Maternal behavioral health symptom profiles in early family life: complexity and context.

Behavioral health problems affect at least 15% of mothers, but few studies have examined how different problems cluster together. Characteri … n co-occurring with substance dependence and more severe when co-occurring with anxiety. Postpartum depression, postpartum anxiety, and smoking during pregnancy were the most robust correlates of being symptomatic in year 3. Mothers in the "Severe depression and anxiety" group were more likely to be in that profile if they reported functional impairment and/or relations …

Postpartum fluoxetine increased maternal inflammatory signalling and decreased tryptophan metabolism: Clues for efficacy.

Perinatal depression (PND) affects approximately 15% of women, and de novo postpartum depression affects approximately 40% of PND cases. Selective serotonin reuptake inhibitors (SSRIs) are a common class of antidepressants prescribed to treat PND. However, the safety and efficacy of SSRIs have been questioned in both clinical and preclinical research. Here, using a preclinical rodent model of de novo postpartum depression, we aim to better understand neuroinflammatory cytokines and tryptophan mechanisms that may be related to SSRI efficacy. Rat dams were treated with high corticosterone (CORT;

Anhedonia in postpartum rats.

Postpartum depression (PPD) is a debilitating illness, yet little is known about its causes. The purpose of this study was to examine a major symptom of depression during the postpartum period, anhedonia, by comparing sucrose preference in female rats that had undergone actual pregnancy or hormone-simulated pregnancy (HSP) to their respective controls. Whereas HSP rats showed significantly less preference than vehicle control rats for 1% sucrose solution during the first three weeks of the "postpartum" period, previously pregnant females showed only slightly depressed sucrose preference for th

Primiparas' prenatal depressive symptoms, anxiety, and salivary oxytocin level predict early postnatal maternal-infant bonding: a Japanese longitudinal study.

PURPOSE: It was reported that maternal-infant bonding failure predicts abusive parenting. Maternal-infant bonding is important to prevent child abuse. This study aimed to investigate the association between prenatal depressive symptoms, anxiety, cortisol, and oxytocin levels, and postnatal maternal-infant bonding. METHODS: The participants completed a self-report prenatal questionnaire that included the Edinburgh Postnatal Depression Scale (EPDS) and State-Trait Anxiety Inventory (STAI) in the second trimester. Blood and saliva were collected in the second trimester. Cortisol levels were meas

Prenatal depression moderates the relationship between maternal trauma exposure and cortisol production and predicts breastfeeding behavior.

Trauma exposure is associated with many negative outcomes for women during the prenatal and postnatal periods, including increased antenatal depressive symptomatology and dysregulation of the body's stress responses. Trauma exposure and its consequences are also tied to women's ability to breastfeed, a crucial component in maternal and infant health. Cortisol is biologically relevant to the breastfeeding process, and is also associated with depressive symptoms, which may interfere with women's ability to successfully maintain breastfeeding. However, no known studies integrate prenatal cortisol

Advocating for Interventions When Depression Complicates Preeclampsia.

Preeclampsia is a condition of pregnancy that is associated with high morbidity and mortality rates. Individuals diagnosed with preeclampsia have an increased chance of developing depression during pregnancy and in the postpartum period. This ultimately increases the risk for negative physical and emotional outcomes. A review of the etiology, pathophysiology, symptomatology, and risk factors for preeclampsia and depression, as well as the impact of COVID-19, can improve outcomes by helping nurses provide evidence-based holistic care. This article focuses on providing enhanced knowledge to help

Physical intimate partner violence among women experiencing homelessness before and during pregnancy.

Intimate partner violence (IPV) and homelessness can have devastating health consequences for pregnant women. Using the Pregnancy Risk Assessment Monitoring System, we assessed differences in the association of physical IPV before and/or during pregnancy with adverse health outcomes between women experiencing homelessness (WEH) and domiciled women. Among 186 891 respondents, representing an estimated 11 489 161 women, 27.1% and 3.4% of WEH and domiciled women, respectively, reported experiencing physical IPV. Physical IPV was associated with higher odds of having postpartum depression and pret

Equity-focused barriers and facilitators to implementing a prenatal yoga intervention in a healthcare system: patient and provider perspectives.

BACKGROUND: Individuals with a history of depression are at increased risk for postpartum depression. Further, racial and ethnic minority groups have higher rates of postpartum depression and are less likely to initiate mental health treatment. Prenatal yoga presents a promising avenue for preventing postpartum depression among these high-risk groups. The purpose of this study was to gather feedback from pregnant patients and healthcare professionals to identify facilitators and barriers to the equitable implementation of an 8-week prenatal yoga intervention to prevent postpartum depression.

Improving perinatal mood and anxiety disorders through integrated infant mental health care in obstetrics: evidence from a program evaluation study.

BACKGROUND: Depression and anxiety in the perinatal period affect many women and have multiple negative impacts on the mother and baby. The Integrated Infant Mental Health approach embeds a Behavioral Health Consultant (IMH-BHC) who has specialized training in Infant Mental Health into OB/GYN clinics. This manuscript reports a quasi-experimental comparison of two groups of women through pregnancy and the first year postpartum, receiving integrated IMH care versus standard OB care. We hypothesized integrated IMH care patients would show less anxiety and depression across pregnancy and postpartu

Optimizing Non-Pharmacological Support for Perinatal Mental Health: A Bayesian Network Meta-Analysis of Intervention Effects by Pregnancy/Postpartum Stage.

PURPOSE OF REVIEW: Non-pharmacological interventions for perinatal mental health lack personalized guidance tailored to the distinct stages of pregnancy. To address this evidence gap, this study employed a network meta-analysis, aiming to identify the optimal non-pharmacological intervention strategies for the perinatal period. FINDINGS: The analysis included 153 RCTs. For prenatal patients, cognitive behavioral therapy (CBT) proved most effective for depression (SMD = -1.27, 95% CI = -1.98, -0.55), while mindfulness was most effective for anxiety (SMD = -1.44, 95% CI = -2.11, -0.77).
