A practical and sensitive method for detection and quantification of soluble complement-fixing immune complexes in sera of patients with various disease states has been developed. The assay is based on inhibition of complement-dependent sheep red cell hemolysis mediated by polyclonal IgM rheumatoid factor. Aggregated human IgG was used as an in vitro model of C-fixing human immune complexes and was quantified by its ability to inhibit hemolysis of sensitized sheep red cells by isolated IgM rheumatoid factor. Hemolytic anemia occurred in a 23-year-old woman with spontaneous cytomegalovirus mononucleosis who had not received a blood transfusion. The patient's serum gave positive reactions in Coombs' antiglobulin test with human cord erythrocytes but did not react with adult "O" cells. Complement-fixation antibody titers against cytomegalovirus and levels of activity against cord erythrocytes increased as the anemia worsened and remained at high levels when recovery from the anemia occurred.

In 1988-1993, a total of 3,732 sera of blood donors from two districts (Strakonice and Nový Jicń) of the Czech Republic were tested for antibodies to Coxiella burnetii. Using the complement-fixation test at titers of 8 (10) and higher, the antibodies were detected in 41 (1.1%). No statistically significant difference in seroprevalence between the Strakonice and Nový Jicín districts was found (chi 2 = 2.536, P = 0.112, d.f. = 1). A 13 years old girl presented an acute episode of fever, productive cough, purulent rhinorrhea and bilateral pulmonary crepitant rales. 36 hours later a maculopapular eruption appeared on the face, extended to all body and became bullous. A case of Legionnaires' disease associated with Q fever is presented; this is probably the first reported case of concomitant infections with Legionella pneumophila and Coxiella burnetii. The pulmonary features of the illness were not typical of either disease. The difficulties of making a diagnosis in such cases are discussed.

Legionnaires' disease (LD) was first identified as an epidemic pneumonia occurring in the USA in 1976. Sporadic cases have since been identified in many countries. This report describes the first 2 cases in South Africa of LD with pneumonia in patients in whom the diagnosis was established by the finding in early acute and follow-up convalescent sera of a greater than fourfold rise in titre with the immunofluorescent antibody test. The microbiological investigations are discussed in some detail and the clinical, radiological and laboratory features of LD are briefly reviewed.

A rare case of hydatid cyst located in the musculature of the neck without any other detectable site is reported. With the cycle of the taenia echinococcus as reminder several pathophysiological hypotheses which might explain the inefficacy of the hepatic and pulmonary filter are discussed. A concise revue of the recent literature concerning diagnosis and treatment of the disease is given.

Investigation for genital chlamydial infections was carried out on 160 patients of fertile age attending a gynaecological outpatient clinic. Thirteen (9%) of the 144 patients with valid specimens were found to be Chlamydia-positive. Seven (54%) of them had antichlamydial antibodies by the complement-fixation test, and 11 (85%) antichlamydial antibodies by a single-antigen immunofluorescence test compared with 21 (16%) and 70 (53%), respectively, of the 131 Chlamydia-negative patients. A patient with culturally proved chronic pulmonary histoplasmosis was treated with amphotericin B. Complement fixation titers decreased but sputum cultures remained positive for Histoplasma capsulatum 5 months after original therapy. Lobectomy and a subsequent course of amphotericin B were necessary. Exceeding the minimal inhibitory concentrations of amphotericin B did not eradicate the organism and declining complement fixation titers failed to have prognostic significance.

From the aspect of laboratory Toxoplasmosis examination, a series of clinically and parasitologically confirmed cases of congenital Toxoplasmosis (CT) were selected. Serological findings (CFT, DT, IgM-IFAT) in infants and mothers were critically examined. The IgM titers in CT as a rule lay between 1:16 and 1:64, the highest recorded being 1:256. Investigations concerning the co-antibodies formed in the course of human leptospirosis and the extractive antigens of leptospiras were reviewed.
