Infected non-union of the humerus after failure of surgical treatment: management using the Orthofix external fixator.

INTRODUCTION: The failure of a humeral fracture to unite after surgical treatment may be due to many factors. When there are additional complications of infection, treatment by conventional methods of internal fixation becomes very difficult. MATERIALS AND METHODS: We treated 8 infected non-union of diaphyseal fracture of the humerus by the Orthofix external fixator. All had previous surgical treatment. Non-union followed plating in 6 cases and in 2 cases after the external fixator. All patients had pain, at least one sinus discharging pus and severe functional impairment of the affected arm.

Introducing the external link model for studying spine fixation and misalignment: current procedures, costs, and failure rates.

OBJECTIVE: This is the last article in a series of 3 articles introducing a new animal model, the external link model (ELM), that permits reversible, nontraumatic control of the cardinal biomechanical features of the subluxation: fixation and misalignment. A detailed description of current ELM procedures is presented and practical issues are reviewed such as expense (dollars and time) and construct failure rates during and after the surgical implant procedure. METHOD: Descriptive report of current ELM procedures, refinements to the spinous attachment units (SAUs), and tabulation of costs and

Quality of life in patients with varus gonarthrosis treated with high tibial osteotomy using the circular external fixator.

Generally, surgeon-driven musculoskeletal evaluation systems are used for evaluating outcomes of patients who are treated with high tibial osteotomy. In this study, we investigated the effects of high tibial osteotomy using circular external fixator on quality of life. Twenty-one high tibial osteotomy of 19 patients were evaluated. Quality of life assessment was made using Short Form -36 at preoperative, before fixator removal and 6 months after fixator removal. After applying fixator, the physical function and physical role scores of Short Form-36 decreased and emotional role score did not im

[Hawkins type III fracture: dislocation of the talus and fracture of the medial malleolus treated by distraction external fixation].

These injuries are extremely rare and severe. The literature describes only individual cases of such injuries. We report a case of a displaced fracture of the talar neck with a complete posterior dislocation and rotation of the body of the talus (Hawkins type III) associated with medial malleolus fracture, treated with the method of urgent open reduction and internal fixation of the talar neck and medial malleolus as well as with the method of distraction external fixation.A 26-year-old male was injured after a fall from a height of over 8 m and was admitted as an emergency to the University O

Use of external fixation and primary wound closure in an open comminuted first metatarsal fracture: a case report.

Management of open, comminuted fractures presents a challenge for the foot and ankle surgeon. Reconstructive surgery for such injuries has a high potential for the development of serious complications, and factors such as the extent of soft tissue injury, neurovascular status to the foot, and fracture stability must be taken into consideration before determining a surgical plan. This article describes the case of a patient who presented with an open, comminuted first metatarsal fracture as a result of a chainsaw injury. The patient was treated with a uniplanar mini-external fixator, deminerali

Use of circular external fixation to maintain foot position during free tissue transfer to the foot and ankle.

Limb salvage techniques of traumatized extremities using free-tissue transfer and microsurgical techniques have become standard reconstructive methods. To our knowledge there is no published data on the incidence or likelihood of equinus following free tissue transfer about the ankle, although in our experience we have perceived an unacceptable incidence of equinus following free tissue transfers about the ankle and therefore initiated prophylactic ring fixation across the ankle. Fourteen patients were placed in circular external fixation spanning the ankle at the time of free tissue transfer

Using an external fixation "kickstand" to prevent soft-tissue complications and facilitate wound management in traumatized extremities.

External fixation represents an extremely effective and versatile means of treating severe musculoskeletal injuries of both upper and lower extremities, particularly in austere environments. A relatively simple modification of standard external fixation techniques can facilitate the care of complex soft-tissue wounds and prevent unnecessary wound complications. Additional carbon bars can be attached to the primary construct to create a "kickstand" that can effectively support the extremity and eliminate the risk for pressure ulcers. This kickstand modification, which also allows improved acces

Treatment of distal radius fractures with a wrist-bridging external fixation: the value of alternating electric current stimulation.

The radiological and functional outcomes of two groups of comminuted intraarticular fractures of the distal radius that were treated with wrist-bridging external fixation, with or without an alternating electric current stimulation (EF + ES or EF group, respectively), were compared. The radial length, radial inclination and palmar tilt were measured on radiographs taken immediately after removal of wrist external fixator and also after bone union had occurred. Furthermore, active range of motion was compared at the final consultation. Bridging callus was observed earlier postoperatively in the

Spiral and oblique fractures of distal one-third of tibia-fibula: treatment results with circular external fixator.

INTRODUCTION: Spiral and oblique fractures of distal 1/3 of tibia-fibula are relatively common fractures of long bones. Due to their types, aetiology, limited coverage and blood supply, these fractures often lead to union and soft tissue problems. MATERIALS AND METHODS: Twenty-seven patients with spiral and oblique fractures of distal 1/3 of tibia-fibula were treated with circular external fixator (CEF) between January 1997 and August 2000. All the fractures were closed. The type of fractures based on AO classification were A1 (n = 8), A2 (n = 6), B1 (n = 11) and C1 (n = 2). RESULTS: The mea

[Fracture of the distal lower limb: two-stage surgical treatment with external fixator and locked-screw plate].

From January 1999 until October 2004 we treated 42 patients with closed fractures of the distal calf by a two-stage treatment plan: first the fracture was stabilized with an external fixator bridging the ankle joint. Second, after an average period of 8.3 days, an open reduction and internal fixation with a locked-screw implant followed. As complications we saw a superficial wound necrosis in 3 cases, 2 patients needed an early bone graft after insufficient bone healing and in one case a deep vein thrombosis of the thigh occurred. A deep infection or osteitis were not seen. 17 patients showed

Supra-acetabular placement of external fixator pins: a safe and expedient method of providing the injured pelvis with stability.

Applying a stable anterior pelvic external fixator frame is a skill that should be mastered by all orthopedic surgeons who treat acutely injured patients. Splinting of an unstable pelvis during resuscitation can help to reduce the volume of the true pelvis, pending definitive surgical stabilization of the pelvic ring. Supra-acetabular pin placement, less familiar to most surgeons than iliac wing pin placement is, can provide a more reliable pin-bone interface and thus allow improved reduction ability with fewer soft-tissue complications. Because of their location, supra-acetabular pins also se

Risk factors for deep infection in secondary intramedullary nailing after external fixation for open tibial fractures.

BACKGROUND: Risk factors for deep infection in secondary intramedullary nailing (IMN) after external fixation (EF) for open tibial fractures were investigated by multivariate analysis following univariate analyses. METHODS: Forty-two open tibial fractures were treated with secondary IMN after EF. The open tibial fractures were classified according to the criteria proposed by Gustilo et al.: type II, 11; type IIIA, 8; type IIIB, 22 and type IIIC, 1. Locked IMNs with limited reaming were performed in 27 patients, and locked IMNs without reaming in 15 patients. The following factors contributing

External fixation of closed metacarpal and phalangeal fractures of digits. A prospective study of one hundred consecutive patients.

Stable and undisplaced phalangeal and metacarpal fractures treated with strapping or functional splinting and controlled active exercises offer from about 70 to 80% of good results. The goal of treatment of closed unstable and displaced fractures should be to achieve similar or better outcome. External fixation combines the simplicity of time-honoured fixation with K-wires and an external frame to increase rigidity. This combination was used in a prospective study of 100 consecutive patients with closed fractures of the proximal and middle phalanges and the distal three-quarters of the metacar

Treatment results for open comminuted distal humerus intra-articuler fractures with Ilizarov circular external fixator.

In open, intra-articular distal humerus fracture caused by gunshot injury, full functional recovery is difficult to obtain. Three basic treatment methods are available: minimal internal fixation, open reduction-internal fixation, and external fixation. In Gülhane Military Medical Academy Department of Orthopedics and Traumatology, 19 of 20 cases of gunshot injuries were treated with circular external fixator between the January 1995 and December 2000. Nine (45%) cases were type III-A, eight (40%) were type III-B, and three (15%] were type III-C. Eight (40%) cases were brought to the hospital 6

Hydroxyapatite coating of external fixation pins to decrease axial deformity during tibial lengthening for short stature.

BACKGROUND: Tibial valgus, a known complication of leg lengthening with external fixation techniques, has been related to the stability of the bone-fixator system and, in particular, to pin loosening. A hydroxyapatite coating has been reported to enhance the quality of the bone-pin interface. The aim of this study was to compare the prevalence of axial deformity after tibial lengthening with hydroxyapatite-coated external fixation pins with the prevalence after tibial lengthening with uncoated pins. METHODS: We conducted a prospective study of thirty-four symmetrical tibial lengthening proced

[Strain gauge measurements on a one-side external fixation: a prospective series of 11 tibial shaft fractures].

PURPOSE OF THE STUDY: External fixation is often the treatment of choice for open complex fractures of the tibia. For closed tibial shaft fractures, it is generally a second choice alternative. The purpose of this study was to determine whether fusion of closed and open fractures of the tibia can be successfully achieved with a one-side external fixator equipped with strain gauges and to evaluate complications of this type of treatment. MATERIAL AND METHODS: A consecutive series of 11 mid shaft fractures of the tibia (4 closed fractures and 7 Gustilo grade I and II open fractures) in 11 patie

Treatment of the sequelae of primary pathology of the hip by correction osteotomy and femoral lengthening using a combined external fixator.

It is not an exceptional event to observe the sequelae of primary pathology of the hip in young patients that, as such, are not of the ideal age for replacement surgery. Moreover, one must not overlook the difficulties involved in replacement surgery for the treatment of inveterate modifications in the normal hip anatomy. Often, among the sequelae of primary pathology of the hip, deformities of the lower limb must also be included, associated or consequent, which also need treatment. For these reasons, the authors, in three selected cases, used a combined external fixator (circular and lateral

Two case reports of a technique of medial external fixation in calcaneal fractures: indirect control of the sustentacular fragment.

In attempted open reduction and internal fixation of displaced calcaneal fractures, comminution of the sustentacular fragment can pose major problems. Two cases of a form of external fixation are presented. The method takes advantage of the strong superomedial and plantar calcaneonavicular ligaments, which link the sustentaculum to the navicular. Traction along these structures (by pins placed in the first metatarsal) indirectly controls the sustentacular fragment, while countertraction from medially placed pins in the tuberosity allows derotation of the fragment and correct repositioning in r
