The Cognitive Architecture of Obsessional Doubt: Inferential Confusion and Feared Selves in OCD-Relevant Intrusions.

Although previous research has examined the individual roles of unwanted intrusive thoughts (UITs), inferential confusion and feared selves in obsessive-compulsive disorder (OCD), no study to date has evaluated their combined effects on OCD severity. This is particularly relevant given that the inference-based approach (IBA) posits a close link between inferential confusion and feared selves in the context of obsessional doubt. The present study with a cross-sectional design investigated the role of inferential confusion and feared selves in mediating the relationship between OCD-relevant UITs

Narrative identity characteristics and personality pathology: An exploration of associations from a dimensional and categorical perspective in a clinical sample of youth.

Narrative identity, as an integral element of personality, has gained increased attention for understanding personality pathology. In this study, associations between narrative identity characteristics (i.e., event valence, theme, contextual coherence, thematic coherence, self-event connection valence, agency, and communion) and personality pathology were examined. Personality pathology was conceptualized as (a) levels of personality (dys)functioning and maladaptive personality traits, (b) six trait facet profiles, and (c) categorical DSM-5 (fifth edition of the Diagnostic Statistical Manual o

Integrating Videoconferencing Into Treatment for Obsessive-Compulsive Disorder: Practical Strategies With Case Examples.

Obsessive-compulsive disorder (OCD), a disabling psychiatric illness, creates substantial societal burden. Evidence-based treatments, including psychopharmacology and exposure with response/ritual prevention (EX/RP), are often inaccessible. Digital health technologies, including videoconferencing, may increase access, but the best way to integrate them with current treatments remains unclear. This column describes the experiences of faculty at the Center for OCD and Related Disorders with videoconferencing-assisted treatment. Through a case series, the authors describe five ways to incorporate

Decision-making impairment in schizophrenia: relationships with positive symptomatology.

Abnormal decision-making (DM) performance has been reported in several neurobehavioral disorders such as schizophrenia, addiction, and obsessive compulsive disorders. The exploration of DM correlates in terms of symptom formation may add more knowledge about the meanings of DM performance in schizophrenia. We examined the Iowa Gambling Task (IGT) and its relationship with clinical symptoms, evaluated by Positive and Negative Symptom Scale (PANSS), in 40 schizophrenic patients and 20 controls. Schizophrenic patients did worse on IGT performance with a significant difference between the two grou

Dopamine agonists and therapy compliance.

Dopamine replacement therapy in Parkinson's disease improves motor symptoms, however some patients suffer from motor and behavioural disturbances attributable to taking doses of medication well beyond the dose required to treat their motor disabilities. Dopamine dysregulation syndrome (DDS) can be regarded as a pattern of compulsive medication use, leading to disabling motor and behavioural features. DDS, probably attributed to pulsatile overstimulation of the limbic dopaminergic system, is characterised by severe dopamine addiction and behavioural disorders such as pathological gambling (PG),

Family Genetic-Risk Profiles Associated With Divorce.

We used Swedish national register data (N = 2,828,777) to examine divorce and its associated patterns of family genetic risk scores (FGRS; personalized measures of genetic risk inferred from diagnoses in relatives) across ten psychiatric disorders: major depression, anxiety disorders, obsessive-compulsive disorder, bipolar disorder, schizophrenia, anorexia nervosa, alcohol use disorder, drug use disorder, ADHD, and autism-spectrum disorder. Individuals who divorced had elevated FGRS across all disorders compared to those who were stably married or never married. FGRS for all disorders were hig

[Repetitive impulse-associated behavioral disorders in Parkinson's disease].

Parkinson's disease (PD) is associated with a number of behavioral disorders which may cause considerable social, professional or financial problems. Impulse control disorders (ICDs), such as pathological gambling, binge eating, compulsive shopping and hypersexuality occur in approximately 13-14% of PD patients. Further behavioral disorders are the dopamine dysregulation syndrome (DDS), a substance dependence characterized by craving for dopaminergic substances and punding (prolonged repetitive activities which are not goal-oriented).Treatment-related risk factors are dopamine agonists for ICD

Negotiating Stigma: Comparing the Experiences of Self-Identified and Clinically Diagnosed Men with OCD in Russia.

This cross-sectional qualitative study investigates how Russian men with obsessive-compulsive disorder (OCD) experience and manage mental health stigma, focusing on differences between those with a clinical diagnosis and those who self-identify with the disorder. Through in-depth interviews with eleven participants, the research explores meaning-making, disclosure practices, coping strategies, and the impact of masculine norms on help-seeking. Findings reveal that self-identified men often leverage the OCD label for personal understanding and peer support, while clinically diagnosed men gain v

Early life experiences and adult attachment in obsessive-compulsive disorder. Part 2: Therapeutic effectiveness of combined cognitive behavioural therapy and pharmacotherapy in treatment-resistant inpatients.

OBJECTIVES: Obsessive-compulsive disorder (OCD) is a chronic mental disorder that is often hard to treat with current treatment options. Therapeutic outcomes are predicted by many factors, ranging from biological to psychosocial. Early life experiences and adult attachment influence the effectiveness of the treatment. This study explores their predictive abilities in the combined treatment of adult inpatients with OCD. METHODS: Seventy-seven patients with OCD, diagnosed according to the ICD-10 criteria, were included in the study, out of which 66 patients completed the treatment. All patients

Examining the Hierarchical Influences of the Big-Five Dimensions and Anxiety Sensitivity on Anxiety Symptoms in Children.

Anxiety sensitivity (AS), namely the fear of anxiety symptoms, has been described as a precursor of sub-threshold anxiety levels. Sexton et al. (2003) posited that increased AS would arise from an elevated neuroticism and that both would act as vulnerability factors for panic disorder (PD), obsessive-compulsive disorder (OCD), and generalized anxiety disorder (GAD) symptoms. Accordingly, this study aimed to (1) evaluate the applicability of this model to a pediatric population and (2) examine the influences of the other Big-Five personality dimensions on the four lower-order dimensions of AS (

Community Structure of a Mental Health Internet Support Group: Modularity in User Thread Participation.

BACKGROUND: Little is known about the community structure of mental health Internet support groups, quantitatively. A greater understanding of the factors, which lead to user interaction, is needed to explain the design information of these services and future research concerning their utility. OBJECTIVE: A study was conducted to determine the characteristics of users associated with the subgroup community structure of an Internet support group for mental health issues. METHODS: A social network analysis of the Internet support group BlueBoard (blueboard.anu.edu.au) was performed to determin

Family History of Mental and Neurological Disorders and Risk of Autism.

IMPORTANCE: Familial aggregation of mental and neurological disorders is often observed in autism spectrum disorders (ASD), but reports have generally focused on single disorders and are limited to first-degree relatives. OBJECTIVES: To examine family history of mental and neurological disorders among first- to fourth-degree relatives and risk of ASD with and without intellectual disability (ID) in index persons. DESIGN, SETTING, AND PARTICIPANTS: In this population-based cohort study, 567 436 index persons were identified from the Stockholm Youth Cohort, an ongoing longitudinal register-lin

Obsessive-compulsive disorder symptoms before and after recovery from bulimia nervosa.

OBJECTIVE: Obsessive-compulsive disorder (OCD) symptoms are common in people who are ill with bulimia nervosa. However, little is known about whether OCD symptoms persist after long-term recovery from bulimia. METHOD: Thirty-one female patients with bulimia nervosa, 29 women who had been recovered from bulimia for more than 1 year, and 19 healthy female comparison subjects completed the Yale-Brown Obsessive Compulsive Scale, which measures OCD-like symptoms. Items related to symptoms of core eating disorders were omitted from the Yale-Brown scale. RESULTS: The Yale-Brown scale scores of the

Low intensity technology-delivered cognitive behavioral therapy for obsessive-compulsive disorder: a meta-analysis.

BACKGROUND: Cognitive behavioral therapy (CBT) is a well-established treatment for people suffering from obsessive-compulsive disorder (OCD) and technology-based CBT applications are an emerging treatment option for people with OCD. These applications involve treatment protocols with automated content delivery and relatively low clinical contact. Whilst such CBT applications are promising, however, further investigation is needed to establish the efficacy of this treatment approach for individuals with OCD. The aim of the present study was to review the efficacy of technology-delivered CBT wit

Psychiatric Comorbidities Associated with Food Addiction in Post-Bariatric Patients: Toward Personalized Mental Health Screening and Postoperative Care.

Background: Food addiction (FA) is an emerging construct that mirrors the behavioral and neurobiological characteristics of substance use disorders. Despite growing interest, its association with specific psychiatric disorders among bariatric patients remains understudied. Objective: Our aim was to examine the prevalence and strength of associations between FA and seven major psychiatric disorders in individuals who underwent bariatric surgery. Methods: In a sample of 100 post-bariatric patients referred for psychiatric evaluation, FA was assessed using the modified Yale Food Addiction Scale 2
