Electrogram transition patterns in left bundle branch pacing: a case report.

INTRODUCTION: Left bundle branch (LBB) pacing (LBBP) demonstrates clinical potential but faces challenges in confirming selective capture via dynamic electrogram (EGM) criteria. CASE REPORT: A 69-year-old male with a complete atrioventricular block underwent LBBP implantation. Real-time EGM monitoring (high-pass/low-pass filters: 200/500 Hz) revealed an abrupt shortening of stimulus-to-V6 R-wave peak time (Sti-V6 RWPT) from 87 to 69 ms during lead deployment, indicating a transition from septal pacing to LBBP. Output reduction (1.6 V → 1.5 V/0.5 ms) eliminated myocardial excitation notches, y

Real-world adoption of left bundle branch area pacing: Insights from the Conduction-System pacing Italian Network Group (C-SING).

BACKGROUND AND OBJECTIVE: Left bundle branch area pacing (LBBAP) is increasingly used for treating bradycardia and heart failure. However, real-world data are limited. The Conduction-System Pacing Italian Network Group (C-SING) collected prospective data on LBBAP procedures in a nationwide context. METHODS: Observational data from 28 Italian sites were analysed for consecutive LBBAP procedures, comparing outcomes based on operator experience levels. RESULTS: From January 2022 to December 2023, 1250 patients (median age 78, 66.2 % male) underwent LBBAP attempt. Most frequent indications were

Right sided approach for left bundle branch pacing using lumen-less lead: Technical considerations and follow-up outcome.

INTRODUCTION: Left bundle branch pacing has gained significant momentum in the last few years. The procedure involves deploying the lead deep inside the interventricular septum through left subclavian vein. We aimed at analyzing the feasibility, efficacy and long-term outcome of left bundle branch pacing (LBBP) using lumen-less lead through the right subclavian vein. METHODS: This was a retrospective-institutional, single center observational study done in consecutive patients who underwent LBBP using 3830 selectsecuretm lead. Left subclavian venous access was the primary strategy for lead im

Rescue left bundle branch area pacing in coronary venous lead failure or nonresponse to biventricular pacing: Results from International LBBAP Collaborative Study Group.

BACKGROUND: Cardiac resynchronization therapy (CRT) using biventricular pacing (BVP) is effective in patients with heart failure, left bundle branch block (LBBB), and reduced left ventricular function. Left bundle branch area pacing (LBBAP) has been reported as an alternative option for CRT. OBJECTIVE: The purpose of this study was to assess the feasibility and outcomes of LBBAP in patients who failed conventional BVP because of coronary venous (CV) lead complications or who were nonresponders to BVP. METHODS: At 16 international centers, LBBAP was attempted in patients with conventional CRT

Left Bundle Branch Area Pacing versus Biventricular Pacing for Cardiac Resynchronization Therapy on Morbidity and Mortality.

BACKGROUND: Left bundle branch area pacing (LBBAP) has emerged as an alternative to biventricular pacing (BVP) for cardiac resynchronization therapy (CRT). We aimed to compare the morbidity and mortality associated with LBBAP versus BVP in patients undergoing CRT implantation. METHODS: Consecutive patients who received CRT from two high-volume implantation centers were retrospectively recruited. The primary endpoint was a composite of all-cause death and heart failure hospitalization, and the secondary endpoint was all-cause death. RESULTS: A total of 491 patients receiving CRT (154 via LBBA

Feasibility of RA-LV pacing in patients with symptomatic left bundle branch block: a pilot study.

Several studies have reported the adverse effects of right ventricular apical pacing. Permanent His bundle pacing is proved to be the most physiological. But it can be technically difficult sometimes. One recent large multicenter randomized trial showed that pacing from left ventricular apex or mid-lateral wall has the greatest potential to prevent pacing-induced reduction of cardiac pump function (by maintaining left ventricular mechanical synchrony) and, therefore, can be considered as physiological site. In our study, we have wanted to see the outcome of left ventricular pacing through coro

Demonstration of posterior fascicle to myocardial conduction block during ablation of idiopathic left ventricular tachycardia: an electrophysiological predictor of long-term success.

BACKGROUND: Idiopathic left ventricular tachycardia (ILVT) is a common form of ventricular tachycardia (VT) in structurally normal heart. Different methods have been proposed for radiofrequency ablation (RFA) of ILVT that have good short-term results but recurrence is higher. Termination of tachycardia during RFA and/or noninduciblity has been the procedural end point. OBJECTIVE: To describe electrophysiological markers that add to long-term freedom from recurrences. METHODS: Fifteen patients with ILVT underwent RFA guided by 3-dimensional electroanatomical mapping. After creating a 3-dimens

Incidence and site of atrioventricular block in patients with chronic bifascicular block.

Four hundred fifty-two patients with chronic bifascicular block and initially intact atrioventricular (AV) conduction were detected, studied, and prospectively followed between January 1970 and March 1978. There were 360 males and 92 females, ages 18--93 years (mean +/- SD, 62 +/- 15 years). Follow-up ranged from 29-2804 days (mean 1066 +/- 97 days). AV block (2 degrees or 3 degrees) developed in 29 patients, nine with apparent cause and spontaneously in 20. Cumulative annual incidence of all heart block for 1--5 years was, respectively, 4%, 5.9%, 8.7%, 10.1% and 11.3%, and for spontaneous blo

Mechanisms determining the ventricular rate in wolff-parkinson-white arrhythmias.

In patients with WPW syndrome the maximal ventricular rate attained during ectopic rapid supraventricular thythms depended on the type of arrhythmia as well as on the physiological properties of the AP. During reciprocating tachycardias the impulse is almost invariably conducted to the ventricles through the AV node. Therefore, the maxiaml ventricular rate is a function of the AV nodal ERP. On the other hand, when atrial flutter or atrial fibrillation were present the ventricular rate could be moderately elevated (when the ERP of the AP was longer than that of the AV node) or very rapid where

Supraventricular tachycardia and sinus rhythm with contralateral bundle branch block patterns.

A contralateral bundle branch block (BBB) aberration during tachycardia with a preexisting BBB strongly suggests the presence of ventricular tachycardia. We report on a middle-aged, female patient presented with wide QRS tachycardia. The patient had orthodromic atrioventricular tachycardia with a left BBB aberration in the presence of a preexisting right BBB due to an abnormal His-Purkinje system. We learned that the contralateral BBB aberration with supraventricular tachycardia could be seen when the His-Purkinje system was abnormal.

Conduction System Pacing "How To": Tips and Tricks.

Conduction system pacing (CSP), encompassing His bundle pacing and left bundle branch area pacing, has emerged as a physiological pacing technique designed to activate the heart's intrinsic conduction system. CSP is a promising alternative to traditional right ventricular pacing for bradycardia and to biventricular pacing for cardiac resynchronization therapy. This article outlines key considerations for achieving successful CSP implantation, including procedural techniques, available tools, and programming strategies.

Permanent conduction system pacing for congenitally corrected transposition of the great arteries: A Pediatric and Congenital Electrophysiology Society (PACES)/International Society for Adult Congenital Heart Disease (ISACHD) Collaborative Study.

BACKGROUND: Congenitally corrected transposition of the great arteries (CCTGA) is associated with spontaneous atrioventricular block and pacing-induced cardiomyopathy. Conduction system pacing is a potential alternative to conventional cardiac resynchronization therapy (CRT). OBJECTIVE: The purpose of this study was to determine the outcomes of conduction system pacing for CCTGA. METHODS: Retrospective data were collected from 10 international centers. RESULTS: His bundle (HBP) or left bundle branch pacing (LBBP) was attempted in 15 CCTGA patients (median age 23 years; 87% male). Previous s

Conduction System Pacing-Optimized Cardiac Resynchronization Therapy for Congenitally Corrected Transposition.

Patients with congenitally corrected transposition of the great arteries (CCTGA) are predisposed to atrioventricular block, pacing-induced cardiomyopathy, and right ventricular (RV) failure. Although biventricular cardiac resynchronization therapy (CRT) and conduction system pacing (CSP) have been described, RV failure may persist. Three cases of CCTGA with intrinsic RV electrical delay are presented where conventional CSP targeting the His bundle or proximal left bundle branches resulted in incomplete electrical resynchronization. With CSP-optimized CRT, there was greater electrical synchrony

Amiodarone Induced Thyrotoxicosis - Fluctuating RVOT and LV Scar VT.

A 61 year old patient with non-ischemic cardiomyopathy and implantable cardioverter defibrillator presented with multiple shocks for ventricular tachycardia (VT). EKG revealed monomorphic sustained VT which was left bundle inferior axis that spontaneously changed into sustained VT which was right bundle superior axis. This was suggestive of an outflow tract VT transforming into a VT probably related to reentry from LV scar. The patient was transferred to our university for VT ablation. However, further investigation revealed amiodarone induced hyperthyroidism which was the cause of his ventric

Radiofrequency ablation on the right ventricular septum changed a bundle branch block pattern of a ventricular tachycardia: What is the mechanism?

A ventricular tachycardia (VT) with a left bundle branch block (BBB) pattern exhibited the earliest activation (EA) at the left ventricular basal septum near the His bundle with no excellent pace map (PM). Radiofrequency ablation at the right ventricular basal septum (opposite site of the EA site) changed the QRS morphology of VT to a right BBB pattern that matched a PM at the opposite site in the left ventricle. VT ablation was successful at the earliest activation site. The VT should have originated from an intramural origin with preferential pathways to the endocardial breakout sites in the

Atrioventricular conduction disturbance characterization in transcatheter aortic valve implantation with the CoreValve prosthesis.

BACKGROUND: Atrioventricular (AV) block is one of the most frequent complications of CoreValve transcatheter aortic valve implantation (TAVI). The aim of this study was to analyze the effects of CoreValve implantation on AV conduction. METHODS AND RESULTS: Electrophysiological study was performed immediately before and after CoreValve implantation in 18 consecutive, permanent pacemaker-free patients. An electrode was placed on the His bundle during valve implantation, and data were continuously recorded during the procedure. With surface ECG, a median (first, third quartile) QRS width of 96 (

Severe involvement of the conduction system in a patient with sclerodermal heart disease. An electrophysiological study.

A case of progressive systemic sclerosis with syncopal symptoms is reported. The presenting ECG pattern was that of an anterior myocardial infarction. The clinical history and the coronary angiography excluded significant coronary atherosclerotic heart disease. The ECG pattern evolved from the infarctual pattern associated with right bundle branch block to probably major degree of right bundle branch block associated with left posterior fascicular block. M-mode echocardiography, heart catheterization and angiographic studies did not reveal significant mechanical impairment of the left or right

Transient right bundle branch block: A rare manifestation in cardiac contusion.

Transient right bundle branch block following blunt cardiac injury is a known but under-recognized manifestation of cardiac contusion. The first case documented in the medical literature occurred in 1952 in a 22-year-old man who was thrown from a motorcycle. Due to their relatively anterior location, the right ventricle and right bundle branch are at particular risk of injury in contusion. We present here a case in which a 24-year-old man suffered a blunt chest trauma leading to a right bundle branch block and elevated troponin levels, consistent with cardiac contusion. His conduction system a

Swallowing-induced paroxysmal supraventricular tachycardia.

A 54-year-old man had palpitations on swallowing without any esophageal or heart disease. Electrophysiological findings, including an A wave in the high right atrial leads appearing prior to the A wave on His bundle electrogram, revealed that the arrhythmia was paroxysmal supraventricular tachycardia originating in an ectopic focus of the atrium with intraventricular aberration. Treatment with verapamil, 120 mg/day, reduced his symptoms in spite of an insignificant decrease in the arrhythmia observed with Holter dynamic electrocardiography.
