Transverse tubule calcium regulation in malignant hyperthermia.

Transverse tubule (TT) calcium transport and permeability were examined in the inherited skeletal muscle disorder malignant hyperthermia (MH). ATP-dependent calcium uptake by TT vesicles isolated from normal and MH-susceptible (MHS) pig muscle had a similar dependence on ionized Ca2+ concentration (K1/2 for Ca2+ of 0.21 +/- 0.04 and 0.25 +/- 0.05 microM for MHS and normal TT, respectively), as well as a similar Vmax (20.9 +/- 2.0 and 23.7 +/- 4.5 nmol Ca/mg protein/min for MHS and normal TT, respectively). Furthermore, the stimulation of calcium uptake by either calmodulin or cAMP-dependent pr…

Low urinary calcium excretion in Bartter's syndrome.

The urinary calcium excretion has been determined in 19 patients with Bartter's syndrome and found to be significantly lower than the calcium excretion in 92 healthy subjects (1.16 +/- 0.82 vs. 4.36 +/- 2.71 mmol/24 h, p less than 0.001). There were no differences in height, weight, glomerular filtration rate, urinary sodium excretion or serum calcium concentration between the patients and the control subjects to account for the disparity in calcium excretion. In the patients, the concentrations for ionized calcium, PTH, 25-OH vitamin D and 1,25-(OH)2 vitamin D were normal. A low urinary calci…

Calcinosis cutis following extravasation of calcium chloride.

A 27-year-old woman with hypoparathyroidism developed multiple firm white-yellow papules along the path of an infiltrated calcium chloride intravenous infusion. A biopsy specimen obtained ten days after the extravasation revealed an urticarial reaction. A subsequent biopsy specimen, obtained 25 days after the extravasation, showed diffuse dermal calcification, confirmed by roentgenographic analysis, with incipient transepidermal elimination. A biopsy specimen obtained 40 days after the extravasation was consistent with an elimination reaction. An increase in mast cells was not noted. Electron …

Gynecomastia associated with calcium channel blocker therapy.

Gynecomastia is a reaction that is not usually associated with calcium channel blocker therapy. The Division of Epidemiology and Surveillance, within the US Food and Drug Administration (Rockville, Md), has received 31 reports of gynecomastia occurring after the use of these drugs. The underlying mechanism of this reaction is unknown, although two patients did report elevated prolactin levels. Gynecomastia developed in two patients, resulting in the surgical removal of the breast and/or nodules before discontinuation of drug therapy. The possibility of an association between calcium channel bl…

Calcium antagonists for chronic stable angina pectoris.

Stable angina pectoris classically occurs on exertion or in response to other well-defined stress, and can be treated successfully, both in regard to symptoms and ability to undertake more exertion, with available calcium antagonists. Numerous reports suggest that response to calcium antagonists is similar to that with beta-adrenergic blockers, although the latter tend to show somewhat greater efficacy. Advantages in favor of calcium antagonists include the relative freedom from side effects that may occur with beta-adrenergic antagonists; the 2 types of substances can be combined usefully and…

Relationship between dystonia and serum calcium levels.

The relationship between acute dystonic reactions and serum calcium levels in 17 acutely psychotic patients was studied. Previous studies have implicated an association between a lowered serum calcium value and acute dystonia. This study failed to note a significant incidence of hypocalcemia in a group of psychotic patients. It is not known whether other factors related to calcium-magnesium balance may be related to dystonia.

Chronic cerebral vasospasm: effect of calcium antagonists.

The efficacy of the calcium antagonists nifedipine and nimodipine was evaluated in a multiinjection canine model of severe chronic cerebral vasospasm. Each of 21 adult mongrel dogs (15 to 20 kg) was assigned to one of four experimental groups. All animals received a total of 15 ml of fresh, unheparinized arterial blood via three cisterna magna injections. Selective left vertebral arteriography was performed 1 week before and exactly 7 days after the simulated subarachnoid hemorrhage. Treatment with calcium antagonists was initiated at 24 hours and continued for 7 consecutive days. Comparisons …

Cyanide-induced neurotoxicity: role of neuronal calcium.

The effect of cyanide on whole-brain calcium levels was determined in mice administered KCN and correlated with the neurotoxic signs manifested during acute cyanide poisoning. KCN (10mg/kg, sc) significantly increased whole-brain total calcium levels from 48.1 +/- 1.8 to 66.5 +/- 3.9 micrograms/g dry wt within 15 min after administration. The levels remained elevated for 3 hr and returned to control readings after 12 hr. Dose-response studies revealed KCN, at doses of 10-15 mg/kg, produced significant elevations of whole-brain calcium 30 min after administration. No measurable effect was obtai…

[Calcium oxalate microcrystalline arthropathy in primary oxalosis].

This paper dealt with the case of a 53 years old man, affected by a chronic renal failure as the initial symptom of a primary oxalosis and treated by hemodialysis three years ago. Two years after the onset of renal failure, the left knee was painful and swollen but no cartilage or bone joint lesion was observed. Presence of intra synovial calcium oxalate crystals suggests that this arthropathy may be related to the primary oxalosis. However the role of other calcium salts under identification evidenced by synovial electron microscopy (apatite ? pyrophosphate ?) is discussed.

Renal interactions between norepinephrine and calcium antagonists.

The combined action of norepinephrine and calcium antagonists on the glomerular filtration rate (GFR) of isolated rat kidneys perfused at a constant pressure of 105 mm Hg was studied. Norepinephrine, 74 +/- 9 ng/ml (mean +/- SEM), increased the renal vascular resistance (RVR) by 50% and GFR declined by 88% (P less than 0.001). The subsequent addition of either diltiazem or verapamil (5 microM) returned RVR nearly to control and increased the GFR by an average of 86% over control (P = 0.007). Diltiazem or verapamil alone had little effect on RVR or GFR. Norepinephrine (20 ng/ml) did not affect …

Cellular action of calcium channel blocking drugs.

The calcium antagonists are an exciting "class" of drugs that are already making an important impact in the treatment of coronary artery disease. All three drugs in use in the US, verapamil, nifedipine, and diltiazem, are effective coronary vasodilators. In addition, verapamil and diltiazem have antiarrhythmic activity against certain supraventricular abnormalities. Since the calcium antagonists dilate peripheral resistance vessels, they may also be useful in hypertension. There are reports also that they may be effective in migraine headache, probably because of their "antispastic" activity i…

Calcium-activated potassium channels in liver cells.

Activation of certain membrane receptors increases the concentration of Ca2+ in the cytosol of hepatocytes. Since in most species these cells possess a PK(Ca) mechanism, the outcome is a rise in PK. This can be blocked by quinine, apamin and certain neuromuscular blocking agents. The binding of labelled apamin to hepatocytes has been studied under physiological conditions, and the relationship between the binding sites and K+ channels is discussed. The physiological role of the PK(Ca) mechanism in hepatocytes is unclear, though it is largely responsible for 'adrenaline hyperkalaemia'.

[Calcium-phosphate metabolism in pre-term infants].

Physiological and clinical aspects are discussed in this review on calcium-phosphate metabolism in pre-term infants. Calcium accumulation in the bone mass of the foetus is related to the gestational age, and mainly occurs during the last weeks of gestation. Therefore, after birth, hypocalcemia is more frequent in pre-term than term infants. However, clinical symptoms of hypocalcemia, e.g. attacks of apnea, hyperexcitability and hypotonia, are rarely observed. Such symptoms depend upon the serum concentration of ionized calcium and this concentration is influenced by various metabolic factors. …

Endocrine regulation of calcium homeostasis during pregnancy.

The principal maternal physiologic adjustment with respect to calcium metabolism is increasing PTH secretion, which maintains the serum ionic calcium level within its characteristically narrow physiologic limits in the face of an expanding extracellular fluid volume, increased urinary excretion, and calcium transfer to the fetus. Additionally, PTH promotes increased renal synthesis of 1,25-(OH)2D3, which acts in concert with PTH to meet the calcium demands of gestation. Whether or not calcitonin secretion increases as well is not clear; if so, this effect may be important in protecting the mat…

Calcium phosphate deposition disease in great danes.

A new clinicopathologic syndrome, possibly familial, in Great Dane dogs, resembles the familial childhood variant of calcium pyrophosphate dihydrate deposit disease in man, except that the mineral deposits were composed of amorphous calcium phosphate or hydroxyapatite rather than pyrophosphate. The syndrome was characterized clinically by paraplegia and incoordination in very young puppies which was caused by concentric constriction of the posterior cervical spinal cord. Canal stenosis resulted from dorsal displacement of the seventh cervical vertebra and deformation of the vertebral articular…
