Pancreatic cystic lesions (PCLs) represent a growing clinical challenge due to their diverse biological behaviors and the substantial overlap in imaging features between benign, premalignant, and malignant entities. Traditional diagnostic approaches relying on cross-sectional imaging or isolated morphologic criteria frequently fail to achieve adequate risk discrimination. Advances in endoscopic ultrasound (EUS) now permit detailed morphologic assessment complemented by cyst-fluid biochemical markers, proteomic signatures, and comprehensive genomic profiling using next-generation sequencing. Here, we report an exceptionally rare event of malignant transformation of an intracranial epidermoid cyst into squamous cell carcinoma (SCC), diagnosed through cerebrospinal fluid cytology. A 61-year-old male with a prior history of left retrosigmoid craniotomy and partial resection of a large cerebellopontine angle epidermoid tumor 13 years ago presented with acute mental status changes and weakness. Imaging revealed recurrent epidermoid cyst elements with associated obstructive hydrocephalus. The patient underwent ventriculoperitoneal shunt placement, and cerebrospinal fluid cytology reveal

CASE SUMMARY: An 11-year-old female spayed Maine Coon cat was referred for assessment of hyporexia, weight loss, vomiting and diarrhoea. An abdominal ultrasound revealed an enlarged and hypoechoic pancreas containing two large fluid-filled structures. Fine-needle aspiration of the cyst-like structures was performed, and cytology revealed moderate-to-marked predominantly suppurative inflammation with yeast cells. Candida glabrata was cultured from the fluid. The patient was treated with oral itraconazole and the clinical signs resolved, although repeat abdominal ultrasound and cytology revealed

Cysticerci of Taenia solium cause cysticercosis, with neurocysticercosis (NCC) as the major pathology. Sensible and specific recombinant antigens would be an source of antigen for immunodiagnosis. The objective of this work was the molecular characterization and evaluation, of three news recombinant antigens (TsF78, TsP43 and TsC28), obtained by screening of a Taenia solium cDNA library. The three cDNA were analysed by bioinformatic programs, subcloned and expresed. The purified proteins were evaluated in ELISA using cyst fluid as control. TsF78 is filamina, TsP43 a peroxidase and TsC28 collag…

We present the rare case of a ruptured choledochal cyst (CC) in a young woman presenting with a two-day history of worsening upper abdominal pain. Imaging revealed a contracted gallbladder, dilated common bile duct (CBD), and a large amount of peritoneal fluid. Percutaneous paracentesis was performed, obtaining bilious fluid. Further imaging revealed cystic dilatation of the CBD and the diagnosis of rupture CC type I was made. The patient was initially managed conservatively with percutaneous drains, IV antibiotic therapy, and sphincterotomy through an ERCP. Elective cyst resection and Roux-en

Hip synovial cysts are rare. However, in patients with Rheumatoid Arthritis (RA) they present in higher frequency than in general population. Herein, we present an unusual case of a 67-year-old man with RA that presented with unilateral leg oedema and Deep Vein Thrombosis (DVT). Computed tomography (CT) scan revealed a giant cystic lesion adjacent to the right hip joint with longitudinal diameter of 14 cm. Magnetic Resonance Imaging (MRI) confirmed the characteristics of the cyst. Interestingly enough, there was evidence of osteonecrosis of the femoral head. CT guided Fine Needle aspiration (F

Fluid-fluid levels on CT and MR imaging are a typical feature of aneurysmal bone cysts but have also been reported in many other osseous lesions containing haemorrhage. We report fluid-fluid levels in four brown tumour in three patients with primary hyperparathyroidism in which the initial radiological diagnosis was thought to be a bone tumour. To the authors' knowledge, this is the first time that association has been reported in the international literature.

The pericardial cyst is a benign, uncommon congenital cystic lesion of pericardial origin located in the anterior and middle mediastinum. Most commonly, pericardial cysts are located in the right anterior cardiophrenic angle. Computed tomography (CT) and magnetic resonance imaging (MRI) of the chest are the non-invasive imaging modalities for the diagnosis of the pericardial cyst. Here we present a case of a 55-year-old male who presented with coughing and chest pain for two weeks. A chest X-ray revealed a soft-tissue opacity mass in the left lower zone. A CT of the chest showed a fluid-densit

A rare case of primary hyperparathyroidism with a functioning parathyroid cyst in whom Tc-99m MIBI scintigraphy failed to detect a parathyroid tumor is presented. A 62-year-old woman with primary hyperparathyroidism was referred for Tc-99m MIBI imaging to investigate a parathyroid adenoma. Plasma levels of intact parathyroid hormone were elevated to 2250 pg/mL. Neck ultrasonography revealed a cystic lesion measured 30 x 42 x 35 mm on the right inferior side of the thyroid gland. The cystic lesion was successfully removed at surgery. Pathologic diagnosis revealed a benign parathyroid cyst. The

The cytologic findings of a ciliated foregut cyst of the pancreas diagnosed by endoscopic ultrasound-guided fine-needle aspiration (FNA) are described. Cytologic features of ciliated foregut cysts include the presence of ciliated columnar cells and detached ciliary tufts in a cystic fluid background with amorphous debris and rare macrophages. These cytologic findings are clearly distinct from those of cystic mucinous neoplasms and other pancreatic cysts with which the ciliated foregut cyst may be confused. To the best of our knowledge, this is the first case reporting the cytologic findings of

Elbow synovial cysts are rare and can mimic more frequently encountered disorders such as lateral epicondylitis, presenting diagnostic challenges. This report describes a woman in her mid-40s with persistent pain and weakness in her right forearm due to a synovial cyst compressing the radial nerve at the Arcade of Frohse. Despite initial suspicions of lateral epicondylitis, deeper investigation using sonography confirmed the presence of a compressive synovial cyst. Ultrasound-guided aspiration of the cyst was performed, yielding clear synovial fluid and providing symptomatic relief. Post-proce

Popliteal cysts are a collection of synovial fluid found in the popliteal fossa that typically form in adults in association with traumatic injuries, degenerative conditions, or inflammatory arthritis of the knee. While often asymptomatic, popliteal cysts may become problematic as enlarging and ruptured cysts may compress surrounding neurovascular structures, resulting in lower extremity edema or peripheral neuropathy. We report a unique case of a symptomatic popliteal cyst in a patient with both compressive neuropathy and venous congestion in the setting of a non-ruptured popliteal cyst after

Mature cystic teratomas (MCT) of the ovary or dermoid cysts are commonly encountered benign ovarian lesions accounting for approximately 70% of benign tumors and originating from germ cells. A rare case of peritonitis in a 17-year-old female patient caused by traumatic rupture of an MCT of the right ovary is herein presented. A meticulous search of the literature has also been performed. Due to deterioration of patient's clinical condition, she was urgently led to surgery and intraoperative investigation of the peritoneal cavity revealed an inflamed ruptured ovarian cyst along with extensive

Pancreatic cysts include a variety of benign, premalignant, and malignant lesions. Endometrial cysts in the pancreas are exceedingly rare lesions that are difficult to diagnose pre-operatively. This report describes the findings in a 43-year-old patient with a recent episode of acute pancreatitis who presented with a large cyst in the tail of the pancreas. Imaging demonstrated a loculated pancreatic cyst, and cyst fluid aspiration revealed an elevated amylase and carcinoembryonic antigen. The patient experienced an interval worsening of abdominal pain, fatigue, diarrhea, and a 15-pound weight …

Osteochondroma is a benign bone tumor composed of a bony protrusion with an overlying cartilage cap. Osteochondromas arise in the scapula in 3% to 5% of cases, making them the most common benign bone tumor in this location. Scapular osteochondromas cause various pathologies of the shoulder girdle, including snapping scapula syndrome, chest wall deformity, shoulder impingement, and bursa formation. Bronchogenic cyst is an exceedingly rare finding in the periscapular area. It is a congenital cystic mass lined by tracheobronchial structures and respiratory epithelium. To our knowledge, there are

Cyst-like cavities in uveal melanoma occur rarely and can simulate a benign intraocular cystic lesion resulting in delayed diagnosis and inappropriate management. Herein, we describe a 66-year-old Caucasian female who presented with a "cystic" ciliary body mass in the right eye oculus dexter (OD). Slit lamp examination OD showed anterior bulging of the iris temporally from an underlying pigmented ciliary body mass and transillumination disclosed slight shadow from the tumor. Ultrasound biomicroscopy (UBM) revealed multiple cyst-like cavities within a tumor, lined by "thick walls" of at least 2
