Exertion injuries in adolescent athletes.

A series of 147 cases of exertion injuries in less than or equal to 15 years old athletes is presented. All injuries occurred during training or athletic performances without trauma and caused symptoms that prevented athletic exercises. There were 67 girls (46%) and 80 boys (54%) in the material. About 90% of them had been training for more than one year before the onset of the symptoms; 65% were interested in track and field athletics, 13% in ball games, 11% in skiing, 4% in swimming, and 3% in orienteering. The rest were interested in other sports. About 33% of the injuries were growth distu

Long-term outcomes of traumatic hip dislocation: a minimum 10-year follow-up study in 18 patients.

BACKGROUND: Traumatic hip dislocations are rare but serious injuries, potentially affecting patients' quality of life and mobility. Given the limited understanding of prognostic factors, this study aimed to identify predictors of long-term clinical outcomes. MATERIALS AND METHODS: Injury characteristics and computed tomography (CT) of patients following traumatic hip dislocation from two level I trauma centers from 2009 to 2015 were analyzed. At follow-up, patients were evaluated for avascular necrosis (AVN), post-traumatic osteoarthritis (PTOA), secondary surgery, complications, and return t

The direct posterior gluteal muscle splitting approach for posterior access to acetabular fractures: Surgical technique and case series.

We present a minimally invasive direct posterior, gluteal muscle splitting approach (PMS) as an alternative to the traditional Kocher-Langenbeck (KL) approach for posterior access to acetabular fractures. We believe it offers significant advantages and provides improved access while maximizing the range of fracture patterns that can be addressed through a posterior approach. One hundred and eighty-four consecutive patients treated with this approach by the senior author (RMS) between 2001 and 2018 were reviewed. The most common individual fracture pattern addressed was a posterior wall (66/36%

Proximal Femur Fracture and Vascular Injury in Adults-Case Report.

Vascular complications in the surgical treatment of hip fractures are rare. Depending on the arterial injury, severe intraoperative bleeding or a subacute hematoma formation with arterial pseudoaneurysm development can occur. In the literature, the more frequently described complications are large local hematomas after osteosynthesis with sliding hip screws. This report shows a case of delayed arterial injury after proximal femur osteosynthesis.

Hepatocarcinoma-intestine-pancreas/pancreatitis-associated protein (HIP/PAP) confers protection against hepatic fibrosis through downregulation of transforming growth factor β receptor II.

Hepatocarcinoma-intestine-pancreas/pancreatitis-associated protein (HIP/PAP) has antimicrobial, antioxidant, anti-inflammatory, mitogenic, and antiapoptotic effects and thus exerts important functions in the maintenance of integrity and homeostasis of several organs, such as the gastrointestinal tract, pancreas, and liver. Although the potent hepatoprotective effect of HIP/PAP has been validated, its impact on liver fibrosis has not been reported. In this study, we evaluated the role of HIP/PAP on hepatic fibrosis and explored the possible underlying mechanisms. We found that the expression of

Lower extremity injuries in lateral impact: a retrospective study.

ABSTRACT A retrospective analysis of the NASS/CDS database from 1993 to 2000 was used to investigate lower extremity injury in lateral impact. The analysis includes the study of the injury patterns, crash characteristics and the interactions between the occupant and the vehicle interior, including injuries to the farside occupants. The findings include significantly different injury patterns for the nearside and farside impacts. In particular, while the proportion of pelvis/hip injuries, with respect to AIS2 and AIS3 lower extremity skeletal injuries and 2-4 and 10-8 o'clock side impacts, was …

Is physical activity, practiced as recommended for health benefit, a risk factor for osteoarthritis?

In this critical narrative review, we examine the role of physical activity (PA), recreational and elite sports in the development of knee/hip osteoarthritis (OA), taking into account the role of injury in this relationship. The process of article selection was unsystematic. Articles were selected on the basis of the authors' expertise, self-knowledge, and reflective practice. In the general adult population, self-reported diagnosis of knee/hip OA was not associated with low, moderate or high levels of PA. For studies using radiographic knee/hip OA as a primary outcome, the incidence of asympt

Hip Microinstability: Understanding a Newly Defined Hip Pathology in Young Athletes.

Microinstability is an increasingly recognized diagnosis in young athletes presenting with hip pain. Causes of microinstability may include abnormality of the hip bony anatomy, acetabular labral tears, joint capsule laxity or injury, and muscle dysfunction. Borderline hip dysplasia is an increasingly recognized factor predisposing to microinstability. The capsuloligamentous structures of the hip, particularly the iliofemoral ligament, provide important restraints to femoral head motion, and iatrogenic defects can predispose patients to instability after surgery. Injury to the acetabular labrum

Do hip strength, flexibility and running biomechanics predict dynamic valgus in female recreational runners?

BACKGOUND: Dynamic valgus has been the focus of many studies to identify its association to an increased risk of running-related injuries. However, it is not known which physical and biomechanical variables are associated with this movement dysfunction. RESEARCH QUESTION: This study aimed to test the correlation between strength, flexibility and biomechanical variables and dynamic valgus in female runners. METHODS: Twenty-nine healthy females ran on a treadmill at 2.92 m/s and performed strength, range of motion and endurance tests. Pelvic, hip and ankle kinematics were measured with a 3D mo

Do Mortality and Complication Rates Differ Between Periprosthetic and Native Hip Fractures?

BACKGROUND: Epidemiological estimates indicate a rising incidence of periprosthetic hip fractures. While native hip fractures are known to be a highly morbid condition, a significant body of research has led to improved outcomes and decreased complications following these injuries. Comparatively, little research has evaluated the relative morbidity and mortality of periprosthetic hip fractures. The purpose of this study was to compare the morbidity and mortality of periprosthetic vs native hip fractures. METHODS: Using the National Surgical Quality Improvement Program (NSQIP) database, 523 pe

Impact of Correcting for Pelvic Tilt on the Assessment of Concentric Hip Extension Torque: Validity and Reliability of Isokinetic Dynamometry.

A low hip extensor strength has been associated with various musculoskeletal injuries and an impaired motor performance. Hip strength is often assessed by using an isokinetic dynamometer. However, the validity and reliability of such testing have not been examined comprehensively. In this study, 14 participants performed maximal concentric contractions on a dynamometer in supine position at 60° s-1, 120° s-1, 180° s-1, and 350° s-1 on two different days to assess inter-day reliability. Hip angle and angular velocity were determined using 3-D motion analysis (3-D MOCAP) and were compared to the

The role of the iliofemoral ligament as a stabilizer of the hip joint.

The purpose of this systematic literature review is to analyse the role of the iliofemoral ligament (ILFL) as a hip joint stabilizer in the current literature.A total of 26 articles were included in the review. The ILFL is the largest hip ligament consisting of two distinct arms and is highly variable, both in its location and overall size, and plays a primary role in hip stability; in the case of hip dislocation, the iliofemoral ligament tear does not heal, resulting in a persistent anterior capsule defect. Clinically, the ILFL is felt to limit external rotation in flexion and both internal a

Injuries associated with femoral shaft fractures with special emphasis on occult injuries.

BACKGROUND: Fractures of the femoral shaft are mostly the result of high-energy accidents that also cause multiple trauma injuries, in particular ipsilateral knee and hip injuries. The purpose of this study was to investigate the incidence of injuries associated with femoral shaft fractures and how many of them were undetected. METHODS: We studied 148 patients (150 femoral shaft fractures) with an average age of 52 (range: 18-97). Femoral shaft fractures were treated with antegrade intramedullary nailing in 118 cases (78.7%), and with open reduction and internal fixation in 32 cases (21.3%). …

Editorial Commentary: Long-Term Follow-Up After Endoscopic Gluteal Repair Plus Hip Arthroscopy Shows Durable Results Using Validated Patient-Reported Outcome Scores That Largely Exceed the Minimal Clinically Important Difference and Patient Acceptable Symptom State.

Gluteus medius and minimus tendon pathology is a common cause of lateral hip pain. In patients who are dissatisfied with their hip condition following nonsurgical treatment, gluteal repair has demonstrated excellent short-, mid-, and, now recently, long-term subjective patient-reported and objective clinician-measured outcomes. In patients with peritrochanteric hip pain, the proportion of their overall hip pain may be influenced by the hip joint due to conditions like femoroacetabular impingement syndrome, acetabular dysplasia, labral tears, and arthritis. Thus, surgical decision-making must i

Effects of a Powered Knee-Ankle Prosthesis on Amputee Hip Compensations: A Case Series.

Transfemoral amputee gait often exhibits compensations due to the lack of ankle push-off power and control over swing foot position using passive prostheses. Powered prostheses can restore this functionality, but their effects on compensatory behaviors, specifically at the residual hip, are not well understood. This paper investigates residual hip compensations through walking experiments with three transfemoral amputees using a low-impedance powered knee-ankle prosthesis compared to their day-to-day passive prosthesis. The powered prosthesis used impedance control during stance for compliant

Classic measures of hip dysplasia do not correlate with three-dimensional computer tomographic measures and indices.

UNLABELLED: Acetabular dysplasia is a precursor to osteoarthritis of the hip, and it causes acute and degenerative injuries of soft tissue stabilizers. Traditional radiographic assessments of dysplasia are useful in moderate and severe dysplasia, but they have questionable reliability in mild dysplasia. Computed tomography (CT) reconstruction provides a method for calculation of acetabular geometry and analysis of existing radiographic methods. We performed a retrospective radiographic review of anteroposterior pelvic films and their corresponding pelvic CT scans. Using 30 skeletally mature pa
