Necrotizing fasciitis is a rare but serious disease, and early diagnosis is essential to reducing its substantial morbidity and mortality. The 2 cases presented show that the key clinical and radiographic features of necrotizing fasciitis exist along a continuum of severity at initial presentation; thus, this diagnosis should not be prematurely ruled out in cases that do not show the dramatic features familiar to most clinicians. Although computed tomography and magnetic resonance imaging are considered the most effective imaging modalities, the cases described here illustrate how sonography s…

A 57-year-old obese female with uncontrolled diabetes mellitus type I presented to the emergency department (ED) with the main complaint of worsening pain and redness in her right groin and inguinal regions. In the ED, a CT scan confirmed the likely presence of necrotizing fasciitis in the right groin and thigh. She was also found to be febrile and septic on admission. She was urgently taken to the OR for extensive debridement then admitted to the ICU for medical stabilization. She was subsequently taken back for serial debridements, and a negative pressure wound therapy with instillation (NPW…

Retroperitoneal fasciitis is a rare but potentially lethal complication of infection. Early diagnosis is crucial and is usually made when there is a high degree of clinical suspicion combined with characteristic imaging findings leading to early surgical intervention. Computed tomography (CT) can play a central role in demonstrating early findings, assessing the extent of disease to help determine the best surgical approach, identifying the primary source of infection, and evaluating the treatment response. The possible presence of retroperitoneal fasciitis should be considered in patients pre…

Necrotizing fasciitis is a soft tissue infection uncommonly described in children and is associated with significant morbidity and mortality if not treated early and aggressively. Reports of cases involving the upper torso are rare in general. In adults, necrotizing fasciitis is most commonly described in the abdomen, perineum, and extremities. For children, particularly neonates, necrotizing fasciitis most commonly involves the trunk presenting as omphalitis. In this report, we describe 2 pediatric cases of necrotizing fasciitis of the chest wall that presented within 6 months from each other…

Nodular fasciitis is a benign proliferative lesion composed of fibroblast-like cells that affects various sites in the body. We describe a patient with nodular fasciitis in the mesentery, encountered during laparotomy for the treatment of ascending colon cancer. The nodular fasciitis in our patient resembled peritoneal dissemination of malignancy on macroscopic observation. Because the treatment options change with concomitant peritoneal dissemination of gastrointestinal tract malignancy, recognition of this rare condition and preparation for unexpected nodular lesions are crucial.

Eosinophilic fasciitis (EF) is a rare localized fibrosing disorder of the fascia whose diagnosis is often suspected based on clinical findings and laboratory values. These lab abnormalities can be transient in early disease and may not always be present. We have reviewed a case series of patients to assess the utility of the various laboratory abnormalities in diagnosing EF. We performed a retrospective review of EF patients seen at Georgetown University Hospital in the Division of Rheumatology during 2009 and 2013. This review included 15 adult patients with EF with a mean age at diagnosis of…

Necrotizing fasciitis is a rapidly progressive life threatening bacterial infection of the skin, the subcutaneous tissue, and the fascia. We present a case of necrotizing fasciitis involving the nose complicated by cavernous sinus thrombosis. Few cases of septic cavernous sinus thrombosis have been reported to be caused by cellulitis of the face but necrotizing fasciitis of the nose is rare. It is very important to recognize the early signs of cavernous thrombosis. Treatment for septic cavernous sinus thrombosis is controversial but early use of empirical antibiotics is imperative.

Necrotizing fasciitis is a rare complication of nephrotic syndrome in children, with a high mortality rate. We report a case with successful outcome with judicious intravenous antibiotics and skin grafting of the bilateral lower thighs.

PURPOSE: To describe our experience with nodular fasciitis in the pediatric population, in an attempt to contribute additional information that may be used to develop a more comprehensive characterization of the disease.
METHODS: A twenty-year retrospective chart review of patients 18 years or younger diagnosed with nodular fasciitis at a single, large, academic institution.
RESULTS: Five patients were treated for nodular fasciitis during the study period. The majority (60%) were male and all presented with rapidly enlarging masses that were excised surgically. The median age at presentation…

Nodular fasciitis is a benign fibroblastic proliferate lesion that is thought to be a response of tissue to injury. It usually presents as a rapidly growing subcutaneous mass. Upper extremities are most commonly affected, followed by the head and neck region. The clinical behaviour and histological presentation mimic those of malignant tumours, thus the lesion is easily misdiagnosed. The authors report here three cases of orofacial nodular fasciitis and review the literature focused on the diagnosis of the disease. Patient one had a history of trauma. The physical examination, radiographic fea…
