INTRODUCTION: To prevent an acute surgery, classic indications for elective sigmoid resections concerning diverticulitis have usually been based on the number of recurrent episodes. Since 2005 these indications have been challenged, primarily because the majority of patients first present themselves with acute complications at their first episode.
METHODS: Between 1990 and 2000, a cohort analysis was conducted involving all patients admitted to the VU University Medical Center with the diagnosis of diverticulitis, with a follow-up until January 2009. To identify those patients who might benef

Potentially life threatening diseases can mimic a groin hernia. We present an unusual case of diverticulitis with perforation and a resulting abscess presenting as a strangulated inguinal hernia. The features demonstrated were not due to strangulation of the contents of the hernia but rather pus tracking into the hernia sac from the peritoneal cavity. The patient underwent sigmoid resection and drainage of retroperitoneal and pericolonic abscesses. Radiological and laboratory studies augment in reaching a diagnosis. The differential diagnosis of inguinal swellings is discussed.

The diagnostic procedure is determined by the severity of the diverticulitis. In complicated cases of diverticulitis, it is necessary to detect those patients with obstructive ileus, perforation, and peritonitis who require instant emergency surgery. In all other cases, diagnostic procedures serve as a tool to determine the best therapeutic options. The CT scan of the abdomen seems to have the highest reliability for determining therapy. Other investigations such as barium enema and coloscopy are not indicated in emergency cases or cannot exactly describe the stage of the diverticular disease.

BACKGROUND/AIM: The objective of this study was to estimate the incidence of the Meckel's diverticulum (MD) and to study its clinical profile and surgical outcome, as well as to check whether diverticulectomy is indicated for asymptomatic MD in adults.
MATERIALS AND METHODS: This is a prospective study of 1332 patients who were operated upon for acute abdomen during the period August 1999 to July 2009 in a single surgical unit. Preoperative abdominal ultrasonography and plain x-ray abdomen (erect) were done depending on the necessity. These patients were subjected to laparotomy/ appendicectom

The impact of specific interventions at resection with primary anastomosis (PRA) for perforated diverticulitis with peritonitis is controversial. The aim of this pooled analysis was to determine whether any specific interventions performed at resection with primary anastomosis in patients with perforated diverticulitis with peritonitis influenced the outcomes.

BACKGROUND: The traditional therapy for perforated sigmoid diverticulitis with peritonitis is emergency colectomy usually with colostomy. We report laparoscopic exploration with peritoneal lavage as an alternative in seven patients who required emergency surgery for diverticulitis.
METHODS: Six patients presented with diffuse peritonitis and one with a failure of percutaneous therapy. All patients were explored laparoscopically and the peritoneal cavity was lavaged with saline in addition to receiving intravenous antibiotics. Patient demographics, clinical response, length of stay, and compli

We report an unusual case of a male patient with a colovesical fistula secondary to diverticulitis presenting in the genitourinary medicine clinic as non-gonococcal urethritis.

BACKGROUND: Colovesical fistula is a rare complication of inflammatory and neoplasic pelvic diseases (0.2%).
AIM: To report a new case.
CASE: We report the case of a 74-year-old woman, with a charged previous medical history and had not had hysterectomy. She was admitted because of cystitis with haematuria associated to faecaluria and pneumaturia confirmed by an intravenous pyelogram. Water-soluble contrast medium show the sigmoidovesical fistula. A sigoid sticture is identified at colonoscopy and biopsy was realised. The patient had had sigmoidectomy with Hartmann's procedure. Histologic st

A 72-year-old man, presenting with hypernephroma and infraclinic adrenal adenoma, developed severe hypokalaemia in the course of treatment for colonic perforation. Amikacin-induced hyperkaliuric hypokalaemia was suspected, and confirmed by withdrawal of the drug as well as observation of an unnoticed similar episode in the medical record of the patient.

In patients with suspected acute diverticulitis, plain abdominal radiographs may demonstrate free gas in the abdomen. Ultrasound can indicate the nature of disease but is not sufficient to assess the extent of the inflammation. If perforation is suspected, a water-soluble contrast agent should be used to demonstrate diverticulitis and peridiverticular abscess. Computed tomography is ideally suited for the evaluation of the disease. it permits the radiologist to tell the clinician the exact extent of the extramucosal inflammation and the involvement of adjacent organs and structures.

The authors report on three patients who had a perforated sigmoid diverticulum after unrelated surgery. If the preceding operation is remote in time from the perforation, detection is relatively easy, but if the perforation occurs shortly after major abdominal surgery, its presentation may be totally masked by the postoperative state. Awareness of this possibility may help alert the clinician to the presence of perforation of a sigmoid diverticulum in unexplained postoperative collapse. Although discovery of these three cases suggests that diverticular perforation is not uncommon after surgery

Current leading managements for diverticular disease cannot prevent the recurrence of diverticulitis, bleeding and/or other complications. There is an immediate need for developing new minimal invasive therapeutic strategies to prevent and treat this disease. Through a biomechanical analysis of porcine colon with diverticular lesions, we proposed a novel adhesive patch concept aiming at mechanical reconstruction of the diseased colon wall. This study aims to evaluate the surgical feasibility (safety and efficacy) of pulmonary visceral pleura (PVP) patch therapy using a pig model of diverticulo

Among 86 adult patients who underwent surgery for intestinal perforation, excluding cases caused by cancer invasion or appendicitis, diverticulitis (33/86, 38%) was identified as the most common cause of perforation, followed by ischaemia (7/86, 8%) and segmental absence of intestinal musculature (SAIM) (5/86, 6%). In patients with SAIM-related perforation, the site of perforation was the sigmoid colon in four cases and the caecum in one. SAIM-related perforation occurred predominantly in older women and most frequently involved the sigmoid colon. The four patients with sigmoid colon perforati
