[A case of brain abscess secondary to bisphosphonate-related osteonecrosis of the jaws in metastatic bone lesions from breast carcinoma].

Bisphosphonates have been used clinically as highly effective drugs in the treatment of hypercalcemia of malignancy, and bone metastasis of solid cancers. Despite these benefits, however, the emergence of bisphosphonate-related osteonecrosis of the jaws has become a growing and significant problem in a subset of patients receiving these drugs. We report a rare case of brain abscess secondary to BRONJ in metastatic bone lesions from breast carcinoma.

Does any therapy exist for vascular calcifications in uremia?

The pathogenesis of vascular calcifications in uremia is not completely understood, but is regarded as multifactorial, involving traditional and nontraditional risk factors. In particular, derangements in divalent ions are considered of outmost importance, but also the role of physiologic inhibitors of calcification is now claimed. The most powerful physiologic inhibitor of calcification is pyrophosphate, but its biochemical instability precludes its clinical use to date. The pharmacologic analogs of pyrophosphate, bisphosphonates, cannot be easily tested for this purpose in renal patients, gi

Femoral insufficiency fractures with bisphosphonate therapy. Cases study.

The aim of this paper is to analyze on clinical cases specific features of femoral insufficiency fractures during bisphosphonates therapy. We are presenting two insufficiency femoral fractures (2 patients) in patients receiving bisphosphonate therapy. These two cases represent new type of femoral fractures. These are low energy or spontaneous fractures of diaphysis and subtrochanteric region. Prodromal pain is often observed. On x-rays thick lateral cortex, its ellipsoid thickening and transverse or oblique fracture line is observed. Many authors link such fractures to long-term bisphosphonate

[Calcium pros and cons: long term use of bisphosphonates. Bisphophonate should not be used for long term].

It is ten years since bisphophonates have been used for osteoporosis treatment in Japan, while the number of long term bisphosphonate treatment patients has been increased. Recently atypical femoral fractures or osteonecrosis of jaw have been reported. Although incidence of atypical fractures is very rare compared to the number of the patients under bisphosphonate treatment, these conditions are thought caused by severely suppressed bone turnover by the treatment. If the osteoporotic patients under long term bisphophonates treatment have not high fracture risk, drug holiday should be considere

[Biphosphonate-induced femoral stress fractures : A new problem and knowledge so far - case report].

During the last several years the treatment of osteoporosis with bisphosphonates has become accepted as a safe and effective procedure. However, recently there have been increasing numbers of reports of rare complications in the literature. Particularly the occurrence of atypical fractures of the femur has become a focus of interest but the problem is insufficiently known and only rarely addressed in the scientific discussion. The case illustrated here and a survey of the important facts in the recent literature highlight essential aspects of long-term bisphosphonate therapy.

[Clinical case of the month. Bilateral optic neuropathy secondary to biphosphonate therapy].

Ocular side effects associated with biphosphonates have been described. Conjunctivitis, uveitis and scleritis are the most common adverse events. We report a case of bilateral toxic optic neuropathy with optic dies edema occurring soon after a pamidronate intravenous infusion in a patient who previously presented both a conjunctivitis and an uveitis of moderate intensity associated with alendronate therapy.

[Paget's disease and hypercalcemia: coincidence or causal relationship?].

Paget's disease is a chronic disease in which osteoclast mediated bone resorption precedes imperfect osteoblast mediated bone repair. Symptoms include bone pain, pathological fractures, osteoarthritis and neurological symptoms. There is evidence that genetic and viral component are involved in the etiology. Hypercalcemia is rare and when it is diagnosed, primary hyperparathyroidism should be ruled out. The authors present a patient with Paget's disease and concomitant hypercalcemia. Evaluation for hypercalcemia revealed an adenoma of the parathyroid. However, despite the removal of the adenoma

Prevention of vascular calcification with bisphosphonates without affecting bone mineralization: a new challenge?

Arterial calcification has been found to coexist with bone loss. Bisphosphonates, used as standard therapy for osteoporosis, inhibit experimentally induced vascular calcification, offering perspectives for the treatment of vascular calcification in renal failure patients. However, Lomashvili et al. report that the doses of etidronate and pamidronate that are effective in attenuating aortic calcification also decrease bone formation and mineralization in uremic rats, limiting their therapeutic use as anticalcifying agents.

Bisphosphonate and nonbisphosphonate-associated osteonecrosis of the jaw: a review.

BACKGROUND: The term "osteonecrosis of the jaw" (ONJ) is used almost synonymously with the term "bisphosphonate-associated ONJ." However, necrosis of the jawbones leading to exposure of bone is associated with other factors. TYPES OF STUDIES REVIEWED: The authors conducted a literature search using PubMed to identify original research articles and case reports that described oral conditions and associated factors that result in sequestrum formation and bone exposure. RESULTS: After reviewing conditions that may lead to necrosis of the jawbones, the authors divided the causes of ONJ into the

Osteogenesis imperfecta: questions and answers.

PURPOSE OF REVIEW: Considerable attention has recently been focused on the pathogenesis, diagnosis and treatment of osteogenesis imperfecta. Two new genes have been defined in patients with recessive severe or lethal osteogenesis imperfecta types. Diagnostic concerns involve testing procedures, either skin biopsies or DNA analysis. Bisphosphonates have been accepted as 'standard of care' for children with osteogenesis imperfecta. However, questions remain as to the selection of patients for treatment, effectiveness in fracture prevention, which bisphosphonates should be used and the duration o

Marketing disease: is osteoporosis an example of 'disease mongering'?

Osteoporosis is often described as a disease, yet the symptoms are imperceptible and reliable diagnostic criteria have not been formulated. It is probably better described as a potential risk factor in patients with underlying illness. However, the marketing of osteoporosis has created the impression that all women (and many men) are at risk of suffering fragile bones, hip fracture and death. Large sums of money have been spent on raising awareness, diagnosing and treating osteoporosis, yet its link to hip fracture is not strong and the drug therapies used may be of little benefit or cause act

[Osteonecrosis of the mandible induced by intravenous bisphosphonates in oncological patients].

Bisphosphonates are synthetic analogs of inorganic pyrophosphate, which are not only potent inhibitors of osteoclast-mediated bone resorption, but also present an important inhibitory activity of angiogenesis. Bisphosphonates become, since 1996, the standard of care in the management of patients with osteoporoses as well as bone metabolism alterations associated with neoplasias. In 2003 the first case-reports of bisphosphonate induced osteonecrosis of the jaws were published. The authors report 2 new cases of patients with osteonecrosis induced by bisphosphonates. Case 1: Seventy one years-old

Benefit of a 17-year long-term bisphosphonate therapy in a patient with Gorham-Stout syndrome.

This case report of a 61-year-old woman suffering from Gorham-Stout syndrome shows osteolyses of the left pelvis, proximal femur and lumbar spine. The therapeutic regime has included two courses of percutaneous radiotherapy and also continuous application of bisphosphonates over 17 years. Despite this antiresorptive therapy, elevated urinary excretion of desoxypyridinoline has indicated the persistence of increased bone destruction. The radiological progression following bisphosphonate treatment was only moderate. However, physical disability is reduced, but without soaring handicaps suggestin

[Non-hormonal treatments of postmenopausal osteoporosis].

In the last decade, several medications have been registered and marketed in osteoporosis. They have demonstrated their antifracture efficacy. Subsequently, the clinical management of osteoporosis becomes more sophisticated and complex. Bisphosphonates (alendronate, risedronate, ibandronate, ibandronate and zoledronate) have demonstrated their efficacy on the axial and appendicular skeleton. On scientific grounds, it seems difficult to distinguish between them, in terms of efficacy and/or safety. A special interest should be focused on the optimisation of patients' adherence, which remains poo

[Osteoporosis, bisphosphonates and jaws osteochemonecrosis].

Bisphosphonates are widely used in the treatment of nonmalignant bone disease like osteoporosis, Paget's disease and malignant disease like malignant hypercalcemia, osteolytic metastasis from breast and prostate cancer and multiple myeloma. Jaws osteonecrosis is described since 2003 and is more often associated with the intraveinous use than oral use. Higher risk of osteonecrosis is noted after 3 years of osteoporosis treatment. The precipitating event is often a tooth extraction or other invasive procedure. There is no effective treatment for this pathology. Careful oral examination is necess

[Bisphosphonate-associated osteonecrosis of the jaws. 3. Two case reports].

The cases presented show that bisphosphonate-induced osteonecrosis of the jaw is difficult to treat. Recently, a classification of bisphosphonate-induced osteonecrosis of the jaw based on clinical appearance was published. On the basis of this classification the seriousness of the osteonecrosis can be evaluated and a method of treatment determined. The common opinion is that treatment should be as conservative as possible. This means that treatment should have as its objective the prevention of the spread of the disease by means of antibiotics and disinfectant mouthwash. Sharp bony edges may b

Possible delayed onset of osteonecrosis of the jaw in association with zoledronic acid.

OBJECTIVE: To report a case of possible delayed-onset osteonecrosis of the jaw after treatment with zoledronic acid. CASE SUMMARY: A 53-year-old white man with no history of allergic drug reactions had been diagnosed as having bronchial epidermoid carcinoma. He received therapy with docetaxel and zoledronate. Because of metastatic progression of the disease, he started treatment with irinotecan and zoledronate. The patient received 18 monthly cycles of zoledronate. One year after the last cycle of bisphosphonate therapy, the patient had one tooth extracted. Three weeks later, he complained of
