Surgery plays an important role in the treatment of glaucoma. For the angle-closure glaucomas, surgery is the primary mode of treatment to restore the normal route of aqueous out of the eye. For the open-angle glaucomas, surgery is usually a secondary treatment modality, indicated after medical therapy fails to halt glaucomatous damage or does not reduce the intraocular pressure to an acceptable level for an individual's eye.

The etiopathogenesis of iridoschisis is unclear, although age-related degeneration and angle-closure glaucoma are its most common associations. We report the case of a 50-year-old man with lens subluxation (familial) in 1 eye and ipsilateral iridoschisis. The inferotemporal edge of the lens was directed anteriorly, pushing the iris forward; this led to iridoschisis and shallowing of the anterior chamber. The patient had bilateral angle-closure glaucoma. A subluxated lens rubbing against the iris may be a mechanical precipitating factor in the development of iridoschisis.

Ocular hypertension was found in 54 (5.4%) of 1000 consecutive patients of an optometrist. In a separate trial, 50 ocular hypertensive subjects were followed for a mean follow-up time of 6.9 years. Five of the ocular hypertensive subjects (10%) developed primary open angle glaucoma and 2 (4%) developed primary angle-closure glaucoma. No single test could have enabled diagnosis in all five cases that developed open-angle glaucoma. Guidelines are recommended for the optometric management of patients having ocular hypertension.

PURPOSE: To study the clinical profile and distribution of various subtypes of glaucoma in a referral practice in North India.
METHOD: A retrospective analysis was done of 2425 patients who attended the glaucoma clinic in a tertiary eye-care centre for five years from January 1995 to December 1999. A detailed history was obtained and a thorough examination was performed, including gonioscopy, disc assessment, applanation tonometry and automated perimetry. Cataract surgery with intraocular lens (IOL) implantation is increasingly preferred over laser peripheral iridotomy as the first-line management of primary angle-closure glaucoma after endorsement by the recently reported EAGLE (Effectiveness of Early Lens Extraction for the Treatment of Primary Angle-Closure Glaucoma) study. However, we present 2 eyes of a 63-year old woman, both of which remained shallow after cataract surgery, with 1 eye developing high and uncontrolled intraocular pressure.
