Outcomes after splenic injury in geriatric trauma: Is splenic embolization helpful?

BACKGROUND: The spleen is one of the most frequently injured organs in abdominal trauma and is often managed nonoperatively with low rates of failure. A common adjunct to nonoperative management (NOM) is splenic artery embolization (SAE) which is controversial in some patient groups. We hypothesized that SAE would confer no benefit in elderly trauma patients undergoing NOM. METHODS: This retrospective cohort study included patients in years 2019 to 2022 of the Trauma Quality Improvement Program database presenting after splenic trauma stratified into adult (age, 15-64 years) and elderly (age

[Special considerations in geriatric anesthesiology : Aspects of perioperative management in older adults].

Older people need a specialized preoperative risk assessment to reliably identify those highly vulnerable risk patients who are in danger of a poor postoperative outcome. In this way these vulnerable risk patients can be separated from those who have successfully and healthily aged and therefore still show a high resilience even in old age. Careful planning of the perioperative management with integration of all disciplines and professions involved can ensure a safe perioperative treatment even for high-risk patients. The corresponding possibilities are presented in this article. The periopera

[Quality control in geriatric psychiatry: development and applicability of documented quality indicators].

The development and suitability testing of quality indicators for geriatric psychiatry. The indicators were developed by a group of experts and evaluated in terms of significance, scientific validity and practicability (according to QUALIFY). After a field test, qualitative interviews were conducted with the evaluators. All 24 indicators originally developed were classified as significant, 18 of them as scientific. Of these, 9 (by experts) and 8 (by evaluators) were rated as practicable. Indicators on delirium prevention, age-specific groups and liberty-depriving measures were rated as non-scien

External validation of geriatric influenza death score: a prospective validation study.

OBJECTIVE: The global challenge of population aging continues to intensify. Current estimates project that by 2050, the world's population will approach 10 billion, with elderly people comprising an estimated 1.6 billion individuals. Due to age-related immunosenescence, older adults face a significantly elevated risk of serious influenza complications compared to younger individuals. During influenza epidemic seasons, determining the appropriate disposition (discharge, hospital admission or Intensive Care Unit admission) of elderly patients with influenza presents a significant challenge. In 2

Practical Implementation and Validation of Geriatric Assessment in Older Adults With Esophageal Cancer.

BACKGROUND/AIM: Esophageal squamous cell carcinoma (ESCC) is associated with poor prognosis, and older adults frequently present with frailty and comorbidities that complicate multimodal treatment. Although international guidelines recommend geriatric assessment (GA) to guide treatment decisions, clinical application in esophageal cancer remains limited in Japan. This study aimed to investigate the feasibility and clinical impact of GA in older adults with ESCC. PATIENTS AND METHODS: Forty patients aged ≥70 years with newly diagnosed esophageal cancer underwent GA at admission. The assessment

[Primary total knee arthroplasty in geriatric patients with tibial plateau fractures].

Fractures around the knee joint are per se challenging to treat, often regardless of the patient's age. Osteosynthesis remains the gold standard for fracture management; however, geriatric patients are often unable to comply with postoperative weight-bearing restrictions on the osteosynthetically treated limb. This can result in a prolonged hospital stay and particularly in aged patients to an increased risk of internal complications. Secondary total knee arthroplasty (TKA) is associated with both higher complication and revision rates compared to primary TKA for acute fractures. The advantage

Mortality predictors in community-dwelling centenarians through comprehensive geriatric assessment.

BACKGROUND: There has been a steady rise in the global population of centenarians. However, information on the factors affecting survival in community-dwelling centenarians remains limited. Comprehensive geriatric assessment (CGA), with its multidisciplinary approach, offers important advantages in identifying predictors of mortality in older adults. This research sought to determine the most common chronic diseases and factors associated with mortality in community-dwelling centenarians using the CGA. METHODS: The clinical documentation of community-dwelling individuals aged ≥ 100 years who a

[How can the suitability of a geriatric patient for uro-oncological treatment be determined?].

BACKGROUND: Geriatric patients undergoing uro-oncological treatments are particularly susceptible to complications in many organ systems. Prior to starting treatment, a careful risk-benefit analysis must be performed, taking into consideration the patient's remaining life expectancy and existing functional deficits. These should be weighed against the side effect profile of the planned treatment. OBJECTIVE: After an introduction of the three-step geriatric assessment to identify geriatric patients, systematically identify functional deficits, and evaluate them in depth, the example of androge

Use of potential gerotherapeutic drugs and mortality in geriatric rehabilitation inpatients: RESORT.

Repurposed existing drugs could serve as promising gerotherapeutic agents. However, their effectiveness in improving clinical outcomes among geriatric rehabilitation inpatients remains unclear. This study investigated the longitudinal observational association between potential gerotherapeutic drugs (angiotensin-converting enzyme inhibitors/angiotensin receptor blockers (ACEi/ARBs), aspirin, metformin, beta-blockers, and bisphosphonates) and changes in physical function, readmission within 90 days, and mortality up to two years in geriatric rehabilitation inpatients from the RESORT cohort. Presumed atypical cor triatriatum dexter in a geriatric dog: A case report.

Membranous structures within the right atrium (RA), such as the Chiari network, eustachian valve, Thebesian valve, and cor triatriatum dexter (CTD), can present with overlapping imaging features and complicated diagnoses. A 17-year-old male Maltese presented with a cardiac murmur, cough, anorexia, and exercise intolerance. Echocardiography revealed a mobile membrane in the RA, dividing it into two chambers. Computed tomography (CT) confirmed this finding, demonstrating differential attenuation of contrast between the chambers. The vascular connections and anatomical relationships of the struct

Microlearning to teach geriatric principles in hospitals: a systematic review and meta-analysis.

BACKGROUND: Hospital-based education in geriatric medicine is often limited in availability and accessibility. Microlearning is defined as short, on-the-go, focused educational interventions (<15 minutes) and offers a practical way to deliver key geriatrics concepts in time-constrained environments. This systematic review investigated the effectiveness of microlearning to learn geriatric medicine principles for hospital-based clinicians. METHODS: This systematic review with meta-analysis (PROSPERO: CRD42023422522) involved a comprehensive search across five databases. Studies evaluating micro…

Age or Frailty: What Predicts Outcomes in Geriatric Patients With Rib Fractures?

INTRODUCTION: Age and frailty are associated with poor outcomes in geriatric patients with rib fractures. To date, no studies have assessed the relative and independent association of each factor using a large national dataset. We therefore sought to determine whether age, frailty, or their combined consideration most effectively predicts outcomes in geriatric patients with rib fractures. METHODS: The 2017-2023 TQIP database was queried for all geriatric patients (age ≥65 y) who sustained at least one rib fracture. A multivariate analysis with propensity score matching was performed to examin

[Evaluation and risk assessment of geriatric ankle fractures? : What must be considered?].

The frequency of fractures in old age is steadily increasing including geriatric ankle fractures. These injuries are associated with increased morbidity and prolonged hospitalization. The consequences of courses prone to complications represent a relevant socioeconomic factor and the goal of treatment is to preserve the patient's mobility and quality of life. To ensure optimal perioperative care, the identification of geriatric patients, evaluation of the functional status and existing risk factors with subsequent interdisciplinary treatment are required. Various surgical procedures are availa

Locked fibular nailing in geriatric ankle fractures: A retrospective case series.

INTRODUCTION: The incidence of ankle fragility fractures is increasing. These insufficiency fractures frequently occur in a frail geriatric population and are associated with an increased risk of skin complications. The objective of this study was to evaluate the clinical and radiological outcomes of geriatric ankle fracture fixation using locked fibular nailing. HYPOTHESIS: We hypothesized that fixation of geriatric ankle fractures with a locked fibular nail would allow bone union without skin complications or mechanical failure. METHODS: This retrospective single-center study included 21 p

Autoimmune Encephalitis in Later Life: A Missed Opportunity in Routine Geriatric Care.

Autoimmune encephalitis (AE) is an increasingly recognized but frequently overlooked cause of subacute cognitive and behavioral decline in older adults, in whom delirium is typically attributed to metabolic, infectious, or vascular disorders. We describe an elderly man who presented with persistent encephalopathy following correction of hyponatremia. Despite normal structural neuroimaging and negative cerebrospinal fluid autoimmune markers, he remained delirious. Electroencephalography revealed diffuse slowing, and fluorodeoxyglucose positron emission tomography demonstrated characteristic hyp

Strategies to optimize the nutritional profile of mammalian meat for infants and geriatric population.

Early-life growth and aging represent physiological stages with increased protein requirements due to rapid tissue development, anabolic resistance and reduced ability to digest and absorb nutrients. Red meat, which includes beef, pork, lamb and goat, provides complete proteins containing all essential amino acids, and micronutrients including iron, zinc, phosphorus, selenium, B vitamins and bioactive molecules such as creatine, taurine and carnitine. This review evaluates the nutritional composition and digestibility of different meat types and meat cuts, and how processing technology may imp
