging serves as a targeted screening tool to identify actionable abnormalities during a window in which intervention may reduce morbidity and long-term sequelae. The authors summarize evidence-based US screening strategies for high-risk newborns and infants encountered in clinical practice, including preterm infants, newborns with a breech presentation, children with congenital an…
