Short-latency somatosensory evoked potentials (SSEPs) of the tibial nerves in spinal cord injuries.

Stimulations of the tibial nerves of 76 spinal cord injured patients revealed short-latency somatosensory evoked potentials (SSEPs) especially P37 recorded from the scalp. The SSEPs findings can be classified by degrees of impairment into 5 different types as follows: Type 1: Absence of P37. This was found in all patients with complete cord lesions as well as patients with loss of joint sense. Type 2: Decreased amplitude and prolonged latency. This was elicited in patients with moderate to severe central cord lesions. Type 3: Prolonged latency only. Type 4: Decreased amplitude only. Type 5: No…

Changes in somatosensory-evoked potentials in limb lengthening. An experimental study on rabbits' tibiae.

To assess the influence of the rate and amount of distraction on the electrophysiologic function of the peripheral nerve during limb lengthening, somatosensory-evoked potential (SSEP) studies were performed on the hindlimbs of 96 rabbits. In Group I, the tibiae were lengthened 0.35 mm per day; in Group II, 0.7 mm per day; in Group III, 1.05 mm per day; and in Group IV, 1.4 mm per day. The studies were done preoperatively and then postoperatively, until six weeks in Group I, five weeks in Group II, four weeks in Group III, and three weeks in Group IV. As lengthening proceeded, the P1 (the first

Intraoperative recordings of spinal somatosensory evoked potentials to tibial nerve and sural nerve stimulation.

Somatosensory evoked potentials (SSEPs) to stimulation of the tibial nerve at the knee (TN-K) and ankle (TN-A), and the sural nerve at the ankle (SN-A), were recorded from 3 or 4 spinal levels during surgery for scoliosis in 11 neurologically normal subjects. With stimulation of all 3 nerves, the propagation velocity along the spine was nonlinear: it was faster over cauda equina and midthoracic cord than over caudal spinal cord. Over the mid-thoracic cord, TN-K SSEP propagation was faster than that of TN-A and SN-A SSEPs, whereas over the caudal spinal cord these values were similar on stimula…

On the limitation of brainstem auditory evoked potentials for indication of susceptibility to noise.

The efficiency of Brainstem Auditory Evoked Potentials (BAEP) as an indicator of susceptibility to hazardous noise was investigated. In earlier studies, subjects were exposed to very intense occupational noise and temporary threshold shift (TTS) was produced by high intensity noise of 115 dB. Correlations between experimental TTS and the BAEP changes were investigated. BAEP indices, which significant correlations with the eventual hearing loss, were found. Similar criteria, which were found significant in the earlier work, were investigated in 94 industrial workers with normal hearing, without…

Influence of a twofold voluntary hyperventilation on visually evoked cortical potentials and human pupillogram.

We studied the direct and aftereffects of twofold hyperventilation (HV) on pattern reversing VEPs and pupillograms (PGs) of 19 healthy volunteers. The VEP-N80 and P100 latencies increased during HV. Both peak times were maintained for a longer period, up to 20 minutes after HV-2 ended. In addition, the PG-latency time during HV and the PG-construction time during and after HV were increased. The results indicated a temporary delay of neural afferent transmission in the visual system during and after HV. A similar delay of the nervous transmission appeared in the efferent part of the system reg

Median nerve somatosensory evoked potentials in patients with lacunar and other small subcortical strokes.

We recorded median nerve somatosensory evoked potentials (SSEPs) in 42 patients with small subcortical strokes associated with lacunar syndromes. Seventeen patients had abnormal SSEPs. The P22 component was most often affected; changes in N20 and N18 were less common. Four patients with pure motor strokes (PMS) had abnormal SSEPs. Abnormalities of P22 correlated best with motor findings; those of N20 with sensory signs and symptoms. Similar SSEP abnormalities were produced by lesions in various locations. P22 and N20 abnormalities occurred independently and together. We conclude: (1) abnormal …

Somatosensory evoked potentials in rat cerebral cortex before and after middle cerebral artery occlusion.

We recorded somatosensory evoked potentials in pentobarbital-anesthetized rats before and after middle cerebral artery occlusion. Trigeminal (vibrissae), median (forelimb), and sciatic (hind limb) nerve stimuli produced consistent, robust, and sharply localized responses in the trigeminal, forelimb, and hind limb regions of the somatosensory cortex of 18 rats. These regions are situated at sequentially greater distances from the center of infarcts produced by middle cerebral artery occlusion. In eight rats, occlusion 1-2 mm below the rhinal fissure abolished somatosensory evoked potentials in …

Lumbosacral evoked potentials and vesicourethral function in patients with chronic suprasacral spinal cord injury.

Persistent detrusor acontractility despite normal somatic reflex activity in some patients with high spinal cord injury is an enigma. Previous work has suggested disordered integration of afferent activity in sacral roots or the sacral spinal cord. Forty male patients with chronic stable suprasacral cord lesions were studied by filling and voiding videocystometrography, and recording lumbosacral evoked potentials from posterior tibial nerve stimulation. Only five of 15 patients with decreased detrusor contractility had abnormal lumbosacral evoked potentials. Similar abnormalities were found in…

Intraoperative cortical somatosensory evoked potentials for detection of sciatic neuropathy during total hip arthroplasty.

Approximately 3% of patients undergoing hip arthroplasty develop postoperative sciatic neuropathy. The factors associated with changes in somatosensory evoked potentials (SSEP) and sciatic neuropathy were examined in patients undergoing hip arthroplasty, to evaluate whether the use of intraoperative SSEP could help reduce the incidence of postoperative sciatic neuropathy. Eighty-eight patients were assigned to either monitored or unmonitored groups. SSEP were recorded following peroneal nerve stimulation, using contralateral stimulation to detect systemic influences on SSEP. Amplitude reductio…

[Changes in somatosensory evoked potentials in patients with vertebrogenic pain syndromes treated by electroacupuncture].

Application of electroacupuncture (EAP) of the segmental points to patients with vertebrogenic algesic syndromes decreased amplitude of N150 and P240 waves recorded from vertex to painful electrocutaneous stimulation in the region innervated by an affected root. As distinct from EAP of segmental point auricular EAP not only decreased the amplitude of late components of evoked potentials (EP), but also increased it, direction of EP changes depending on the character of sensitivity disorders. It is suggested that effect of EAP-evoked EP changes in patients with hypalgesia is determined by two op…

Somatosensory potentials evoked by magnetic stimulation of lumbar roots, cauda equina, and leg nerves.

Cortical somatosensory evoked potentials (SEPs) were studied by noninvasive magnetic stimulation at T-10, T-12, and L-5 vertebral levels and in mid-gluteus muscle and ankle in 27 normal subjects and 7 patients with neurological diseases. Cortical components P2 and N2 were recorded in all normal subjects. The mean peak latencies of P2 were 20.3 +/- 0.9 (standard deviation), 21.1 +/- 1.2, 23.5 +/- 1.4, 27.9 +/- 2.0, and 38.1 +/- 1.8 msec at the T-10, T-12, L-5, midgluteal and ankle sites of stimulation, respectively. No substantial difference in morphology of P2 and N2 was seen between magnetic …

Visual evoked potentials in Negro carriers of the gene for tyrosinase positive oculocutaneous albinism.

Visual evoked potential testing was performed on 15 Negro carriers of the gene for tyrosinase positive oculocutaneous albinism in order to detect whether they have the same visual pathway decussation anomalies as do homozygotes. No subject showed 01-02 asymmetry on monocular testing, indicating that decussation follows the normal pattern. It is concluded that visual evoked potential testing is probably not useful in the detection of Negroes heterozygous for the gene for tyrosinase positive oculocutaneous albinism.

[A case of alpha coma in acute brainstem dysfunction--consecutive electroencephalograms and evoked potentials].

A 31-year-old woman was admitted because of severe headache and dysarthria in December 1985. Neurological examination on admission revealed severe impairment of consciousness, anisocoria, absent light reflex on the right side, and evidence of left hemiparesis, but other brainstem reflexes were intact. A CT scan taken shortly after arrival demonstrated a large hematoma in the right temporal lobe and the right cerebellum. Breathing became irregular and intubation was needed. An emergency operation was performed. After the operation the patient remained comatose without any spontaneous respiratio

The use of somatosensory evoked potentials in the evaluation of the central nervous system.

SEPs may be recorded over the spine and scalp to stimulation of any accessible mixed or sensory nerve in the extremities. SEP abnormalities are useful in detecting lesions in central somatosensory pathways. They do not establish a specific diagnosis, but they may suggest or support a diagnosis made on clinical grounds. They have been used particularly to detect subclinical lesions in multiple sclerosis, but their role in following the course of this disorder is unclear. SEPs have been used as a prognostic guide in patients with hemispheric stroke and in patients who are comatose following head

Somatosensory evoked potentials following median and tibial nerve stimulation in patients with Friedreich's ataxia.

The changes in evoked potentials following median and tibial nerve stimulation in nine patients with clinically defined Friedreich's ataxia are reported and discussed. The response originating in the brachial plexus (Erb's point potential) was absent or reduced in amplitude with no prolongation of peak latency, and the response generated in the cauda equina (N18) was absent in all cases. Conduction time from the brachial plexus to cervical spine and medulla oblongata was normal, whereas the central conduction time (N13a/N20, N13b/N20) was delayed. There was moderate to marked attenuation of th…

Monitoring of sensory evoked potentials is highly reliable and helpful in the operating room.

Because short-latency evoked potentials are relatively resistant to anesthetic agents, they can be used to monitor neural pathways during surgical procedures. The use of median nerve somatosensory evoked potentials to localize the central sulcus is an established aid of indisputable value in neurosurgical procedures involving cortical incisions for resection of certain epileptic foci, vascular malformations, or neoplasms near the central area of the brain. Likewise, recording of intraoperative nerve action potentials is currently regarded as indispensable in management of the neuroma-incontinu…

Role of somatosensory evoked potentials in the diagnosis of peripheral nerve lesions: recent advances.

Short-latency somatosensory evoked potentials (SEPs) have become an important tool in the investigation of peripheral nerve lesions. Electrically evoked SEPs are most suitable because they provide results with small variations and are readily repeatable. Techniques for testing 10 different upper and lower limb nerves, dermatomes, cutaneous fibers of trigeminal nerve, and nerves supplying urogenital areas are now available. The established, principal areas of application are in the investigation of brachial plexus lesions, proximal injuries of individual nerves in upper and lower limbs, and pai…

Isolated brain-stem death. Case report with demonstration of preserved visual evoked potentials (VEPs).

Brain-stem death is a state in which brain-stem functions have ceased whereas the hemispheres still have electrical activity. We report a patient with a haematoma over the cerebellum and the pons who showed a preserved electroencephalogram and VEPs whereas all clinical signs of cerebral death was present. Normal VEPs in isolated brain-stem death have not yet been reported. We conclude that in any case of infratentorial lesion, an electroencephalogram should be recorded for the diagnosis of brain death.

Lumbar-spinal somatosensory evoked potentials in the rat after stimulation of the tibial nerve.

Spinal somatosensory evoked potentials after stimulation of the tibial nerve were recorded from the lumbospinal cord of rats. Their components and the respective latencies recorded over L5/6 and L1/2 in 40 normal animals are described. Using this method exact statements concerning lesions particularly of the proximal segment of the peripheral nerves and their roots can be made.

[Brain stem auditory evoked potentials for assessment of isolated fourth ventricle in three patients].

Isolated fourth ventricle is an untoward but interesting complication caused by the shunt procedure, which has been recognized since the introduction of CT scanning. Auditory brain stem evoked potentials (BAEPs) were recorded in a patient with isolated fourth ventricle for assessment of brain stem function. Case 1. A 9-year-old boy. He had undergone repair of the lumbosacral meningocele at birth, followed by shunting procedures, including repeated shunt revision, for hydrocephalus which subsequently developed. CT scanning on admission revealed typical findings of isolated fourth ventricle, whi…
