Long-term results of an obesity program in an ethnically diverse pediatric population.

OBJECTIVE: To determine if beneficial effects of a weight-management program could be sustained for up to 24 months in a randomized trial in an ethnically diverse obese population. PATIENTS AND METHODS: There were 209 obese children (BMI > 95th percentile), ages 8 to 16 of mixed ethnic backgrounds randomly assigned to the intensive lifestyle intervention or clinic control group. The control group received counseling every 6 months, and the intervention group received a family-based program, which included exercise, nutrition, and behavior modification. Lifestyle intervention sessions occurred

[Treatment of obesity in pediatric patients in Germany: anthropometry, comorbidity and socioeconomic gradients based on the BZgA Observational Study].

BACKGROUND: In recent years, the prevalence of overweight and obesity in children and adolescents has markedly increased. Even though numerous treatment options are available for patients, these are heterogeneously structured and insufficiently evaluated. PATIENTS: In this initial report of specific therapy under routine care circumstances, data from 1916 patients aged 8-16.9 years were analyzed. Patients were characterized based on cluster of therapy, sociodemographic, psychosocial and medical features. Furthermore, the quality of care procedures with regard to the diagnosis of comorbidities

Predictors of weight loss in overweight veterans with knee osteoarthritis who participated in a clinical trial.

Individuals with osteoarthritis (OA) of the knee have a high prevalence of obesity. The objective of this study was to determine predictors of weight loss among participants with knee OA in a clinical trial. A secondary analysis of data from a randomized clinical trial was used. Multiple regression analysis was used to predict weight loss after identification of predictor variables from the data. Hypothesized predictors of weight loss included demographic, clinical, and behavioral characteristics. The only baseline variable that was significantly related to weight loss was the Center for Epide

Catatonia and psychosis associated with sibutramine: a case report and pathophysiologic correlation.

Sibutramine is a serotonin and norepinephrine reuptake intake inhibitor approved for the management of obesity. It has various CNS adverse effects and a few case reports of psychiatric manifestations. Here, we report a case of severe catatonic and psychotic symptoms associated with sibutramine overuse in the patient who had no prior psychiatric history. The symptoms subsequently resolved completely with cessation. Given the widespread use of sibutramine, this case would be of interest to practitioners of internal medicine and psychiatry. Moreover, as sibutramine may be used for weight manageme

An experimental test of the relationship between self-esteem and concern about body shape and weight in restrained eaters.

OBJECTIVE: Although self-esteem and overconcern with body shape and weight are considered to be closely connected in bulimia nervosa, little empirical research has been done to investigate the alleged link. METHOD: In this study, we examined experimentally whether overconcern with body shape and weight was connected with self-esteem in an analogue sample of high restrained eaters by means of a subliminal lexical decision task. RESULTS: It could indeed be demonstrated that low self-esteem and overconcern with body shape and weight are associated in high restrained eaters: after priming low se

Factors associated with eating disorder symptoms in a community sample of 6th and 7th grade girls.

Nine hundred thirty-nine 6th and 7th grade girls participated in the baseline phase of a prospective study designed to examine a set of potential risk factors for the development of eating disorders. Of the 939,839 girls (89%) completed the bulimia nervosa section of the Structured Clinical Interview for DSM-III-R disorders. One girl received the diagnosis of bulimia nervosa, another 35 were classified as a symptomatic group. Using analysis of covariance (ANCOVA), controlling for age and stage of sexual maturation, symptomatic and asymptomatic groups were compared on the following measures: Ea

An attempt to modify unhealthful eating attitudes and weight regulation practices of young adolescent girls.

This is the first long-term, controlled study evaluating the effectiveness of a prevention curriculum designed to modify the eating attitudes and unhealthful weight regulation practices of young adolescent girls. Nine hundred sixty-seven sixth and seventh-grade girls were randomized to experimental healthy weight regulation curriculum or no-treatment control classes. A prevention intervention was developed around three principal components: (1) Instruction on the harmful effects of unhealthful weight regulation; (2) promotion of healthful weight regulation through the practice of sound nutriti

Long-term weight control study. II (weeks 34 to 104). An open-label study of continuous fenfluramine plus phentermine versus targeted intermittent medication as adjuncts to behavior modification, caloric restriction, and exercise.

Between weeks 34 and 104, we explored different schema for administering fenfluramine plus phentermine in open-label fashion. At week 34, the original placebo group participants began taking fenfluramine plus phentermine (placebo-to-active group). Those receiving fenfluramine plus phentermine between weeks 6 and 34 either continued to receive medication or began targeted intermittent therapy. Participants who did not lose 10% of initial weight received an augmented dose (60 mg fenfluramine plus 30 mg phentermine. The placebo-to-active group lost an additional 9.1 +/- 0.8 kg (mean +/- SEM) in t

Eating behavior in eating disorders: response to preloads.

Eating behavior in eating-disordered subjects was investigated by recording food intake and subjective ratings following three preloads differing in calories, weight and connotation. Subjects were patients with a DSM-IIIR diagnosis of anorexia nervosa or bulimia nervosa and nonpatient volunteers (normal-weight or overweight dieters, and normal-weight nondieters). After all preloads, anorectics ate significantly less than all other subjects except normal-weight dieters, and anorectics rated hunger and desire to eat consistently lower and fullness greater than all other subjects. When analysis o

Acanthosis nigricans in obese women with hyperandrogenism. Characterization of an insulin-resistant state distinct from the type A and B syndromes.

Acanthosis nigricans and hyperandrogenism are commonly found in patients with extreme target cell resistance to insulin, as in the type A and B syndromes of insulin resistance. However, the significance of concurrent acanthosis nigricans and hyperandrogenism in other clinical settings is not clear. We observed acanthosis nigricans to be present in 5% (15 of 300) of patients being evaluated for hyperandrogenism, and carried out studies of insulin binding and action in a group (7) of these women. Although none were diabetic, all were insulin resistant as assessed by hyperinsulinemia when fasting

Unusual sudden death.

In contrast to usual sudden death seen in the course of coronary artery disease, individuals dying suddenly from other causes form a complex array of situations. In some the causes are readily identifiable. No simple pattern is available to identify the potential candidate, but on review of the many causes some moves by the physician may be helpful. For example, a more complete physical evaluation of young individuals participating in competitive athletics is in order. This is particularly true if the athlete reports an episode of unexplained syncope. This may well be the warning of a propensi

Medical therapy of obstructive sleep apnea.

Guidelines for the medical therapy of obstructive sleep apnea are difficult to define precisely. While some elegant investigations have been completed, most study populations have been small. Also, the long-term effects of most forms of therapy are not known. Some patients will respond to a given form of therapy or combination of therapies while others will not. In most instances the responders cannot be recognized prior to the institution of therapy and a cycle of trial and error ensues. One of the best nonsurgical approaches appears to be weight loss, albeit unsuccessful in most cases. Almos

Type IV hyperlipoproteinemia: effects of a caloric restricted type IV diet versus physical training plus isocaloric type IV diet.

Serum lipids, plasma insulin and glucagon, aerobic capacity, and body composition were examined in middle-aged men (X age = 44.2 years) with type IV hyperlipoproteinemia to determine the relative effectiveness of a caloric restricted type IV hyperlipoproteinemia diet (group A) versus physical training plus an isocaloric type IV diet (group B). After 9 weeks of the above interventions, reductions (P less than 0.01) in mean cholesterol levels from 213 to 186 (12% change) and from 205 to 185 mg/dl (9% change), and in triglyceride levels from 332 to 211 (29% change) and from 263 to 138 mg/dl (42%

Diet Quality and Physical Fighting Among Youth: A Cross-National Study.

The predictors of engaging in physical violence appear to be quite diverse, covering an array of social and psychological risk factors. The present study expands upon this literature by exploring the link between diet quality and physical fighting during early adolescence using cross-national data from the 2005/2006 Health Behaviors of School Children (HBSC) survey. The results reveal that the rate of physical fighting was 40% higher among youths with lower quality diets, whereas the odds of frequent physical fighting were 68% higher, relative to youths with better diets. Ancillary analyses re

Genetic factors may load the gun, but environmental factors pull the trigger: MedDiet and DII in rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease that causes progressive joint destruction. It affects ~1% of the world population and is more common in women aged 20-45 years than in men. RA causes involvement of small joints such as the joints of the hands and feet, pain, swelling, stiffness and loss of function in the joints. In recent years, Mediterranean diet and anti-inflammatory diet models have attracted attention in the medical nutrition therapy of patients with RA. These dietary patterns have been suggested to reduce disease risk and complications and improve di

Anti-obesity and anti-diabetic activities of leaves extract from Morus nigra L. in mice submitted to a hypercaloric diet.

Obesity is a major public health concern that is associated with endocrine and metabolic disorders. Morus nigra L. is traditionally used in folk medicine owing to its anti-obesity and anti-diabetic effects. The effect of the lyophilized aqueous extract of the leaves of M. nigra L. (MN) on body weight gain and adipose tissue deposition in mice fed a hypercaloric diet was analyzed. A total of 46 male mice were divided into four groups: standard diet (SD), standard diet and extract (SD+MN), hypercaloric diet (HFD), and hypercaloric diet and extract (HFD+MN) for 12 weeks. The macroscopic, physiolo
