We herein report a case of rupture of the urinary bladder due to emphysematous cystitis. The patient was an 77-year-old female who was hospitalized for treatment of a cerebral infarction and diabetes mellitus. Emphysematous cystitis was observed during examinations for fever. Despite urinary drainage and antibiotic therapy, she ran a fever for 3 weeks and had decreased urine volume. Even though uroplakins (UPs) are believed to serve a strong protective barrier against toxic materials, cyclophosphamide (CP) causes extensive cystitis. We investigated the expression of UPs in the urothelium in CP induced mouse cystitis. A total of 27 ICR female mice received a single intraperitoneal injection of 200 mg CP/kg. We herewith report the first case of uncomplicated urinary tract infection due to Corynebacterium striatum in an ambulatory patient without any other predisponent risk factors. C. striatum is a ubiquitous saprophyte of human skin and mucous membranes, which has been occasionally associated with infection in patients hospitalized or immunocompromised patients with underlying diseases. We conclude that C. striatum should be considered an emerging pathogen in both immunocompetent and immunocompromised hosts.
