Management of Carpal Tunnel Syndrome.

The American Academy of Orthopaedic Surgeons (AAOS) has developed Appropriate Use Criteria (AUC) for Management of Carpal Tunnel Syndrome. Evidence-based information, in conjunction with the clinical expertise of physicians, was used to develop the criteria to improve patient care and obtain best outcomes while considering the subtleties and distinctions necessary in making clinical decisions. To provide the evidence foundation for this AUC, the AAOS Evidence-Based Medicine Unit provided the writing panel and voting panel with the 2016 AAOS Clinical Practice Guideline titled Management of Carp

Decision aids for people considering taking part in clinical trials.

BACKGROUND: Several interventions have been developed to promote informed consent for participants in clinical trials. However, many of these interventions focus on the content and structure of information (e.g. enhanced information or changes to the presentation format) rather than the process of decision making. Patient decision aids support a decision making process about medical options. Decision aids support the decision process by providing information about available options and their associated outcomes, alongside information that enables patients to consider what value they place on p

Treating Vertebral Compression Fractures: Establishing the Appropriate Diagnosis, Preoperative Considerations, Treatment Techniques, Postoperative Follow-Up and General Guidelines for the Treatment of Patients With Symptomatic Vertebral Compression Fractures.

Vertebral compression fractures (VCFs) result from either trauma or a pathologic process that weakens the bone by conditions such as osteoporosis or tumor. The incidence of VCFs has been rising over the last few decades in accordance with the aging population. These fractures can result in severe pain, physical limitation and disability, as well as increased morbidity and mortality. Patients with VCFs are optimally treated by accurate and early diagnosis and treatment. An effective method to treat these fractures is percutaneous vertebral augmentation, which is a set of minimally invasive proc

Supporting the Development of Clinical Reasoning of Preprofessional Novices in Dysphagia Management.

Proficient clinical reasoning is a critical skill in high-quality, evidence-based management of swallowing impairment (dysphagia). Clinical reasoning in this area of practice is a cognitively complex process, as it requires synthesis of multiple sources of information that are generated during a thorough, evidence-based assessment process and which are moderated by the patient's individual situations, including their social and demographic circumstances, comorbidities, or other health concerns. A growing body of health and medical literature demonstrates that clinical reasoning skills develop

Developing a clinical decision tool to support paramedics when assessing and managing children with minor head injury.

BACKGROUND: Head-injured children are commonly transported to the Emergency Department (ED) by ambulance. However, most of those conveyed are deemed non-serious and are discharged at triage. Hospital clinicians use clinical decision tools to support their assessment of head-injured children; however, this is generally to determine whether a computed tomography (CT) scan is indicated. Currently, there is no pre-hospital clinical decision tool designed to support paramedics when assessing and managing head-injured children at scene. The aim of this study was to determine consensus amongst expert

Accurate and rapid screening model for potential diabetes mellitus.

BACKGROUND: Prediction or early diagnosis of diabetes is crucial for populations with high risk of diabetes. METHODS: In this study, we assessed the ability of five popular classifiers (J48, AdaboostM1, SMO, Bayes Net, and Naïve Bayes) to identify individuals with diabetes based on nine non-invasive and easily obtained clinical features, including age, gender, body mass index (BMI), hypertension, history of cardiovascular disease or stroke, family history of diabetes, physical activity, work stress, and salty food preference. A total of 4205 data entries were obtained from annual physical exa

Generating implementation evidence in clinical trials of vaccines and immunization-related technologies to reduce evidence-to-policy delays.

Evidence-to-policy delays pose a significant threat to the timely adoption of novel vaccine products despite their proven efficacy. Understanding the "how to" (i.e., implementation aspects) of innovative vaccine products can facilitate decision-making to fast-track prioritization and introduction. This perspective highlights the need to integrate implementation research within clinical trials of vaccines and immunization-related technologies to facilitate the generation of policy-relevant implementation evidence. We argue that implementation context, mechanisms, strategies, adaptation, and tra

AI-Assisted Dialysis Decision-Making: Assessing Agreement Between ChatGPT and Nephrologist in Initial Dialysis Indication and Prescription in Emergency and ICU Settings.

BACKGROUND Artificial intelligence (AI) is increasingly explored as a clinical decision-support tool in nephrology; however, its real-world applicability for dialysis decision-making in emergency and intensive care unit (ICU) settings remains insufficiently studied. Hemodialysis initiation and prescription are complex, time-sensitive, and dynamic processes that require expert clinical judgment. MATERIAL AND METHODS This retrospective observational study evaluated agreement between AI-generated (ChatGPT) and nephrologist-made dialysis decisions in emergency and ICU settings. Adult patients unde

Beyond the binary: a nationwide study of physician decision orientations toward artificial hydration in terminal cancer care.

BACKGROUND: Artificial hydration (AH) in terminally ill cancer patients remains ethically and clinically controversial. Although evidence suggests limited benefit near the end of life, AH is frequently administered, particularly in East Asian settings where it may carry symbolic meaning. This study conceptualized physician decision-making as a clinical spectrum and examined demographic, clinical, and ethical factors associated with AH decision orientations. MATERIALS AND METHODS: A nationwide cross-sectional survey of palliative care-trained physicians in Taiwan assessed clinical practices an

Embolic stroke of undetermined source: A cardiovascular approach to diagnostic uncertainty.

Embolic stroke of undetermined source (ESUS) is a clinical event with an undelineated embolic etiology after the initial stroke workup. Once stroke specialists confirm ESUS, evaluation of rhythm disorders such as atrial fibrillation, specific cardiac pathologies (cardiomyopathy, left atrial cardiopathy, left-atrial septal pouch, aortic or mitral valve disease, cardiac neoplasm, right-to-left shunts, or device-related thrombus), and acquired hypercoagulable states, which may occasionally be related to malignancy, should be performed. While most ESUS patients require appropriate antiplatelet the

Assessing Decision Fatigue in General Practitioners' Prescribing Decisions Using the Australian BEACH Data Set.

BACKGROUND: General practitioners (GPs) make numerous care decisions throughout their workdays. Extended periods of decision making can result in decision fatigue, a gradual shift toward decisions that are less cognitively effortful. This study examines whether observed patterns in GPs' prescribing decisions are consistent with the decision fatigue phenomenon. We hypothesized that the likelihood of prescribing frequently overprescribed medications (antibiotics, benzodiazepines, opioids; less effortful to prescribe) will increase and the likelihood of prescribing frequently underprescribed medi

Digital planning and decision making for severe congenital oligodontia with Angle class-III malocclusion.

As maxillofacial surgery becomes increasingly digitized and the transformative impact of pre-surgical scanning and computer simulation is recognized, this clinical paper presents an algorithm for the selection of interventions in severe congenital oligodontia with Angle class-III malocclusion (OCIII) utilizing such technologies. A complex, multifactorial condition with varying degrees of craniofacial involvement, OCIII is associated with edentulous facial appearance, mandibular prognathism and deep underbite, as well as malocclusion. Our methodology involves the integration of CBCT imaging, in

Development of a Goal Elicitation Measure to Support Choice about Urinary Diversion by Patients with Bladder Cancer.

PURPOSE: Patient centered care aims to align treatment with patient goals, especially when treatment options have equivalent clinical outcomes. For surgeries with lasting impacts that alignment is critical. To our knowledge no psychometrically tested preference elicitation measures exist to support patients with bladder cancer treated with cystectomy, who can often choose between ileal conduit and neobladder diversions. In this study we created a scale to measure how patient goals align with each type of urinary diversion and the associated surgical outcomes. MATERIALS AND METHODS: We perform

Infection point of care tests bridge the experience gap of antibiotic STOP decisions: clinicians versus students.

BACKGROUND: Point of care tests (POCTs) offer accurate rapid diagnostics for infection, but not reduced antibiotic overuse in antibiotic stewardship (ABS) studies. Prescribing behaviour shaped by clinical experience may influence antibiotic decisions more than test performance. Understanding prescribing behavioural differences may inform ABS education. OBJECTIVES: To find out whether antibiotic decision making differ among medical students and intensive care clinicians when offered POCT use. METHODS: An observational study depicted four simulated clinical vignettes of hospital acquired pneum

Evaluating accountability, transparency, and bias in AI-assisted healthcare decision- making: a qualitative study of healthcare professionals' perspectives in the UK.

BACKGROUND: While artificial intelligence (AI) has emerged as a powerful tool for enhancing diagnostic accuracy and streamlining workflows, key ethical questions remain insufficiently explored—particularly around accountability, transparency, and bias. These challenges become especially critical in domains such as pathology and blood sciences, where opaque AI algorithms and non-representative datasets can impact clinical outcomes. The present work focuses on a single NHS context and does not claim broader generalization. METHODS: We conducted a local qualitative study across multiple healthcar

Lifetime Management for Aortic Stenosis: Strategy and Decision-Making in the Current Era.

Aortic stenosis, the most common valvular disease in the Western world, has traditionally been treated with surgical aortic valve replacement (SAVR) but is increasingly treated by transcatheter aortic valve replacement (TAVR). Whereas patients older than 65 years are preferably treated with bioprosthetic tissue valves, there is considerable uncertainty in the choice between TAVR and SAVR. We present various considerations for optimizing the lifelong management of patients receiving bioprosthetic valves (SAVR or TAVR). To maximize life expectancy and to minimize cumulative lifetime risk, we sug
