Comparative efficacy of fascia iliaca compartment block using liposomal bupivacaine versus ropivacaine for preoperative analgesia in elderly patients with intertrochanteric femur fracture: a randomised controlled trial protocol.

INTRODUCTION: Intertrochanteric femur fractures are common in the elderly, with effective pain management being crucial. Fascia iliaca compartment block (FICB) is a well-established method for preoperative analgesia in these patients, but whether liposomal bupivacaine is more effective for this application remains uncertain. This trial aims to evaluate whether liposomal bupivacaine provides superior preoperative analgesia compared to ropivacaine in elderly patients with intertrochanteric femur fracture. METHODS: This randomised controlled trial will include 84 elderly patients with intertroch

Severe necrotizing soft tissue infections (SENSEI) study: Protocol for a multi-centered audit.

BACKGROUND: Severe soft tissue infections are surgical emergencies typically accompanied by sepsis. Early recognition and management optimizes patient outcomes. Single-centered studies on these infections are limited due to their rare incidence. MATERIALS & METHODS: We propose a protocol for the SENSEI (Severe Necrotizing Soft Tissue Infections) study, a multi-centered audit across the United Kingdom to assess current clinical practice for treating severe soft tissue infections in adult and paediatric populations. This will be compared with standards predetermined by the Surviving Sepsis Camp

Erector Spinae Plane Blocks for the Early Analgesia of Rib Fractures in Trauma (ESPEAR): protocol for a multicentre pilot randomised controlled trial with feasibility and embedded qualitative assessment.

INTRODUCTION: Patients with rib fractures commonly experience significant acute pain and are at risk of hypoxia, retained secretions, respiratory failure and death. Effective analgesia improves these outcomes. There is widespread variation in analgesic treatments given to patients including oral, intravenous and epidural routes of administration. Erector spinae plane (ESP) blockade, a novel regional analgesic technique, may be effective, but high-quality evidence is lacking. METHODS AND ANALYSIS: To determine if a definitive trial of ESP blockade in rib fractures is possible, we are conductin

How--and when--to obtain consent for do-not-resuscitate orders. Clinical guidelines and strategies for resolving conflicts.

Consider a do-not-resuscitate (DNR) order when a patient's presumed consent for cardiopulmonary resuscitation (CPR) is in question, the patient has an illness that is terminal or severe and irreversible, or he or she is permanently unconscious or likely to have cardiac or respiratory arrest. The patient with decisional capacity has the right to give or withhold consent for a DNR order. State law may limit a surrogate's authority to request that CPR be withheld. Remember, a DNR order does not restrict a patient's access to intensive care. Nurses, patient advocates, social workers, and clergy me

Development of a core outcome set for the evaluation of shared decision-making interventions in healthcare: a study protocol.

INTRODUCTION: Shared decision-making (SDM) is a process whereby patients are supported to reach decisions about their healthcare in collaboration with healthcare professionals. Despite decades of SDM research, the impact of using SDM interventions (eg, patient decision aids, decision coaching, question prompt lists, training and feedback or service changes) on clinical and health service outcomes remains uncertain. This is partly because synthesis of the existing literature is hindered by substantial heterogeneity in the evaluation of interventions to facilitate SDM. A core outcome set (COS) i

Study of topical anaesthetic for paediatric laser procedures (STAPLE): a protocol for a feasibility and pilot randomised controlled trial.

INTRODUCTION: Cutaneous vascular anomalies and scars can cause significant physical and psychosocial difficulties for children, but can be ameliorated with pulsed dye laser (PDL) and neodymium-doped yttrium aluminium garnet (Nd:YAG) laser treatment. Given that multiple rounds of treatment are often required, and that the procedures are painful, achieving adequate analgesia is imperative in this setting. Paediatric procedural pain management guidelines suggest that multimodal non-pharmacological and pharmacological analgesia should be used for such procedures; however, the place of topical anae

PREACT-digital: study protocol for a longitudinal, observational multicentre study on digital phenotypes of non-response to cognitive behavioural therapy for internalising disorders.

INTRODUCTION: Cognitive behavioural therapy (CBT) serves as a first-line treatment for internalising disorders (ID), encompassing depressive, anxiety or obsessive-compulsive disorders. Nonetheless, a substantial proportion of patients do not experience sufficient symptom relief. Recent advances in wearable technology and smartphone integration enable new, ecologically valid approaches to capture dynamic processes in real time. By combining ecological momentary assessment (EMA) with passive sensing of behavioural and physiological information, this project seeks to track daily fluctuations in s

Randomised feasibility trial of outpatient versus inpatient laparoscopic cholecystectomy for benign gallbladder disease (OUTchol): study protocol.

INTRODUCTION: Cholecystolithiasis is a common condition and most cases are asymptomatic. In symptomatic cases, gallstone disease results in hospitalisation and significant healthcare costs. Laparoscopic cholecystectomy is the standard treatment and is traditionally performed in an inpatient setting. Recent studies indicate that outpatient laparoscopic cholecystectomy may offer comparable safety and effectiveness yet the feasibility and cost-effectiveness are not conclusively defined. METHODS AND ANALYSIS: This randomised-controlled, open-label feasibility trial will be conducted at two surgic

Embryo selection through non-invasive preimplantation genetic testing with cell-free DNA in spent culture media: a protocol for a multicentre, double-blind, randomised controlled trial.

INTRODUCTION: Morphological evaluation is used to select embryos for in vitro fertilisation. However, it does not fully reflect the implantation potential. Preimplantation genetic testing for aneuploidies (PGT-A) can detect embryonic aneuploidy, but biopsy procedure is invasive. Currently, a non-invasive PGT (ni-PGT) approach using spent medium is being evaluated. However, the clinical benefit of ni-PGT has not been clearly demonstrated. A multicentre randomised trial is needed to verify whether ni-PGT can be an new effective tool for evaluating embryos. METHODS AND ANALYSIS: Overall, 1148 co

Platelet transmission electron microscopy for the assessment of poor biological response to antiplatelet agent: pilot descriptive and prospective study - ELECTROSTROKE.

INTRODUCTION: Ischaemic stroke is the leading cause of adult disability. Thus, a strategy based on an efficient antiplatelet therapy has been developed. The monitoring of antiplatelet therapy is now limited to high risk and poor prognosis patients. Indeed, the biological monitoring of the antiplatelet therapy with available platelet function assays do not provide a global integrative approach. Platelet transmission electron microscopy, recently validated for assessing distinct ultrastructural abnormalities is a reliable morphological platelet structural analysis tool which could be used to col
