Can Illness Perceptions Predict Lower Heart Rate Variability following Acute Myocardial Infarction?

Objective: Decreased heart rate variability (HRV) has been reported to be a predictor of mortality after myocardial infarction (MI). Patients' beliefs and perceptions concerning their illness may play a role in decreased HRV. This study investigated if illness perceptions predict HRV at 3 months following acute MI. Methods: 130 patients referred to a tertiary cardiology center, were examined within 48 h and 3 months following acute MI. At admission, patients' cognitive representations of their MI were assessed using the German version of the self-rated Brief Illness Perception Questionnaire (B…

Sleep related bradyarrhythmic events and heart rate variability in apparently healthy individuals.

OBJECTIVE: It is thought that abnormal cardiac impulses of the autonomic nervous system during sleep are responsible for sleep-related bradyarrhythmias. Despite a proposed common etiopathogenesis and having common name of "sleep-related bradyarrhythmias," precise importance of sinoatrial or atrioventricular (AV) node involvement remains elusive. This study aimed to determine whether there is a difference in sleeprelated bradyarrhythmias from the point of view of heart rate variability (HRV). METHODS: Patients were evaluated using 24-hour Holter electrocardiogram monitor. After careful medical…

Bailout intravenous esmolol for heart rate control in cardiac computed tomography angiography.

OBJECTIVE: To evaluate the efficacy and safety of a heart rate (HR) reduction protocol using intravenous esmolol as bailout for failed oral metoprolol regimens in patients undergoing coronary computed tomography angiography (CCTA) with 64-slice multidetector computed tomography (64-MDCT). METHODS: Patients who underwent cardiac 64-MDCT in a single institution between 2011 and 2014 were analyzed. Those with HR above 60 beats per minute (bpm) on presentation received oral metoprolol (50-200 mg) at least one hour before CCTA. Intravenous esmolol 1-2 mg/kg was administered as a bolus whenever HR …

Control of the heart rate of rat embryos during the organogenic period.

The aim of this study was to gain insight into whether the first trimester embryo could control its own heart rate (HR) in response to hypoxia. The gestational day 13 rat embryo is a good model for the human embryo at 5-6 weeks gestation, as the heart is comparable in development and, like the human embryo, has no functional autonomic nerve supply at this stage. Utilizing a whole-embryo culture technique, we examined the effects of different pharmacological agents on HR under normoxic (95% oxygen) and hypoxic (20% oxygen) conditions. Oxygen concentrations ≤60% caused a concentration-dependent …

Heart rate response to therapeutic hypothermia in infants with hypoxic-ischaemic encephalopathy.

AIM OF THE STUDY: Neonatal encephalopathy (NE) of hypoxic-ischaemic origin may cause death or life-long disability which is reduced by therapeutic hypothermia (TH). Our objective was to assess HR response in infants undergoing TH after perinatal asphyxia. METHODS: We performed a retrospective case series, from a single-centre tertiary care NICU. We included ninety-two infants with NE of likely hypoxic-ischaemic origin, moderate or severe, treated with TH (n=60) or normothermia (n=32) who had 18 month outcome data and at least 12 HR recordings the first 24h after birth (1998-2010) Bristol, UK.

Effect of tocolytic drugs on fetal heart rate variability: a systematic review.

INTRODUCTION: Tocolytics may cause changes in fetal heart rate (HR) pattern, while fetal heart rate variability (HRV) is an important marker of fetal well-being. We aim to systematically review the literature on how tocolytic drugs affect fetal HRV. MATERIALS AND METHODS: We searched CENTRAL, PubMed and EMBASE up to June 2016. Studies published in English, using computerized or visual analysis to describe the effect of tocolytics on HRV in human fetuses were included. Studies describing tocolytics during labor, external cephalic version, pre-eclampsia and infection were excluded. Eventually, …

Predicting Bradycardia in Preterm Infants Using Point Process Analysis of Heart Rate.

OBJECTIVE: Episodes of bradycardia are common and recur sporadically in preterm infants, posing a threat to the developing brain and other vital organs. We hypothesize that bradycardias are a result of transient temporal destabilization of the cardiac autonomic control system and that fluctuations in the heart rate signal might contain information that precedes bradycardia. We investigate infant heart rate fluctuations with a novel application of point process theory. METHODS: In ten preterm infants, we estimate instantaneous linear measures of the heart rate signal, use these measures to ext…

Vagal Nerve Stimulation Evoked Heart Rate Changes and Protection from Cardiac Remodeling.

This study investigated whether vagal nerve stimulation (VNS) leads to improvements in ischemic heart failure via heart rate modulation. At 7 ± 1 days post left anterior descending artery (LAD) ligation, 63 rats with myocardial infarctions (MI) were implanted with ECG transmitters and VNS devices (MI + VNS, N = 44) or just ECG transmitters (MI, N = 17). VNS stimulation was active from 14 ± 1 days to 8 ± 1 weeks post MI. The average left ventricular (LV) end diastolic volumes at 8 ± 1 weeks were MI = 672.40 μl and MI + VNS = 519.35 μl, p = 0.03. The average heart weights, normalized to body wei…

Heart rate variability during adolescent and adult social interactions: a meta-analysis.

Social interaction skill is important for psychological wellbeing, stress regulation, protection from disability and overall life satisfaction. Increase in activity of the vagus nerve, measured by heart rate variability (HRV), is associated with social interaction skill and decreased stress. In this meta-analysis we collated statistics from thirteen studies consisting of 787 participants who were participating in social interactions while HRV was simultaneously collected. Results revealed that while dyadic social interactions do not increase HRV generally from a baseline state, negative dyadic…

Heart rate variability and arterial oxygen saturation response during extreme normobaric hypoxia.

The primary purpose of this study was to assess the response of autonomic cardiac activity and changes in the arterial oxygen saturation (SpO2) during normobaric hypoxia and subsequent recovery. Heart rate variability (HRV) and SpO2 were monitored in a supine position during hypoxia (FiO2=9.6%) for 10min, and normoxic recovery in 29 subjects. Spectral analysis of HRV quantified the autonomic cardiac activity by means of low frequency (LF) (0.05-0.15Hz) and high frequency (HF) (0.15-0.50Hz) power transformed by natural logarithm (Ln). Based on the SpO2 response to hypoxia, the subjects were div…

High-Frequency Heart Rate Variability Linked to Affiliation with a New Group.

This study tests the hypothesis that high levels of high-frequency heart rate variability (HF-HRV) predisposes individuals to affiliate with new groups. Resting cardiac physiological recordings were taken before and after experimental sessions to measure trait high-frequency heart rate variability as an index of dispositional autonomic influence on heart rate. Following an experimental manipulation of priming of caring-related words, participants engaged in a minimal group paradigm, in which they imagined being a member of one of two arbitrary groups, allocated money to members of the two grou…

A literature review of heart rate variability in depressive and bipolar disorders.

OBJECTIVE: Autonomic nervous system dysfunction has the potential to adversely impact general medical health and is known to exist in a number of psychiatric disorders. It reflects alterations in the function of several regions of the central nervous system. Measurement of heart rate variability provides a non-invasive tool for studying autonomic function. While the literature relating to the technical process of heart rate variability and aspects of depressive disorders has been reviewed in the past, research relating to both depressive and bipolar disorders has not been comprehensively revie…

Decreased heart rate variability responses during early postoperative mobilization--an observational study.

BACKGROUND: Intact orthostatic blood pressure regulation is essential for early mobilization after surgery. However, postoperative orthostatic hypotension and intolerance (OI) may delay early ambulation. The mechanisms of postoperative OI include impaired vasopressor responses relating to postoperative autonomic dysfunction. Thus, based on a previous study on haemodynamic responses during mobilization before and after elective total hip arthroplasty (THA), we performed secondary analyses of heart rate variability (HRV) and aimed to identify possible abnormal postoperative autonomic responses i…

Heart rate and respiratory response to doxapram in patients with panic disorder.

Panic disorder (PD) is characterized by anticipatory anxiety and panic, both causing physiological arousal. We investigated the differential responses between anticipatory anxiety and panic in PD and healthy controls (HC). Subjects (15 PD and 30 HC) received an injection of a respiratory stimulant, doxapram, with a high rate of producing panic attacks in PD patients, or an injection of saline. PD subjects had significantly higher scores in anxiety and panic symptoms during both conditions. Analysis of heart rate variability (HRV) indices showed higher sympathetic activity (LF) during anticipat…

Short-term heart rate variability in older patients with newly diagnosed depression.

Dysfunction of the autonomic nervous system has been considered to be a risk factor for major depressive disorder (MDD) and cardiovascular disease (CVD). The aim of this study was to evaluate short-term heart rate variability (HRV) in elderly patients with newly diagnosed MDD. Thirty MDD patients over 60 years old newly diagnosed by a structured interview were enrolled, free from antidepressants. Socio-demographic data, blood tests, and heart rate variability (HRV) obtained from 5-min ECG were gathered. The MDD group showed significantly lower very low frequency power, low frequency power, hig…

Selective Heart Rate Reduction Improves Metabolic Syndrome-related Left Ventricular Diastolic Dysfunction.

BACKGROUND: Enhanced heart rate observed in metabolic syndrome (MS) contributes to the deterioration of left ventricular (LV) function via impaired LV filling and relaxation, increased myocardial O2 consumption, and reduced coronary perfusion. However, whether heart rate reduction (HRR) opposes LV dysfunction observed in MS is unknown. METHODS: We assessed in Zucker fa/fa rats, a rat model of MS, the cardiovascular effects of HRR induced by the If current inhibitor S38844 (3 mg · kg(-1) · d(-1)). RESULTS: Delayed short-term (4 days) and long-term (90 days) HRR induced by S38844 reduced LV en…

Heart rate variability during sleep at high altitude: effect of periodic breathing.

BACKGROUND: Heart rate variability (HRV) during sleep in normal subjects at high altitude shows a decrease in parasympathetic tone associated with an increase in the sympathetic one, which tends to be reversed with acclimatization. However, periodic breathing (PB) during sleep may influence this effect detected by HRV spectral analysis. PURPOSE: The aim of our study was to investigate HRV during sleep periodic breathing (PB) at high altitude in normal subjects at two different times of acclimatization, i.e., two different levels of hypoxemia. METHODS: Recordings of six healthy climbers (aged…

Resting high-frequency heart rate variability is related to resting brain perfusion.

We examined the neural correlates of resting cardiac vagal activity in a sample of 432 participants (206 male; 61 African American; mean age 42 years). Pulsed arterial spin labeling was used to quantify whole brain and regional cerebral blood flow at rest. High-frequency heart rate variability (HF-HRV) was used to measure cardiac vagal activity at rest. The primary aim was to determine whether brain regions implicated in regulating cardiac vagal reactions were also related to cardiac vagal activity at rest, and whether these associations varied by sex or race. Brain areas previously related to…

Performance limits of ICA-based heart rate identification techniques in imaging photoplethysmography.

Imaging photoplethysmography is a relatively new technique for extracting biometric information from video images of faces. This is useful in non-invasive monitoring of patients including neonates or the aged, with respect to sudden infant death syndrome, sleep apnoea, pulmonary disease, physical or mental stress and other cardio-vascular conditions. In this paper, we investigate the limits of detection of the heart rate (HR) while reducing the video quality. We compare the performance of three independent component analysis (ICA) methods (JADE, FastICA, RADICAL), autocorrelation with signal c…
