Safety and tolerability of agomelatine: focus on hepatotoxicity.

Hepatotoxicity related to antidepressive pharmacotherapy is a major safety concern, particularly considering that severe forms of hepatic failure with fatal outcome have been reported. Severe hepatotoxic adverse drug reactions were also reported for agomelatine (AGM), an antidepressive agent, which was approved for the treatment of major depressive disorder (MDD) in adults by the European Medicines Agency (EMA) in February 2009. Its general safety and tolerability profile appears to be favourable or similar in comparison to other antidepressants, particularly regarding metabolic aspects, sexua

Dementia in the oldest old: a multi-factorial and growing public health issue.

The population of oldest old, or people aged 85 and older, is growing rapidly. A better understanding of dementia in this population is thus of increasing national and global importance. In this review, we describe the major epidemiological studies, prevalence, clinical presentation, neuropathological and imaging features, risk factors, and treatment of dementia in the oldest old. Prevalence estimates for dementia among those aged 85+ ranges from 18 to 38%. The most common clinical syndromes are Alzheimer's dementia, vascular dementia, and mixed dementia from multiple etiologies. The rate of p

Current opinions on tendinopathy.

Tendinopathy is characterized by pain in the tendon and impaired performance sometimes associated with swelling of the tendon. Its diagnosis is usually clinical but ultrasonography and magnetic resonance imaging can refine the diagnosis. Tendinopathy is highly prevalent and is one of the most frequently self reported musculoskeletal diseases in physical workers and sports people. Nevertheless, it is very difficult to carry out general epidemiologic studies on tendinopathy because of the varying sports cultures and sports habits in different countries. The aetiology of tendinopathy seems to be

The impact of perinatal probiotic intervention on the development of overweight and obesity: follow-up study from birth to 10 years.

BACKGROUND: The achievements in combating the increasing trend of overweight and obesity have thus far been inadequate. The recently discovered instrumental role of the gut microbiota in host metabolism may offer a novel target in the prevention and management of obesity. OBJECTIVE: To evaluate the impact of perinatal probiotic intervention on childhood growth patterns and the development of overweight during a 10-year follow-up. PATIENTS AND METHODS: Altogether 159 women were randomized and double-blinded to receive probiotics (1 × 10(10) colony-forming units of Lactobacillus rhamnosus GG,

Detection of virulence factors in Pseudomonas aeruginosa strains isolated from contact lens-associated corneal ulcers.

PURPOSE: In microbial keratitis associated with contact lens use, Pseudomonas is the most common etiologic agent. The purpose of this study was to report on the microbiological findings of 8 P. aeruginosa strains isolated from contact lens-associated corneal ulcers. METHODS: Scrapings from contact lens-related corneal ulcers were inoculated for culture. Identification and antibiotic susceptibility testing were performed by using the Vitek system (bioMérieux). The Pseudomonas' ability to form biofilm; produce gelatinase, elastase, and alkaline protease; and adhere to and invade human corneal e

Development of real-time reverse transcriptase PCR assays to detect and serotype dengue viruses.

Serotyping dengue virus (DENV) from suspect human specimens is crucial for developing sound epidemiological control measurements early in the transmission season and for effective patient management. We modified DENV consensus D1 (mD1) and serotype-specific TS2 (mTS2) and redesigned serotype-specific TS1 (rTS1) and TS4 (rTS4) as described previously in the conventional capsid and premembrane gene (C-prM) protocol (R. S. Lanciotti, C. H. Calisher, D. J. Gubler, G.-J. Chang, A. V. Vorndam, J. Clin. Microbiol. 30:545-551, 1992). In addition, we designed two new sets of amplimers and probes, locat

Negative effects of chronic inflammatory periodontal disease on diabetes mellitus.

Periodontal disease is the result of a complex interplay of bacterial infection and host responses, and is often modified by various systemic diseases such as diabetes mellitus. Such diseases are capable of affecting the periodontium and/or the treatment of periodontal disease. However, recent research has changed our concept of how periodontal disease should be treated. Here we present several concerns directed towards the periodontal therapy of patients with diabetes mellitus based on our studies. When treating periodontitis patients who have diabetes mellitus it is important to consider the

Clinical and molecular spectrum of primary hyperoxaluria type 1 in Tunisia: pediatric presentation and minimum observed prevalence.

BACKGROUND: Primary hyperoxaluria type 1 (PH1) is a rare autosomal recessive disorder caused by pathogenic variants in the AGXT gene. In regions with a high prevalence of consanguinity, the burden of PH1 is expected to be increased; however, data on its epidemiology in Tunisia remain scarce. This study aimed to estimate the minimum observed clinical prevalence of PH1 in Tunisia, to assess the expected genetic burden based on published pathogenic allele frequency data, and to describe the clinical and molecular spectrum of the disease in this cohort. METHODS: We retrospectively analyzed geneti

Disproportionate Cardiovascular Risk in Women with Type 2 Diabetes: A Narrative Review of Diet, Metabolic Phenotypes, and Gene-Diet-Epigenetic Interactions Across the Life Course.

Background: Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality among individuals with type 2 diabetes mellitus (T2DM). Although women generally exhibit a more favorable cardiovascular risk profile than men in the general population, this protection is substantially reduced in the presence of diabetes, resulting in a disproportionately greater relative increase in CVD risk among women. Objective: This review aims to integrate the roles of metabolic phenotypes, dietary exposures, and genetic susceptibility in shaping cardiovascular risk in women with T2DM, with a f

Assessing rheumatoid arthritis prevalence from 1990 to 2040: viewing global inequities through the lens of sociodemographic development.

OBJECTIVES: Rheumatoid arthritis (RA) imposes a substantial global health burden, contributing significantly to chronic disability and escalating healthcare costs. Accurate, long-term prevalence forecasts are essential to inform strategic planning for rheumatology services and health resource allocation. METHODS: Hence, we developed and calibrated a sex-specific illness-death model (IDM) at global, regional, and national scales using GBD data (1990-2021). To uncover underlying epidemiological patterns, we augmented our projections with a frontier analysis that evaluates the efficiency of RA b

Clonal Hematopoiesis of Indeterminate Potential as an Emerging Interdisciplinary Risk Factor in Alzheimer's Disease: Current Evidence and Future Directions.

Clonal hematopoiesis of indeterminate potential (CHIP) is an age-related condition affecting over 10-20% of individuals older than 70 years, characterized by the expansion of hematopoietic stem cell clones carrying somatic mutations in leukemia-associated driver genes in the absence of overt hematologic disease. Initially recognized as a precursor to hematologic malignancies, CHIP has since been implicated in diverse non-malignant disorders, notably increasing the risk of cardiovascular events by 40%. Recent epidemiological and experimental evidence suggests a potential disease-modifying influ

Long-term night shift work and cardiometabolic health in healthcare workers: a review of mechanisms and precision intervention strategies.

Long-term night shift work has become one of the major risk factors for the cardiometabolic health of healthcare workers. Traditional perspectives have largely attributed this to sleep deprivation. However, recent studies have revealed that circadian misalignment itself constitutes an independent pathogenic factor. This article provides a systematic review of the epidemiological evidence, pathophysiological mechanisms, and precision intervention strategies in this field. Existing studies indicate a clear dose-response relationship between night shift exposure and cardiometabolic risk, with thi

Psychiatric Risk Implications From Behavioral and Neural Effects of Adolescent Exposure to Environmental Insecticides: A Systematic Review of Rodent Studies.

Adolescence is a sensitive neurodevelopmental period marked by remodeling of brain circuits that support cognitive development and emotion and behavior regulation. These maturation processes heighten psychiatric vulnerability to environmental exposures, including to toxicants such as insecticides. Epidemiological studies show widespread adolescent insecticide exposure and increasingly link this to psychiatric outcomes, but underlying neural mechanisms remain poorly understood. Preclinical studies can clarify these associations and identify insecticide-induced mechanisms that may disrupt neurod

Why internists should care about Helicobacter pylori: recapitulating gastric cancer prevention.

Infection with Helicobacter pylori (H. pylori) is a leading but also modifiable cause of gastric cancer. However, evidence indicates that the goal of a primary prevention of gastric cancer is still far from being achieved. Main obstacles derive from inadequate screening and management of H. pylori infection but also from other external risk factors, responsible for about a quarter of future cases of gastric cancer. The need for population-based H. pylori screen-and-treat programs remains not fully addressed, mainly due to concerns about cost, to underpowered screening efficacy of policies only

Association of Emotional Stress and Adaptive Behavior with Stroke Risk: A Mendelian Randomization Study.

Although emotional stress and adaptive behavior are known to influence cardiovascular health, direct evidence linking them to stroke remains limited. This study aims to clarify the associations between emotional stress, adaptive behaviors, and the risk of stroke. We conducted a two-sample Mendelian randomization analysis using data from the UK Biobank, the European Bioinformatics Institute, the Integrative Epidemiology Unit, and the FinnGen project. Indirect effects were estimated using the product of coefficients method. Low satisfaction with family relationships was associated with increased
