This study compared the efficacy and safety of Lispro insulin and regular insulin in the management of hyperglycemic emergencies (HE). Fifty patients who presented in HE to the Emergency unit of Obafemi Awolowo University Teaching Hospitals Complex, Ile Ife participated in the study. Complaints referable to the abdomen are common emergency department presentations. Many of these conditions prove to be benign and self-limited, whereas others are potentially catastrophic. Because serious and benign intra-abdominal conditions share many relatively nonspecific symptoms, it is often difficult to identify patients who have life-threatening problems early in the course of their disease. Apart from relieving the patient's symptoms, the emergency physician's primary role is to detect and stabilize life-threatening conditions in a rapid and cost-effective manner.

The management of the patients with acute abdomen syndrome requires an accurate and rapid diagnostic. Spiral CT is the investigation of choice in these cases because it is rapid, cost-effective and provides a global perspective of abdominal structures. Also, helical CT offers the possibility to obtain thinner contiguous images and to perform coronal or sagittal reconstruction, as well as the possibility to make the acquisitions during the different vascular phases.

A retrospective study of all records of patients operated on for biliary lithiasis during the years 1995, 1996 and 1997 was performed in a general hospital setting to compare the costs of management according to presentation. This study shows that acute and chronic cholecystitis have statistically significantly different costs. The reimbursement plans, base on diagnostic related groups do not take this into account. This system disadvantages those centers which preferentially treat acute pathologies.

All consultants and trainees in anaesthesia in a large teaching hospital were surveyed.
