A 55-year-old fisherman sustained an unusual and irreducible fracture-dislocation of the ankle joint. An entrapped medial malleolar fragment was discovered by computed tomography between the distal tibia and lateral malleolus. The fragment was removed during an emergent open reduction, during which deltoid ligament repair and internal fixation of the bimalleolar fracture were undertaken. At the 18-month follow-up visit, the patient was working normally and was asymptomatic. At previous visits, he demonstrated a full range of ankle motion and had normal radiograph.

We recently encountered a child who fell on the left outstretched hand. She complained of left elbow pain subsequently and radiographs showed a supposedly isolated radial head dislocation. This was spontaneously reduced when the left forearm was extended for another radiograph. She was discharged from the emergency department with her left elbow in a long backslab, but returned within half an hour with recurrence of the radial head dislocation without repeat trauma. Only when the radiograph was reviewed was a subtle plastic bowing deformity of the ulna seen. This proved the axiom, that in fore

We retrospectively reviewed the results of closed treatment in 7 patients with acute locked posterior fracture-dislocations of the shoulder. Shoulders were reduced with patients under general anesthesia and were splinted in neutral rotation for 6 weeks. Follow-up averaged 46 months (range, 24-88 months). All shoulders were reduced within 14 days. Humeral head defects ranged from 18% to 32% of the articular surface, and all patients had documented posterior instability under anesthesia after relocation. Of the 7 patients, 6 had a stable shoulder. The mean American Shoulder and Elbow Surgeons sh

PURPOSE: Many skeletal traction devices have been described to treat fracture dislocations of the proximal interphalangeal (PIP) joint. Most of these techniques are technically challenging or involve cumbersome frames. We present a design modification that enhances the stability of a simple dynamic fixation system described previously and report our results with this technique.
METHODS: A previously described simple dynamic fixator with no rubber bands was applied to 6 patients who sustained fracture dislocations of the PIP joint. The middle finger was involved in 3 patients, the ring finger …

Bilaterally dislocated intraarticular calcaneus fractures in childhood are rare. This case report shows that adequate preoperative tissue management, open anatomical reduction, and lateral plate osteosynthesis by an experienced surgeon according to the principles of adult surgery achieve a good functional result.

Bilateral simultaneous anterior dislocation of the shoulders with bilateral 3-part fracture of the proximal humeri is unusual. A 42-year-old man presented with pain and restriction of movement on both shoulders. He was injured by a heavy object falling over his back while he was leaning forward holding an overhead bar. His arms were abducted and externally rotated. The injury was not correctly diagnosed, and the patient was treated with repeated manipulations and splintage for 2 weeks. Radiological examination revealed bilateral anterior dislocation of the shoulders with displaced 3-part fract

Our goal with this study was to better define and characterize fracture-dislocations of the olecranon and to provide additional data regarding complications and elbow function after operative treatment. Twenty-six patients with fracture-dislocations of the elbow were reviewed retrospectively. Ten had anterior and 16 had posterior fracture-dislocations. Five of 10 patients with anterior injuries and all of the patients with posterior injuries had an associated fracture of the coronoid process of the ulna. One of 10 patients with anterior and 13 of 16 patients with posterior injuries had fractur

We describe a case of transphyseal hip fracture-dislocation in a 7.5-year-old patient who was treated initially by open reduction and internal fixation. Soon after the injury, the femoral head developed avascular necrosis. The treatment was focused on maintaining adequate hip range of motion and providing femoral head containment with a combined subtrochanteric femoral osteotomy and shelf acetabuloplasty. The patient's young age and good hip remodeling potential contributed to the favorable clinical outcome 3 years after the injury. The long-term prognosis remains guarded, however.

We reviewed 42 patients (mean age 37.7+/-14.2 years) with closed fracture dislocations of Lisfranc's joint treated with percutaneous screw fixation. Mean follow-up was 58.4+/-17.3 months. The aim was to compare dislocations in which a perfect anatomical reduction had been reached with dislocations in which reduction was only near anatomical. The mean American Orthopaedic Foot and Ankle Society score for all patients was 81.0+/-13.5. There were no significant differences in outcome scores between patients with perfect anatomical reduction and patients with near anatomical reduction. However, pa…

Fracture of the femoral head after hip dislocation is a relatively rare injury often associated with a poor functional outcome. Twenty-six patients who sustained femoral head fractures were evaluated using radiographs, clinical examinations, and a validated outcome scoring system. The Short Form-12 was used to assess functional outcome. Patients whose fractures were stabilized with 3-mm cannulated screws and washers had a poor functional outcome. When evaluated with an odds ratio analysis, the use of Kocher-Langenbeck posterior approach was associated with a 3.2 times higher incidence of the p

Between October 1995 und December 1997, 57 patients with displaced 2-, 3- and 4-part fractures of the proximal humerus were treated by open reduction and internal fixation with two one-third tubular plates which were applied to the anterior and lateral aspect of the proximal humerus. For the follow-up evaluation shoulder function was assessed in 38 patients after an average of 16.8 +/- 4.2 months using the Constant-score: 32% of the patients showed excellent, 37% good, 21% satisfactory, and 10% unsatisfactory results. The high stability of this technique allows intensive physiotherapy in the e

STUDY DESIGN: A retrospective, single-patient case report.
OBJECTIVES: To report on a 9-month-old infant with fracture-dislocation at the thoracolumbar junction with locking of vertebral bodies, probably with incomplete neurologic injury and partial postoperative recovery.
SUMMARY OF BACKGROUND DATA: Fracture-dislocation at the thoracolumbar junction is rare in infants, the upper cervical spine being the level usually affected. Incomplete neurologic injury is uncommon after such a severe shear injury.
METHODS: The patient was followed through personal examination and chart review from initi

A twenty-year-old, active male with a high congenital dislocation of the hip sustained a displaced subcapital fracture (accompanied by a comminuted transarticular fracture of the ipsilateral distal femur). Such a combination of injuries poses a challenging problem; the English literature does not cite any report describing the occurrence of subcapital fractures in congenitally dislocated hips. This article reviews the treatment options of the fractured neck of the femur, and the reasons for electing to perform a Girdlestone resection arthroplasty.

This study was undertaken to evaluate the occurrence of Charcot joint changes in diabetic patients after fractures and/or dislocations of the foot and ankle. There were 20 fracture/dislocations of the foot and ankle in 18 patients, with an average follow-up of 27 months (range 14-70 months). There were eight fractures of the midfoot, six fractures of the ankle, four fractures of the hindfoot, and two fractures of the forefoot. Eight fractures were followed by the development of Charcot changes: five in the midfoot and one each in the forefoot, hindfoot, and ankle. Of nine fractures recognized

Twenty-eight three- and four-segment fractures of the proximal humerus were treated by plate fixation (n = 22), screw or Kirschner wire stabilization (n = 5), and primary endoprothesis (n = 1). There were no early postoperative complications. Fourteen months after the operation, 80% of the patients examined showed good results; 4 cases had poor results. In comparison to other reports our results are positive and support plate fixation as the method of choice in these situations.
