Management of Displaced Radial Neck Fractures with Closed Technique using Titanium Elastic Nail and Review of Literature: A Case Report.

INTRODUCTION: Injuries around the elbow are common in pediatric population. Amongst them, most common is fractures of the supracondylar part of humerus, followed by condylar fractures. Fractures of the neck of the radius account for five-ten percent of the total injuries. Various techniques for closed reduction of the fracture have been described in literature. Open reduction and fixation is indicated when closed reduction fails to provide satisfactory results and the angulation remains more than 45-60 degrees. CASE REPORT: We present two cases of severely displaced radial neck fractures trea

Knee laxity associated with a proximal diaphyseal tibial and fibular fracture.

If operative treatment of a proximal diaphyseal tibial and fibular fracture is required, in most cases, it involves reduction and stabilization of only the tibial fracture. In this case report, after stabilization of the tibial fracture with an intramedullary nail, the patient continued to demonstrate significant varus knee laxity, despite an intact proximal tibiofibular joint and undisplaced fibular head. The stability of the knee was achieved only with internal fixation of the segmental fibular fracture. This case shows the importance of assessing knee laxity in all cases of proximal tibial

Fixation of Extra-articular Proximal Tibia Fractures: Biomechanical Comparison of Single and Dual Implant Constructs.

OBJECTIVES: This biomechanical study seeks to define the relative effectiveness of contemporary single and dual implant constructs for fixation of an extra-articular proximal tibia fracture model. METHODS: An extra-articular proximal tibia fracture model was created using synthetic tibias. Four constructs were tested. Constructs included (1) lateral locked plate (LLP), (2) intramedullary nail (IMN), (3) combined LLP and IMN (PN), and (4) LLP and medial locked plate. Specimens were axially loaded through the medial plateau to evaluate construct stiffness and the ability to resist varus collaps

Biomechanical Study of Intramedullary Versus Extramedullary Implants for Four Types of Subtrochanteric Femoral Fracture.

OBJECTIVES: To compare the biomechanical performance of proximal femoral nail anti-rotation (PFNA), the "upside-down" less invasive plating system (LISS), and proximal femoral locking plate (PFLP) in fixing different fracture models of subtrochanteric fractures. METHODS: Thirty composite femurs were divided into three equal groups (PFNA, PFLP, and reverse LISS). The implant-femur constructs were tested under axial compression load (0-1400 N) from models I to IV, which represented the Seinsheimer type I subtrochanteric fracture, type IIIa subtrochanteric fracture with the posteromedial fragmen

Pediatric Phalanx Fractures.

Phalangeal fractures are the most common type of hand fracture that occurs in the pediatric population and account for the second highest number of emergency department visits in the United States for fractures. The incidence of phalangeal fractures is the highest in children aged 10 to 14 years, which coincides with the time that most children begin playing contact sports. Younger children are more likely to sustain a phalangeal fracture in the home setting as a result of crush and laceration injuries. Salter-Harris type II fractures of the proximal phalanx are the most common type of finger

Rotational stability of femoral osteosynthesis in femoral fractures - navigated measurements.

Rotational malalignment after intramedullary nailing of femoral fractures is common, and symptoms occur when malrotation reaches 15 degrees . Intraoperative measurement of rotation remains difficult, and multiple techniques have been described to address this. Regardless of the method used, rotational toggling may occur between the interlocking screws and the screw holes. We hypothesized that a clinically significant amount of rotation may occur with standard statically locked intramedullary nails. Mid-shaft diaphyseal fractures were created in 24 cadaveric femurs. Specimens were divided into

Conversion of failed proximal femoral internal fixation to total hip arthroplasty in osteopetrotic bone.

Osteopetrosis is a rare disease. When fractures are encountered, fixation is extremely difficult. A 38-year-old man with osteopetrosis who sustained a displaced Pauwels type IV fracture with 3 failed internal fixations required total hip arthroplasty. Two main problems were perceived during preoperative planning: the removal of failed and broken metalwork and reaming of the tight intramedullary canal to allow seating of the femoral stem. We present technical solutions to aid the surgeon in the management of this difficult type of case using custom-made tungsten carbide instrumentation.

Treatment of infected tibial nonunion with angular deformity and shortening using the Masquelet technique combined with the Ilizarov technique: A case report.

The induced membrane technique (Masquelet technique) combined with the Ilizarov technique may provide a novel strategy for managing infected nonunion with angular deformity and limb shortening within a relatively short treatment period. We report a 45-year-old male with a Gustilo-Anderson type IIIA open tibial fracture complicated by fracture-related infection, 33° valgus deformity, and 50-mm limb shortening. Treatment was performed in three stages: radical debridement with rapid soft-tissue lengthening at 3 mm/day using an Ilizarov external fixator; definitive fixation with an intramedullary

The effect of haemorrhagic shock and resuscitation on fracture healing in a rabbit model: an animal study.

AIMS: Little is known about the effect of haemorrhagic shock and resuscitation on fracture healing. This study used a rabbit model with a femoral osteotomy and fixation to examine this relationship. MATERIALS AND METHODS: A total of 18 male New Zealand white rabbits underwent femoral osteotomy with intramedullary fixation with 'shock' (n = 9) and control (n = 9) groups. Shock was induced in the study group by removal of 35% of the total blood volume 45 minutes before resuscitation with blood and crystalloid. Fracture healing was monitored for eight weeks using serum markers of healing and rad

Functional Outcomes of Tibia Fractures Treated With Intramedullary Interlocking Nails by Suprapatellar Approach: A Prospective Study.

Background Tibia fractures are the most common lower extremity fractures. The subcutaneous anatomy of this long bone predisposes it to high fracture frequency in a high-energy trauma. The tibia is a major weight-bearing, long tubular bone that is axially and rotationally unstable when fractured, which ideally necessitates its surgical fixation in adults. Tibia fractures can be treated with a variety of choice of implants and surgical approaches. This study aims to assess the clinical and functional outcomes of a tibia fracture treated with intramedullary interlocking nails by a suprapatellar a

Treatment of comminuted tibial shaft fractures in four dogs with the use of interlocking nail connected with type I external fixator.

The treatment of comminuted tibial shaft fractures in canine patients is burdened by significant risk which involves bone healing complications, such as delayed bone union. Complications may result from iatrogenic damage to blood vessels during fracture stabilization. To minimize this risk, treatment methods increasingly often rely on the concept of biological osteosynthesis. One of such methods involves the treatment of fractures with the use of new hybrid fixator consisted of an interlocking nail connected with type I external fixator. Connection of the nail with external fixator has been re

Reverse guide wire technique for percutaneous anterior column screw of the acetabulum: A second look at the technique and a step-by-step guide. A surgical technique.

INTRODUCTION: Pelvis and acetabulum fractures are serious injuries, especially in polytrauma patients. Traditionally, open reduction with internal fixation is the way to go; fortunately, percutaneous fixation has evolved with the advancement of understanding pelvis anatomy and fixation pathways. Nonetheless, the debate regarding indications, safety, radiation exposure, and other complications emerged. Our paper presented a reverse guide wire technique for percutaneous anterior column/superior pubic rami intramedullary screw fixation. PRESENTATION OF CASE: A 24-year-old male presented after a

Risk factors for contralateral hip refractures in patients aged over 80 years with intertrochanteric femoral fractures.

PURPOSE: The purpose of this study was to identify which of the risk factors would contribute to the contralateral fracture in very elderly patients after intramedullary nail fixation. METHODS: Clinical data of 227 intertrochanteric fracture patients aged 80 years or older were retrospectively reviewed. Intramedullary nails (IMNs) were used on all of the patients. Potential risk factors for contralateral hip refractures were determined using univariate and logistic regression analyses. RESULTS: Contralateral hip refractures occurred in 11 patients (4.84%). Univariate analysis revealed that a

Intramedullary devices for tibial fracture stabilization.

Reamed tibial nails that allow interlocking have been the most significant recent advance in the management of tibial fractures. Interlocking nails have expanded the indications for intramedullary fixation to include most nonarticular tibial fractures. Regardless of the amount of comminution, tibial fractures extending from just distal to the tibial tuberosity to 5 cm above the ankle joint may now be safely stabilized with interlocking nail devices.

Articular Surface Involvement With Retrograde Headless Screw Fixation of the Proximal Phalanx.

PURPOSE: Intramedullary (IM) screw fixation of proximal phalanx (P1) fractures is a treatment option increasing in popularity. This study aimed to quantify the articular surface loss after retrograde screw insertion and to determine the range of motion (ROM) of the proximal interphalangeal (PIP) joint while the defect in the P1 head is engaged with the base of the middle phalanx (P2). METHODS: Twelve fresh frozen cadaver hand specimens were analyzed for prefixation ROM of the PIP joint. A retrograde screw was placed using a percutaneous technique under fluoroscopic guidance. Following screw i

The Use of the Fracture Table Post for Guidewire Placement in Hip Fracture Surgery.

Hip fractures, common among elderly patients, may result in significant disability and mortality. Fixation techniques often involve cannulated screws, sliding hip screws, or intramedullary nails. Optimal implant placement requires precise guidewire insertion from the lateral cortex into the femoral head. Variable femoral anatomy, particularly femoral version, can complicate guidewire placement despite fluoroscopy use, often requiring multiple screw passage attempts. The authors present a novel visual reference using a fracture table inguinal post to estimate femoral version and improve guidewi
