Cranial hemophilic pseudotumor. Case report.

Hemophilic pseudotumor is a rare complication of hemophilia occurring in 1% to 2% of individuals with a severe factor VIII or IX deficiency. This pseudotumor has been defined as a progressive cystic swelling involving muscle that is produced by recurrent hemorrhage and may be accompanied by roentgenographic evidence of bone involvement. The case is presented of a 12-month-old child with mild factor VIII deficiency (10% of normal factor VIII activity), who developed a pseudotumor of the skull.

Room temperature, microtray chromogenic assay of factor VIII:C.

Commercial reagents of non-human origin were used to provide a room temperature, microtray chromogenic assay for factor VIII. The assay allows determination of factor VIII in the range 15-150% of normal plasma levels with a coefficient of variation of 3-4%. Haemophilic plasmas and factor VIII concentrates may also be assayed by varying the pre-dilution step. A good correlation was demonstrated with results obtained using an established one-stage clotting assay (r = 0.9985).

Detection of heterozygotes in both parents of homozygous patients with Von Willebrand's disease.

Three patients with severe Von Willebrand's disease are shown to be homozygotes. They were born from unaffected parents. New techniques using a factor-VIII-related antigen assay by the Laurell method and a ristocetin-induced platelet aggregation assay demonstrated abnormalities in these two tests in both parents of the probands. Factor-VIII-related of heterogotes could not be differentiated from normal factor-VIII-related antigen by the immunodiffusion technique, crossed immunoelectrophoresis, and filtration on a sepharose 4b column.

The effects of epinephrine infusion in patients with von Willebrand's disease.

Epinephrine infusion causes variable increases in the components of the Factor VIII (antihemophilic factor) complex in patients with von Willebrand's disease. The increase in antihemophilic factor procoagulant activity was greater than that of Factor VIII-related antigen and von Willebrand factor activity in two patients with von Willebrand's disease. Similar increases in the three individual factors were demonstrated in two other patients. A 4-10-fold increase in Factor VIII-related properties was identified in each of these individuals after infusion. One patient has been studied with very s…

Surgical management of endometriosis in a severe Hemophilia A female patient and the role of transfusion medicine specialist: A case report with review of literature.

Hemophilia A is an X-linked recessive bleeding disorder occurs due to deficiency of factor VIII (FVIII). The disease manifests exclusively in males though it rarely occurs in females due to complex pathophysiological mechanisms. We present a rare case of female hemophilia due to skewed X-inactivation which adversely affected the quality of patient life. She presented with recurrent abdominal pain and was diagnosed with severe endometriosis and underwent total abdominal hysterectomy with left salpingo-oophorectomy and appendicectomy. She was infused recombinant factor VIII both prophylactically…

Acquired Hemophilia A: A Rare but Potentially Fatal Bleeding Disorder.

Acquired hemophilia A is a disorder of rare entity, resulting in spontaneous bleeding in individuals with no history of bleeding disorders. It is believed to be caused by spontaneous inhibition of clotting factor VIII by autoantibodies, and is usually associated with other autoimmune conditions. The hallmark of this condition is mucocutaneous bleeding leading to ecchymosis, melena, hematoma or hematuria. Our discussion revolves around the case of an elderly male with no history of anticoagulant use presenting with hematuria. Imaging showed left kidney hemorrhage, his labs were significant for …

Hemophilia: role of organ homografts.

The concentration of factor VIII and partial thromboplastin times became normal and have remained normal for 140 days after orthotopic tranplantation of a normal liver to a hemophilic dog. Transplantation of a normal spleen into a hemophilic recipient did not result in a significant increase in factor VIII although the splenic graft was viable for at least 47 days. Transplantation of normal marrow to a lethally irradiated hemophilic dog did not result in an increase in factor VIII during 34 days of observation.

The role of Willebrand factor in platelet - blood vessel interaction, including a discussion of resistance to atherosclerosis in pigs with von Willebrand's disease.

Von Willebrand pigs have all the manifestations of the severe human disease. The role of Willebrand antigen (VIII R:AG) and ristocetin cofactor (VIII: RWF) was assessed in these pigs by (1) transfusion and (2) "in vitro" bleeding time assay. The skin bleeding time became normal when the level of transfused Willebrand factor (VIII R:AG/RWF) was raised in the plasma above 30 U/dl. After single or repeated transfusions, skin capillary endothelium and platelets were still distinguished from normal by VIII R:AG deficiency. When incisions in excised porcine skin ("in vitro" bleeding time) were perfu

Two-Year Outcomes of Valoctocogene Roxaparvovec Therapy for Hemophilia A.

BACKGROUND: Valoctocogene roxaparvovec delivers a B-domain-deleted factor VIII coding sequence with an adeno-associated virus vector to prevent bleeding in persons with severe hemophilia A. The findings of a phase 3 study of the efficacy and safety of valoctocogene roxaparvovec therapy evaluated after 52 weeks in men with severe hemophilia A have been published previously. METHODS: We conducted an open-label, single-group, multicenter, phase 3 trial in which 134 men with severe hemophilia A who were receiving factor VIII prophylaxis received a single infusion of 6×1013 vector genomes of valoc…

Closure of a ventricular septal defect in a patient with von Willebrand disease.

Few reports of open heart surgery being performed in patients with von Willebrand disease (vWD) have been documented. We describe herein the case of a 5-year-old girl with a ventricular septal defect (VSD) and vWD who underwent patch closure under cardiopulmonary bypass (CPB). The hematological parameters relating to vWD, namely, factor VIII, von Willebrand factor, and factor VIII-related antigen were monitored perioperatively. These laboratory findings were elevated after the termination of CPB, and consistently maintained within the normal range for 4 months postoperatively. The operation wa…

Gene therapy of the hemophilias.

Development of hemophilia gene therapy depends on testing gene transfer vectors in hemophilic and nonhemophilic animals. Available animal models include factor VIII or factor IX knockout mice as well as dogs with spontaneous hemophilia A or B. Large animals (particularly dogs) more closely replicate the requirements for correction of human hemophilia than do mice. Small animals are more convenient to maintain and require significantly less vector for testing than do large animals. Nonhemophilic animals (mice or nonhuman primates), whose endogenous factor VIII and factor IX complicate analysis …

[Anesthetic management of emergency cesarean section in a patient with acquired von Willebrand syndrome].

Acquired von Willebrand syndrome (AvWS) is generic name of bleeding disorders caused by acquired decrease of von Willebrand factor. A 25-year-old woman with AvWS underwent emergent cesarean section under combined spinal and epidural anesthesia. Before operation, her APTT was within normal ranges. Platelet count was about 80,000 and factor VIII and von Willebrand factor's activities were 37% and 21%, respectively. Therefore, we administered 10 units of platelet concentrate and 2,000 units of factor VIII concentrate intravenously before the operation. The emergent operation was done without any …

[DDAVP: alternative to replacement treatment in mild haemophilia A and von Willebrand-Jürgens syndrome (author's transl)].

1-Deamino-8-D-arginine vasopressin (DDAVP), a vasopressin analogue, increases factor VIII levels in plasma. A female carrier of haemophilia A with known bleeding diathesis and markedly reduced factor VIII activity was successfully treated with DDAVP during bilateral Caldwell-Luc's operation for chronic maxillary sinusitis. An estimated 5000 U factor VIII concentrate was calculated to have been saved. DDAVP is the first alternative to replacement therapy in the treatment of moderate and mold forms of haemophilia A and von Willebrand's disease.

Factor 8 detection by hemagglutination inhibition: hemophilia A and von Willebrand's disease.

Factor VIII activity was detected immunologically in both the serums and plasmas of 14 normal individuals and 14 patients with hemophilia A. A hemagglutination-inhibition test with rabbit antibody to highly purified (10,000-fold) factor VIII from humans was used. Serums and plasmas from eight patients with von Willebrand's disease showed little or no factor VIII activity in this test, an indication that the test may serve as a specific assay for differentiation between von Willebrand's disease and hemophilia A.

Postpartum Acquired Hemophilia A.

Acquired hemophilia A (AHA) is a bleeding disorder that occurs from aberrant production of autoantibodies that target factor VIII. The underlying cause of AHA is unclear but can present postpartum. Very few cases have reported instances of AHA coexisting with other hematological disorders, such as sickle cell trait (SCT). Although rare and no direct correlation between the two, critical situations involving intractable bleeding can intensify the severity of these disorders. A 31-year-old pregnant female with a medical history significant for SCT presented to the hospital for a C-section. Short…

Open-heart surgery in von Willebrand's disease.

The case is presented of a hemophilia carrier, also affected by von Willebrand's disease, who underwent aortic valve replacement. The clinical and laboratory findings of von Willebrand's disease (prolonged bleeding time, low factor VIII, and abnormal platelet activity) are discussed, and a protocol for management of patients with low factor VIII levels (such as hemophilia carriers and subjects with hemophilia A or von Willebrand's disease) undergoing open-heart surgery is proposed. Our case proves that corrective cardiac surgery in similar circumstances is feasible so long as adequate levels o…

Successful liver transplant from a hemophilia A donor with no development of hemophilia A in recipient.

BACKGROUND: Hemophilia A is an X-linked inherited bleeding disorder caused by deficiency of coagulation factor VIII. Factor VIII is activated as part of the intrinsic coagulation cascade. It plays a crucial role as the cofactor in the intrinsic "tenase" complex activating factor X to assist in clot formation. Absence or mutation of this coagulation factor leads to excessive bleeding. Clinical manifestations of hemophilia relate to bleeding from impaired hemostasis, sequelae from bleeding, or complications of coagulation factor infusion. Diagnostic criteria for Hemophilia A include factor VIII …

Acquired hemophilia A treated with rituximab in a 62-year-old female with rheumatoid arthritis: a case-based review.

Acquired hemophilia A (AHA) is a rare autoimmune disorder with unpredictable hemostasis that is caused by autoantibody formation against coagulation factor VIII. AHA can occur in the context of autoimmune inflammatory rheumatic disorders. Here we report the case of a 62-year-old female with an 11-year history of rheumatoid arthritis (RA) who presented with cutaneous and mucosal bleeding. Activated partial thromboplastin time was prolonged and not corrected by the mixing test. Factor VIII activity was decreased, and the anti-factor VIII antibody was positive. AHA associated with RA was diagnose…

Heterogeneity of von Willebrand's disease: study of 40 Iranian cases.

Forty Iranian patients with von Willebrand's disease were tested for bleeding time, platelet retention to glass beads, ristocetin-induced platelet aggregation, and assay of factor VIII procoagulant activity (VIII:C), Willebrand factor activity (VIIIR:WF), and factor VIII-related antigen (VIIIR:AG) by two methods (Laurell and immunoradiometric assay). In 22 cases from 11 families, levels of VIII:C, VIIIR:WF and VIIIR:AG (Laurell) were below 5% and the immunoradiometric assay showed total lack of VIIIR:AG in all cases (sensitivity of the method 0.01%). In 10 of these families, the parents were r…

Coagulant component in Cerastes cerastes (Egyptian sand viper) venom.

A coagulant component has been purified from Cerastes cerastes venom, using gel filtration on a Sephadex G 100 (fine) column followed by chromatography on a Bio-Rex 70 column. This compound had a proteolytic effect and could coagulate human plasma deficient in factor VIII or (VIII + IX) with the formation of a firm clot. It could also clot plasma deficient in factor X, but the clot formed was soft and not complete. The compound had no effect on platelet aggregation and was nontoxic. This compound is believed to be primarily a factor X activator, as it could replace factor VIII in hemophilic pl…
