Cervicovagopathy: ligamentous cervical instability and dysstructure as a potential etiology for vagus nerve dysfunction in the cause of human symptoms and diseases.

Vagus nerve dysfunction is implicated in the pathophysiology of many different symptoms and diseases that plague humanity. In many cases, the etiology of this condition remains elusive. One potentially implicating factor is cervical spine pathology, as the 2 vagus nerves are located in the carotid sheath just anterior to the cervical vertebrae. We propose that cervicovagopathy occurs primarily by the slow stretching of the posterior cervical ligaments because of a forward head-facedown lifestyle from excessive cell phone and computer usage. While the excessive stretch and compression on the va…

Cervical Disc Arthroplasty Device Failure Causing Progressive Cervical Myelopathy and Requiring Revision Cervical Corpectomy.

BACKGROUND: Cervical disc arthroplasty is a well-established alternative to anterior cervical fusion but requires precise placement for optimal outcomes. We present the case of a 2-level cervical disc arthroplasty with suboptimal implantation of the interbody devices, requiring revision corpectomy. Supplemental video, Supplemental Digital Content 1 ( http://links.lww.com/CLINSPINE/A358 ) content of the revision surgery is also provided. This report highlights the importance of proper implant sizing and position and reviews the nuances of surgical revision. METHODS: A retrospective review of t

Dental surgery and anesthetic precautions of a patient with Down syndrome and juvenile rheumatoid arthritis: a case report.

Down syndrome and juvenile rheumatoid arthritis have been associated with ligament laxity, specifically the atlantoaxial ligament, which maintains the proper positioning of the cervical first and second vertebrae. The joint consists of different pathological processes, and it is paramount that individuals with either condition be screened appropriately before surgery is contemplated. The purpose of this paper was to present a case of an individual with both conditions and describe precautionary measures that were undertaken to safely complete dental treatment under general anesthesia and avoid

Subarachnoid hemorrhage and spinal root injury caused by acupuncture needle--case report.

The authors report a case of subarachnoid hemorrhage and spinal root injury caused by an acupuncture needle buried in the posterior neck about 30 years before onset. A 33-year-old female presented with sudden onset of severe occipital headaches. Plain x-ray films of the cervical spine revealed a fine gold needle, about 1.5 cm in length, between the C1 and C2 vertebrae. The needle was piercing the spinal nerve root through the dural vein, and was removed. Postoperatively, the pain exacerbated by neck movement disappeared.

Diagnosis and surgical management of an extraskeletal osteosarcoma of the brachial plexus in a Belgian shepherd dog.

An 11-year-old Belgian shepherd presented with progressive right thoracic limb lameness. Computed tomography and magnetic resonance imaging revealed a brachial plexus neoplasm and cytology was suggestive of a sarcoma. The patient underwent forequarter amputation and hemilaminectomy of the fifth cervical to first thoracic vertebrae with nerve root transection. Histopathology confirmed extraskeletal osteosarcoma. Twelve months post-operatively, the dog was exercising normally, and computed tomography did not identify local recurrence or metastatic disease. This is the first recorded case of cani

Three different mechanisms of death: an unusual form of a child murder by asphyxia.

We report a very unusual case of murder of a 4-year-old male white child who died of asphyxiation. Asphyxia occurred due to 3 factors: manual strangulation, hyperextension of the neck, and atlantoaxial subluxation. The offenses were carried out by a single assailant (the stepfather of the child) who strangled the child with his right hand, using his left hand to pull the hair of the child, forcing the head back and causing hyperextension of the neck, thereby dislocating the first and second cervical vertebrae.

Extensive spinal epidural hemorrhage associated with von Willebrand's disease--a case report.

We have experienced a case of spontaneous spinal epidural hemorrhage associated with von Willebrand's disease. The patient was repeatedly attacked by bleeding of the urinary tract, intramuscular hemorrhage of the right thigh in spite of the administration of AHF. She had a complication of pneumonia and died on the 94th day of hospitalization. The autopsy revealed extensive spinal epidural hemorrhage from the lower part of the first cervical to the 11th thoracic vertebrae. Because of its rarity in previous literature, this case is presented.

Severe chest polytrauma and vertebral number variation of an old woman from the Early Middle Ages.

This study describes a case of polytrauma of the axial skeleton accompanied by a spinal anatomical variation of an early medieval old female skeleton (dated to the 8th-9th century AD), with the purpose of clarifying the origin of the condition and to understand its impact on the life quality of the individual. The skeleton was subjected to macroscopic and X-ray examination. An anthropological analysis discovered the skeleton is well preserved, gracile, and with significant traces of physical activity. A detailed survey of the spine revealed there are six cervical, 12 thoracic, and six lumbar v

Contiguous discitis and osteomyelitis in children.

Magnetic resonance imaging of 16 patients with contiguous discitis and osteomyelitis provided a specific diagnosis and defined the anatomic extent of vertebral and soft-tissue involvement. Altered signal changes were evident in the disc, adjacent vertebra in the end plate and metaphyseal equivalent regions, and the anterior prevertebral tissues. Significant posterior spread and disc herniation were not evident. Fourteen patients had lumbar involvement; two had cervical involvement. The patients were followed-up for an average of 4 years 5 months. Scoliosis has developed in one patient, and fou

Finite element modeling to predict the influence of anatomic variation and implant placement on performance of biological intervertebral disc implants.

BACKGROUND: Tissue-engineered intervertebral disc (TE-IVD) constructs are an attractive therapy for treating degenerative disc disease and have previously been investigated in vivo in both large and small animal models. The mechanical environment of the spine is notably challenging, in part due to its complex anatomy, and implants may require additional mechanical support to avoid failure in the early stages of implantation. As such, the design of suitable support implants requires rigorous validation. METHODS: We created a FE model to simulate the behavior of the IVD cages under compression

[Pseudoluxation C2-C3 in childhood].

Cervical spine injury is rare in children. It is seen primarily in those who sustain significant, severe blunt trauma, occurring in 1 to 2 percent of such cases. The interpretation of cervical spine radiographs and the clinical examination in children may be difficult. The objective of this case clinic is to report on a physiological radiographic finding of the cervical spine in childhood that trends to be misjudged as pathologic: C2-C3 pseudosubluxation. We presented a boy, aged 5 years, who came to our Traumatic pediatric emergency center for cervical pain after falling from a height. In late

[Mediastinal bronchogenic cyst associated with a multiple malformation syndrome].

The authors report a case of a patient presenting with a bronchogenic mediastinal cyst, which presented as a super-infection of the cyst. The cyst was associated with malformation of the cervical vertebra, cardiovascular abnormalities and congenital deafness, placing this in the group of rare complex polymalformation syndrome with Klippel-Feil Syndrome. The authors also stress the value of computed tomography, and above all of nuclear magnetic resonance in the analysis of mediastinal tumours and in particular, of bronchogenic cysts.

Case report: Focal heterotopic ossification in paravertebral muscles as a cause of neurogenic lameness in a dog.

This case report describes a 17-month-old Pudelpointer with recurring motor impairment localized to the left thoracic limb. A neurological exam highlighted lameness in that limb, accompanied by pre-scapular swelling. Radiographs and magnetic resonance imaging detected an osseous structure in soft tissues close to the fifth cervical vertebra, and subsequent surgery uncovered adjacent cervical spinal nerve impingement. Histology of the bony structure revealed heterotopic ossification in paravertebral muscles. Mild bone re-formation at the operating site was detected after a 2-year period, but th

Surgical removal of a microchip from a puppy's spinal canal.

A 1.6 kg, six-week-old Tibetan Terrier was admitted with a 12-hours history of acute onset of progressive tetraparesis following insertion of a microchip to the dorsal cervical region. Neurological examination indicated a lesion to the Ce(1) to Ce(5) spinal cord segments. Radiographic examination confirmed the intra-spinal location of a microchip foreign body at the level of the second cervical vertebra. Microchip removal was achieved following dorsal hemi-laminectomy; significant intra-operative haemorrhage was encountered. The puppy was ambulatory at day seven. Follow-up telephone interview
